• Free personalized assistance by professional Quit Coaches

• A series of up to five scheduled telephone sessions - with additional calls as needed

• HealthChoice members are eligible for up to 12 weeks of patches, gum, or lozenges via mail order

• Trained in well-researched coaching skills

• 240 hours of cessation training

• Educated up to graduate level

• Over 50% with 3+ years experience in counseling

Three-Pronged Approach

• Physical addiction is treated with medication

where possible

• Psychological dependency is treated with

cognitive behavioral coaching

To Enroll

• Contact the Oklahoma Tobacco Helpline at 1-800-QUIT-NOW

• Identify yourself as a HealthChoice member

• Members living outside of Oklahoma call 1-866-QUIT-4-LIFE (1-866-784-8454)

To Meet Attestation Requirements

• Enroll and complete a minimum of three calls with a Quit Coach

• Complete the program prior to the deadline within the calendar year of your Option Period
