Position Summary The Team Leader, Carer Support - Aged has a pivotal role in the provision and development of services to carers of people who are frail aged or have dementia, and in supervising team members to ensure that the needs of carers are met in a timely and responsive manner. The Team Leader will develop positive relationships with key stakeholders and contribute to the development, coordination and promotion of services for families and carers of older people in the Northern Metropolitan Region of Melbourne.
Organisational Accountabilities
Merri Community Health Services (MCHS) provides a range of services including dental, allied health disciplines, generalist and specialist counselling programs, aged and disability services, case management, family support, carer support programs, and social support programs for people with a mental illness as well as health promotion programs. Service Development
Coordinate annual plans, including promotional and community development activities
Maintain positive relationships with key stakeholders including relevant government services and service providers as required
Actively contribute to the development of ongoing regional networks
Participate in the development and monitoring of aged respite and carer support programs
Assist with the development and/or review of relevant program procedures
Contract Management
Actively promote effective relationships with key outsourced service providers, and ensure compliance with Community Care Common Standards, best practice, and funding and service agreements
Provide monthly reports to the Manager on service activity and funding obligations
Monitor monthly budgets especially brokerage expenditure, and provide regular reports to the Manager
Actively coordinate and monitor outsourced aged care and dementia services
Provide ongoing evaluation of aged and dementia services to ensure cost effectiveness

Carer Support Program, Dementia Education and Training for Carers Program (DETC), Consumer Directed Respite Care Program (CDRC)
Monitor waitlists and coordinate the allocation of cases to team members
Oversee the use of brokerage in purchasing and delivering respite and support services to carers
Provide timely and professional support to carers by prioritising referrals to the service
Respond to complex carer issues including complaints and grievances
Supervision and Support of Staff
Coordinate recruitment of new staff Provide professional and line-management supervision to staff
including induction and orientation of new staff Conduct annual staff appraisals and the development of
workplans with team members Develop and implement training plans for staff Ensure staff meet all targets as per funding and service
agreements through active monitoring and problem solving
Other Duties Ensure team members comply with data collection requirements of the service and funding bodies.
Contribute to other relevant MCHS activities as directed by the CEO through the Manager CarerLinks North
Undertake regular supervision with line manager Undertake professional development in accordance with an
annual workplan developed in consultation with the Manager CarerLinks North as part of the annual staff review process
Demonstrated commitment to Occupational Health and Safety policies.

Key Selection Criteria
Essential Tertiary qualification in Social Work or equivalent Relevant experience and/or knowledge of the aged sector Relevant experience and capacity to provide high level support
and supervision to staff Relevant experience in delivering respite and carer support

High level skills including: o Client service skills particularly in dealing
with internal and external clients from diverse backgrounds and with special needs

o Experience in contract management
Desirable Qualification in management A relevant second language Victorian Drivers Licence
Training, Qualifications and Registrations
Tertiary qualification in Social Work or equivalent Note: Appointment to this position is subject to passing a National Police Check
