He pulled it out of a well-maintained cardboard file which contained 13 years worth of rigor- ously recorded capillary glucose values. Extreme care had been taken to calli- graphically record three glucose values every day, with notes on activities, the date when a new test strip box was opened, and even the corresponding lunar cycle. He was questioned about the four-figure values jotted near the insulin injections and self-monitoring of blood glucose (SMBG) values. Apparently, he had been accounting for every insulin injection and lancet use that he had done to date! As the monitoring sheet showed, he was on the 8025 th finger prick and the 8926 th insulin injection of his life. Not sur- prisingly, he worked as an accountant. Surprisingly, his HbA1c was 9%! Why then, this paradox of poor control in the face of motivated self-monitoring? SMBG is an important aspect of diabetes self-care and is crucial in patients with poor glycaemic control to help titrate ther- apy. It constitutes one aspect of ‘patient empowerment’ which features highly on the list of desirables in the National Service Framework standards for dia- betes. 1 Yet, anyone involved in caring for the diabetic patient is well aware of the dif- ficulty in getting patients to do regular if not intensive monitoring. Often it is the worried well who do monitoring with its financial implications. Motivating factors for patients to monitor their capillary glucose vary. For some it may be the fear of complications, for some the need for better quality of life or the need to be in control, and for others a need to conform to standards expected by the treating physician or to avoid the berating session at the diabetes clinic. The neat and extremely methodical recording hints at an obsessive compulsive streak in our patient. The monitoring exercise seemed to be an end in itself, giving him a feeling of control and a false sense of security. Concern about the recorded high values might have produced further anxieties, with or without self-blame, as well as attempts to improve glycaemic control although studies show that this is not always the case. 2,3 Rigorous monitoring that does not facilitate better control despite all efforts can be exasperating for patients. Thankfully, in our patient the obsessive monitoring was producing no recognisable detriment to mental health. He seemed to have a good understanding of target values, but no desire to achieve them. Motivation will not translate into productive changes unless all aspects of empowerment are addressed. Many fac- tors may prevent a proactive approach to recorded values including patients’ health beliefs, false perceptions of current con- trol, prevailing state of asymptomaticity, an inability or unwillingness to prioritise, fear of hypoglycaemia or weight gain, fear of generating further anxiety, denial etc. Often, even when intensive monitor- ing is done, lack of collateral data makes interpretation impossible. Our patient’s detailed charting had been offering opportunities to improve control at every visit and perhaps even allow further research into glycaemic excursions and lunar cycles! On the one hand there is the guilt-ridden patient who is embarrassed at not having taken the effort to monitor, and on the other there is the beaming chap waiting expectantly for the pat on the back for having monitored and recorded diligently despite it not having made the slightest difference to his HbA1c. It is perhaps more painful for the health care worker to see the latter, with poor glycaemic control despite excellent monitoring rather than due to lack of it. Good intentions are a start but, unless they are followed through, the numbers just won’t add up despite the best of accounting. A desire in the patient to achieve target values needs to be gener- ated to ensure translation of the monitor- ing effort into effective remedial action – which makes one wonder: shouldn’t we be paying our patients rather than GPs for values monitored and targets achieved?
