TERMS OF REFERENCE OF MANAGER (HR) Background: The Government of Karnataka has created Suvarna Arogya Suraksha Trust as a special purpose vehicle to provide cashless treatment (tertiary) to the needy citizens of Karnataka. At present, it is implementing Vajpayee Arogyashree Scheme to BPL population. TERMS OF REFERENCE OF

MANAGER (HR)

Background:

The Government of Karnataka has created Suvarna Arogya Suraksha Trust as a

special purpose vehicle to provide cashless treatment (tertiary) to the needy citizens of

Karnataka. At present, it is implementing Vajpayee Arogyashree Scheme to BPL

population. It is also planning to introduce Rajiv Arogya Bhagya, Jyothi Sanjeevini and

Mukyamatrigala Santwana Scheme to cover the other sections of Karnataka.

Medical aid to the eligible beneficiaries is provided through empanelled

Network Hospitals, including private hospitals. 451 benefit packages and 50 follow up

packages are developed to treat seven types of terminal ailments.

In this context, Suvarna Arogya Suraksha Trust proposes to engage Manager

(HR) to support the Suvarna Arogya Suraksha Trust in enabling the entire scope of HR

which includes recruitment & selection, training, employees performance appraisal,

employee retention and liaison with Government officials etc and also to facilitate

real-time communication between providers, ISA & the Trust.

I. Objectives of the Manager (HR) :

1. To interact with the Trust, ISA and other services provider under VAS

Scheme.

2. Ability to work independently & effectively with Trust.

3. To develop collaborate relationship with Trust, ISA & Network Hospital etc.

II. Outlines of the Tasks to be carried –

1. To Frame a clear and easily implementable HR Policies

2. Drive strategic work force planning

3. To nurture and develop a motivated HR Team to meet Trust requirement.

4. Execute manpower planning and budgeting to have a road-map for recruitment

assignments

5. Manpower hiring as per the recruitment plan agreed along with the

Department heads.

6. To provide employee development and counselling assistance to employees

/team members to enhance employee performance and productivity

7. To take adequate measures to retain good employees, ring fence high potential

employees

8. To identify more efficient ways of working and foster innovation

9. To identify potential costs savings by means of greater use of shared services

and outsourcing

10. Administration activities like housekeeping, travel & ticketing, looking after

day to day office activities, attendance records, recruitment process, salary

increment, preparing Official Memorandum for appointment and various other

letters , studying performance of the employees .

11. To maintain all establishment files.

12. Movement of salary files of Officers and Staff.

13. To process all bills relating to rent, electricity, telephone and water for payment.

14. Reply to RTI.

15. To purchase stationery.

16. To maintain personal files of all the Officers.

17. To prepare Training Programmes monthly /quarterly.

18. To assess and plan the Trainings.

19. To prepare scheduling of training sessions and booking / notifying of all

relevant stake holders.

20. Taking responsibility for ensuring all training rooms / venues, equipment,

catering arrangements and other requirements are booked up or canceled in

advance.

21. Develop and organize training manuals, multimedia visual aids and other

educational materials.

22. Sending out appropriate course materials to all participants in advance to

training courses.

23. Keeping data of training materials up to date.

24. To ensure that course contents are obtained in advance.

25. To ensure that printing of training materials for training courses are done in

advance.

26. To maintain all staff’s training attendance.

27. To keep training records and files up to date.

28. To prepare training budget and to take prior approval.

29. To co-ordinate with Resource Person for conducting Training.

30. To co-ordinate with all trainings organized by World Bank and other

institutions.

31. To co-ordinate with Audit Team during Audit process and support them with

necessary details and documents (related to all Trainings).

32. Any other work entrusted by the Executive Director.

III. III. Reports to be furnished: Reports as requested by Executive Director.

V. IV. Period of Consultancy - Period of contract is for 1 year extendable on satisfactory

performance.

VI. V. Data, Input to be provided by the Trust: Office Accommodation, Computer,

Telephone, Sim Card with Fax etc. will be provided at SAST Office.

VI. Remuneration - Rs. 40,000/- per month.

VII. Review - The performance will be reviewed by the Executive Director, Suvarna

Arogya Suraksha Trust, Bangalore.

VIII. Liability Clause: The incumbent is liable for inaccuracies, frauds,

misappropriations, willful negligence and any other act resulting in loss to the

Trust will attract the provisions of Indian Criminal Procedure Code and is

subjected to criminal prosecution.

IX. Mutual Notice: In the event of default, the Trust may terminate this Agreement by

issuing 1 month termination notice setting out the underlying event of default and

termination date. The Consultant may also terminate this Agreement with prior

notice of 2 months or by paying 2 month Salary to the Trust.

CO-ORDINATOR (QUALITY ASSURANCE)

Background:

The Government of Karnataka has created Suvarna Arogya Suraksha Trust as a

Suvarna Arogya Suraksha Trust proposes to engage a Consultant for Quality

Assurance to support the Vajpayee Arogyashree Scheme in enabling real-time

communication between providers, the insurers & the Trust and also to measure the

patient satisfaction.

Quality assurance defines quality assurance as all activities that contribute to

defining designing, assessing, monitoring and improving the quality of health care.

Quality assurance ensures that all processes work effectively and synchronously to

achieve quality health care.

Objectives of the Co-ordinator:

1. To assist the Executive Director in designing and managing the Activities.

2. To interact with the ISA, Network Hospitals and beneficiaries about the services

provided under VAS Scheme for utilization.

3. To provide career tracks who manage and direct quality assurance, health care

compliance and utilization review program operations. The work includes

development, implementation and evaluation of operational goals, objectives,

policies and procedures.

4. To develop collaborate relationship with Trust, ISA, Network Hospital &

Beneficiaries.

5. To develop and administer the quality education process formulating quality

improvement programmes for employees.

Outlines of the Tasks to be carried –

1. To interpret and implement quality assurance standards in order to ensure

quality care to patients.

2. To monitor and ensure that there is hospital-wide compliance with laws, policies,

and standards for quality of patient care and facility licensure and/or

accreditation.

3. To review quality assurance standards, study existing hospital policies and

procedures and interview hospital personnel and patients to evaluate

effectiveness of quality assurance program.

4. To recommend and support implementation of plans for improving quality and

patient safety aspects to be included in the Scheme, not limited to.

5. Feasible quality and patient safety criteria that could be required as a minimum

for the empanelment of health facilities, and any suggestions for future staging

thereof to move them increasingly towards National Accreditation Standards.

Detailed definitions and verification mechanisms for each criteria to be included.

6. Minimum reporting requirements on quality and patient safety criteria for

empanelled health services providers as feasible in the present context, and any

suggestions for future staging thereof. Detailed definitions and verification

mechanisms for each reporting requirement to be included.

7. Detailed audit methodology for Trust/ISA to verify compliance to suggested

criteria, where needed, as well as for calculating and reporting the suggested

quality indicators.

8. Identify the procedures and consumable for which minimum quality criteria are

available or need to be worked on by the State.

9. Feasibility of incentive mechanisms such as differential pricing for NABH

accredited providers, or for reporting of quality data.

10. Any other mechanism to improve the quality and patient safety standards for the

empanelled providers which would be consistent with program design and

guidelines and feasible in the current context.

11. To review and evaluate patients medical records, applying quality assurance

criteria.

12. To select specific topics for review, such as problem procedures, drugs, high

volume cases, high-risk cases or other factors.

13. To utilise the principles of quality assurance hospital audit, investigate and

inspect records or facilities to assure quality patient care and identify

discrepancies in eligibility determinations.

14. To plan and administer all aspects of quality assurance, health care compliance,

and utilize review programs; or are licensed health care professionals conducting

inspections of service delivery and service providers.

15. To modify quality assurance plans as required in response to observations made

and data prepared.

16. To implement health care service plans.

17. To approve change controls and deviation reports and recommend corrective

action as required.

18. To provide data analysis, trending, reporting and presentation on individual and

departmental statistics as related to areas requiring improvement.

19. To define and make explicit the rights of health service users and healthcare

providers.

20. To establish a system of follow-up and response to complaints and suggestions.

21. To establish systems to recognize good performance.

22. To develop an information system to link productivity and costs.

23. To promote the certification of healthcare providers and facilities.

24. To strengthen medical arbitration to resolve conflicts between patients and

service providers.

25. To compile statistical data and to write narrative reports summarizing quality

assurance findings.

26. To monitor unusual occurrences, report follow-up procedures and report

monthly comparisons.

27. To review quality assurance standards, study existing policies and procedures to

evaluate effectiveness of quality assurance program.

28. To perform quality-assurance function to accomplish business coordination,

monitoring and reporting of quality-assurance studies according to the quality

assurance plan.

29. Responsible for knowing current Quality Assurance regulations and informing

the Executive Director of any new and/or revised regulations imposed.

30. To selects specific topics for review such as problem procedures, drugs, high

31. To assists Trust with the coordination of audit information and recommends

appropriate data-gathering mechanisms, procedures, etc.

32. To assist in developing and maintaining an unusual occurrence procedure.

completeness and compliance to requirements to ensure that quality assurance

standards and regulatory requirements are met.

34. To keep the Executive Director informed of studies in process and progress

thereof, discusses problems and completion of audit procedures.

35. To maintain current and accurate records of all relevant communications, audits,

corrective action plans, and effectiveness monitoring.

36. Any other work entrusted by the Trust.

Reports to be furnished by the consultant

(a) Monthly progress reports.

(b) Statistical data along with narrative reports summarizing quality assurance

findings.

Period of Consultancy - Period of contract is for 1 year extendable on satisfactory

performance.

Data, Input to be provided by the Trust:

Office Accommodation, Computer, Telephone with Fax etc. will be provided at

SAST Office.

Remuneration - Rs. 35,000/- per month.

Leave Facility – Can avail 12 Casual Leave & 10 Medical Leave.

Review - The performance of Consultant will be reviewed by the Executive Director,

Suvarna Arogya Suraksha Trust, Bangalore.

CONSULTANT FOR MONITORING THE ACTIVITIES OF GULBARGA DIVISION

(INDIVIDUAL CONSULTANT)

Background

The Government of Karnataka has created Suvarna Arogya Suraksha Trust as a

special purpose vehicle to provide cashless treatment (tertiary) to the needy citizens of

Karnataka. At present, it is implementing Vajpayee Arogyashree Scheme to BPL

population. It is also planning to introduce Rajiv Arogya Bhagya, Jyothi Sanjeevini and

Mukyamatrigala Santwana Scheme to cover the other sections of Karnataka.

Medical aid to the eligible beneficiaries is provided through empanelled

In this regard, Suvarna Arogya Suraksha Trust proposes to engage a Regional

Consultant for monitoring the activities of Gulbarga Division and to support the

Vajpayee Arogyashree Scheme in enabling real-time communication between

providers, the insurers & the Trust and also to measure the patient satisfaction.

1. Objectives of the Consultant:

(a) To assist the Executive Director in designing and managing the Activities.

(b) To monitor the VAS Health Camps conducted to the people in designated areas who

lack access to such services on various grounds.

(c) To monitor the activities of all the empanelled Network Hospitals of the concerned

districts.

(d) To monitor the activities of VAMCO

(e) To monitor the activities of VAMCCO

(f) To monitor the roles and responsibilities of DHO & THO

(g) To monitor the activities of District Co-ordinators.

(h) To monitor the activities of Arogyamitras.

2. Outlines of the Tasks to be carried –

(a) To monitor the duties and responsibilities of VAMCO which is as follows:

1. To guide the patient in all aspects.

2. He shall ensure that all required evaluation including diagnostic tests are done

free of cost for all beneficiaries and the details of the same along with reports are

captured in the Trust portal.

3. He shall upload the OP/IP status of the patient.

4. He shall sign the investigation request.

5. He shall cross check whether diagnosis is covered under the Scheme. If doubtful

about the plan of management, then should coordinate with treating specialist

along with Package list.

6. He should facilitate the admission process of patients without any delay.

7. After admission, he shall collect all the necessary investigation reports before

sending for approval.

8. He shall upload the admission notes and preoperative clinical notes of the

patient.

9. He shall ensure that preauthorization request is sent only for patients who are

admitted.

10. He shall ensure before sending Preauthorization that all essential documents like

BPL card, Patient photo and also necessary reports as per the standard protocol,

like CT Films, X-Ray films, Angio CD etc., are uploaded in the system.

11. He shall coordinate with Trust doctors as need arises.

12. Pre-authorizations kept pending from Trust shall be verified on a regular basis

and necessary corrections to be done by VAMCO.

13. He shall furnish daily clinical notes (Pre-Operative and Post-Operative).

14. He shall upload 3 Photographs of the Patient taken preoperative bedside,

immediate Post-operative showing operation wound and at the time of

discharge.

15. He shall update surgery and discharge details and hand over signed copy of the

Summary along with follow-up advice in pre-printed stationary.

16. He shall ensure free follow–up consultations, routine investigations and

distribution of drugs to be supplied by the NWH to the beneficiaries.

17. He shall ensure to update the details of on-bed status of patients time to time as

per the format on the display board placed at the Arogyamitra Kiosk / reception

desk.

(b) To monitor the activities of Network Hospitals that:

1. The Network Hospital had established Vajpayee Arogyashree Assistance

Counter / Kiosk at the reception of the hospital free of cost.

2. The Network hospital shall provide the following infrastructure and network

facility to the counter: P.C, Printer, Scanner, Digital camera, Webcam, Barcode

reader, Mike, Speakers, Stationary etc. The system and other peripherals should

be provided exclusively for the use of Arogyamitra who can use the resources at

any point of time.

3. The Network Hospital shall provide a dedicated 2 Mbps broadband connectivity

to the computer to be exclusively used by the Arogyamitra to access the web for

online MIS, e-preauthorization etc.

4. The Network Hospital shall allow Arogyamitra access to the wards and patient’s

data to facilitate onward transmission to the ISA for preauthorization, claims,

correct MIS etc.

5. The Network hospital shall update the details like date of surgery, discharge of

the beneficiary, etc. in the Trust portal.

6. The Network hospital shall intimate Arogyamitra and VAMCO regarding

emergency admissions of the Beneficiary during non office hours.

7. The Network Hospital agrees not to deny admission for the beneficiary for want

of preauthorization.

8. The Network Hospital shall agree to provide a separate ward for Vajpayee

Arogyashree Beneficiaries.

9. The Network Hospital agrees to provide separate operation theatre days and

weekly schedules for the surgeries/therapies to be performed for the

Beneficiaries.

10. The Network Hospital agrees to handle a minimum number of cases in each

specialty including trauma cases based on their available infrastructure.

11. The Network Hospital agrees to submit the vacancy level in pre-operative wards,

ICU, Post operative wards and also upload the same in the Trust portal on a

daily basis.

(c) To Monitor the activities of VAS Health Camp:

1. The Network Hospital is conducting free medical camps at least once in a month

at the place specified by the Trust to identify the members of the BPL families

who may require surgeries covered under the Scheme as per the schedule given

by the Trust for such surgeries. The Health Camp Guidelines should be

scrupulously followed.

2. The Network Hospital shall carry necessary diagnostic equipment such as ECG,

Echo, Ultrasound etc. to these free medical camps.

3. The Network Hospital shall provide services of concerned specialists namely

Cardiologists, CT Surgeon, Neurosurgeons, Urologists, Oncologists,

Gynecologists, Plastic Surgeon, Pediatric Surgeon, General Physicians at the

camp to facilitate proper evaluation of the patients.

4. The Network Hospital shall submit the camp confirmation and indent online as

given in camp guidelines in the prescribed format to Trust at least one week in

advance of the stipulated date.

5. The Network Hospital shall inform all the stakeholders such as District

Administration, concerned public representatives, PHC/TH/ DH staff etc well in

advance for successful conduct of the camp.

6. The Network Hospital shall spread awareness about the camp through Publicity

in coordination with District coordinator, Regional coordinator, PHC staff and

Arogyamitras.

7. The Network Hospital Provider shall provide patient data to Trust in the

prescribed form at the end of the camp.

8. The Network Hospital shall enter the details of the patients screened and

referred at the camps on the Trust portal on the same day of the Camp.

9. The Network Hospital shall coordinate constantly with the Medical Camps Cell

of the Trust in all matters related to Medical Camps.

10. The patients referred from the camp shall be followed up and transported to the

Hospital within 10 days of the camp unless the patient is not willing, in which

case the same should be recorded and updated on the Trust Portal.

11. The Network Hospital shall have an Officer, designated as Vajpayee

Arogyashree Medical Camp Coordinator (VAMCCO), for the Scheme to

coordinate with Trust through Arogyamitras.

12. The Network Hospital should promptly inform the Trust about changes, if any,

in the VAMCCO designated during the tenure of the agreement.

13. The Network Hospital shall give the full-time services of VAMCCO to

coordinate all activities related to camps and patient follow-up from camps.

(d) To Monitor the duties and responsibilities of Vajpayee Arogyashree Medical

Camp Coordinator (VAMCCO) which is as follows:

1. Confirmation of camps online and indenting online.

2. Carrying out the IEC activities within camp area at least 7 days before the camp

date.

3. Providing facilities like shamianas, chairs, screening enclosures.

4. Providing common medicines in the camps.

5. Arranging Health Education Exhibits and Pamphlets etc., in the camp.

6. Arrange for distribution of incentives to the medical officers.

7. Coordinating and ensuring participation of specialists.

8. Arranging the diagnostic equipment

9. Coordinate with PHC doctors/Government Doctors, Public Representatives,

SHG groups and Local Administration.

10. Raising claims online for the camps conducted.

11. Follow – up of patients referred from Camps

12. Any other responsibilities assigned by the Trust

13. The VAMCO & Arogyamitra should counsel the patient for all the precautions

to be taken for the post-operative care.

14. All patients who require follow-up medicines will be advised by the Provider to

come back on 11th day of discharge for first mandatory follow-up. The date of

first follow-up will be generated by the Trust Portal along with the Discharge

Summary.

15. The subsequent follow-ups for the above cases will be as per the follow-up

guidelines.

16. Provider shall agree to provide follow-up services for a period of ONE YEAR

under the Scheme.

17. Provider shall agree to provide free post surgical physiotherapy services,

wherever required during the tenure of the agreement.

18. Facilities available at the Camp:

Network Hospital personnel, i.e. VAMCCO, shall asses the capacity of the

available facilities in the designated location of the camp in advance and seek

efforts to provide additional facilities for the comfort of beneficiaries such as:

Providing shade for waiting patients by erecting shamianas.

Providing pedestal fans.

Sitting arrangements for waiting patients by providing sufficient number of

chairs.

Drinking water for patients

Screening enclosures for patient

Snacks

Any other activity chosen by the hospital

Organising the camps: The hospital shall conduct the camp as per the schedule

and by undertaking the activities as given in the indent. The hospitals shall

ensure that Vajpayee Arogyashree Medical Camp Coordinator (VAMCCO) is

earmarked for the purpose and is sent at least a week in advance to the camp

area to undertake IEC activities as planned and arrange for the facilities to be

provided for the camps. The hospital camp coordinator shall ensure that the

schedule of the camp is informed to all concerned stakeholders in the local area

of the camp including the people’s representatives.

The following documentation have to be completed during the camp:

Each patient has to be given an outpatient-cum-prescription card. The

details of medicines to be disbursed shall be mentioned in this card.

Those patients who are treated as outpatients shall be given medicines as

prescribed in the outpatient-cum-prescription card. The Provider shall

maintain a drug dispensing register for maintaining record of drugs

disbursed at the camp and shall obtain the signature/ thumb impression of

the patient on the register for being disbursed the medicines to the

concerned patient. The same shall be scanned and uploaded at the time of

claiming the camp expenditure.

Those patients who are referred shall be given Vajpayee Arogyashree

referral card with details of date of reporting to the hospital, name of

consultant and mobile number of Network Hospital Arogyamitra.

The details of all outpatients and referred patients will be recorded by the

Arogyamitras in the camp register in triplicate. A copy of the same duly

signed by the government Medical Officer, Arogyamitras and Network

hospital doctor shall be kept with the Network Hospital and the same

shall be scanned and uploaded/sent at the time of claim of camp amount.

At the end of the camp the incentive shall be given to the participating

Government Medical Officers and the receipt shall be obtained as per the

prescribed format.

The Vajpayee Arogyashree Medical Camp Coordinator (VAMCCO) of the

Network hospital shall also take a declaration for being conducted the

health camp successfully from the concerned Medical Officer of the PHC.

The Arogyamitra of the PHC/CHC shall also sign the same. The Network

Hospital shall upload/send the same at the time of claiming the camp

expenditure.

(e) To Monitor the role of District Administration in conducting the Camps:

1. Spreading awareness on the camps through all possible means.

2. All public representatives including Ministers, MPs, MLAs, MLCs, ZP

chairperson, and all people’s representatives may be informed of camp schedule

in advance so that they may participate in medical camps. The camp can be

inaugurated by local MLA or by other people’s representative depending on

availability. Some time the camp can be earmarked for inauguration.

3. Drinking water for patients may be arranged through Panchayath Secretaries

and Arogyamitras.

4. Minimum facilities like drinking water and food for doctors and staff

participating in the camp may be arranged through the Arogyamitras or the local

Medical Officers. The cost of this will be borne by the Network Hospital.

5. Medical Officers in the nearby PHCs and government hospitals shall attend

camps for general check-up. In camps where only one network hospital is

participating two medical officers shall be deputed and where two network

hospitals are participating four medical officers shall be deputed.

6. The patients referred from the camps shall be followed up to report to the

network hospital. The Arogyamitras (minimum two per camp if one network

hospital is participating and four per camp if two network hospitals are

participating) shall ensure referral cards are given to the referred patients in the

camp with details of date of reporting to the hospital, place of appointment,

name of consultant and mobile number of Network Hospital Arogyamitras.

7. The District Coordinator of the ISA shall speak to the VAMCCO of the Network

Hospital and ensure that all the activities mentioned above are taking place in

time and as planned. The details of Medical Officers and Arogyamitras

participated in the camps shall be given to the VAMCCO.

(f) To monitor the roles and responsibilities of the local health care officials:

DHO:

Shall be overall supervising officer of health camp activities in the district.

Shall provide support to the Network Hospital and the staff of the Health

Camp to carryout effective services in the area.

Shall be responsible for monitoring and evaluation of the Health Camp as

and when required.

Shall be responsible to take appropriate action in case of any medical

emergencies and out breaks in the area of operation.

Shall be free to call any person from the Network Hospital for taluka level

or district level periodical meetings, without jeopardizing the functioning

of the Health Camps.

Shall collect the progress report in a prescribed format periodically,

incorporating it in the district report.

THO:

Shall be monitoring officer for Health Camp, operating in the Taluka.

Shall coordinate with the DHO for requirements of the camp.

Shall be responsible to take appropriate action in case of emergencies, out

breaks in designated area

Shall be responsible to check the qualifications of the staff as and when

required.

PHC Medical Officer:

Shall assist the Network Hospital in conducting proper screening of

patients.

Shall ensure proper treatment is administered to patients requiring

primary health care services.

He/she shall be responsible for highlighting any health status trends of the

local population and their specific needs.

Medical Officer of the concerned PHC shall also monitor the regularity

and quality of services rendered by the Health Camp.

Shall also deploy any staff of his PHC to take up IEC activities in the area

along with IEC materials.

Spread awareness of the Health Camps among the population of the

Health Camp area.

Shall furnish captured data on the camp to the district administration and

Trust as and when required.

Can promote other health programmes and health education activities,

etc. at the Health Camp

Monitoring and Evaluation:

The Health Camps shall be monitored by concerned Taluka Health Officer,

Medical Officers and other departmental officers periodically. The outcome

indicators will be analyzed from time to time by the Trust.

(g) To monitor the roles and responsibilities of Network Hospital Arogyamitras:

1. He/She should deal with the patients in a friendly and pleasant manner.

2. He/She should be in apron during duty hours and strictly adhere to duty timings

3. He/She should show patience and empathy while dealing with patients.

4. He/She should always keep the CUG switched on round the clock and should

attend to all the Incoming calls politely.

5. He/She should bring to the notice of their superiors any irregularity or

inadequacy noticed.

6. Maintain Help Desk at reception of the Hospital.

7. He/She should receive the patients, verify the documents, register them and

direct them to the VAMCO (Vajpayee Arogyashree Medical Coordinator) for

further screening and management.

8. Facilitate the patient for a cashless transaction.

9. Obtain photograph of the patient - bedside.

10. Facilitate early evaluation and prevent delay in approvals by submitting the

preauthorization complete in all respects.

11. Should ensure that patient is on bed from the time of pre-authorization request is

sent till the approval is obtained.

12. He/She should do regular rounds in the wards and ensure that the patient is

getting all the benefits of the Vajpayee Arogyashree.

13. Ensure that Hospital is giving free Quality Food to all the beneficiaries who are

In-Patients.

14. Facilitate the patient to get the transport charges reimbursed and to ascertain

whether follow-up Medicines are served as per the guidelines.

15. At the time of discharge, he/she should take a photograph of the patient standing

in front of the Vajpayee Arogyashree KIOSK besides the Arogyamitras with

Discharge Summary in one hand and transportation amount and slip in the other

hand and upload the same in the website.

16. Obtain feed back from the patient.

17. Counsel the patient regarding follow-up.

18. Coordinate with PHC/Government Hospital Arogyamitra for follow-up of

beneficiaries.

19. Coordinate with the Trust and ISA officers for any clarifications.

20. He/she should inform the Call-Center immediately in case of a Death and do

send the Death Reports.

21. Send Daily reports as per the formats given by the Head Office to the Call-Center

and to the Team Leader/Co-coordinator.

22. Facilitate Network Hospital in conducting Health Camps as scheduled.

23. Facilitate Network Hospital in sending claims.

24. There should be clear communication between Night shift and Day shift

Arogyamitras and handing over the Duty rooster must be smooth and complete.

25. All Grievances should immediately be brought in to the notice of Grievance

Department directly or through Hierarchy.

26. Arogyamitra should be un-biased while judging the 48 Hrs deadline given for

the patient in furnishing BPL Card at Vajpayee Arogyashree Counter to avail

Scheme benefits. The Arogyamitra shall help the patient in producing the BPL

card before the deadline.

27. He/She should facilitate the hospital in giving prior phone intimation to the Trust

for carrying out the emergency surgeries.

28. He/She should ensure that all the updating like surgery, post-operative notes and

discharge details are done in time on the website.

3. Reports to be furnished by the consultant

(a) Monthly progress reports relating to monitoring activities at field level,

Arogyamitras, Network Hospitals, IEC Activities and District Administrator.

(b) Annual Reports at the end of year including the analytical report of various

monitoring reports at different levels.

(c) Tentative tour programme to be sent before 25th of every month to the Trust

for approval.

(d) Tour dairy to be submitted by 5th of succeeding month.

(e) To take efforts to improve active involvement of Network Hospital.

(f) Any other work entrusted by the Trust.

4. Period of Consultancy - Period of contract is for 1 year extendable on satisfactory

performance.

5. Data, Input to be provided by the Trust:

Office Accommodation will be provided at DC Office of concerned district.

6. Remuneration - Rs. 50,000/- per month & vehicle on hire basis as per the Terms

and Conditions of Government regarding Hiring of Vehicle.

The performance of Consultant will be reviewed by the Executive Director,

Suvarna Arogya Suraksha Trust, Bangalore.

8. Mutual Notice: In the event of default, the Trust may terminate this Agreement by

issuing 1 month termination notice setting out the underlying event of default and

termination date. The Consultant may also terminate this Agreement with prior

notice of 2 months or by paying 2 month Salary to the Trust.
