* GENERAL RULES AND GUIDELINES *

All children must have a packed, ready-made lunch along with

their morning tea and afternoon tea as well as a drink bottle.

In accordance with our Healthy Eating Policy, children are not allowed soft drinks, lollies

or chocolate at the Holiday Programme.

However, on our Movie Trip Day, children may be bring along spending money

to buy refreshments with at the movies

They are responsible for looking after their own money.

We also request where possible, children DO NOT BRING nuts, nut products or egg

in their lunches as we do have children attending the Programme

WHO HAVE ALLERGIES TO NUTS, NUT PRODUCTS AND EGGS

Children are not allowed cell phones while attending the Programme.

Any phones that are brought along to the Programme

will be looked after by Holiday Programme Staff until home time.

We also ask that children leave all electronic games and devices at home

as well as any valuables.

If your child has any medication or asthma inhalers,

please make sure they are given to a staff member along with any current action and care

plans.

A Medical Consent Form must also be filled out by the Parent / Caregiver.

These are available from the Holiday Programme Coordinator or the School Office.

* SUN SAFETY PROCEDURES *

The staff will follow recommendations of the Cancer Society to protect children.

All Staff will be provided with Cancer Society information.

Parents are asked to provide their children with hats, appropriate clothing

for sun coverage and sunscreen for when outside.

Staff will act as role models by staying covered up.

* TRIP REQUIREMENTS *

ALL CHILDREN MUST BE AT SCHOOL BY 8.45 AM ON TRIP DAYS!

ON ALL TRIPS, children must have raincoats as well as suitable clothing and footwear

that is appropriate for the particular trip that they are attending.

They must also have a packed lunch with them as well as a drink bottle.

Please make sure they also have all necessary medication or inhalers.

PLEASE NOTE: On our Movie Trip Day, children may bring along spending money

As we need to book our movie tickets in advance, once the children make their choice,

Please fill out ALL SECTIONS on the following pages and return those pages

to the Edendale School Holiday Programme ALONG WITH YOUR PAYMENT.

NO BOOKINGS WILL BE ACCEPTED WITHOUT PAYMENT!

ALL BOOKINGS AND PAYMENTS ARE NON REFUNDABLE

ALL TRIP DAYS ARE NON REFUNDABLE!

NO CANCELLATIONS WILL BE ACCEPTED FOR TRIPS!

If your child is sick and unable to attend a day that they are booked in for, we may be able to

transfer their payment and booking to another day in the same Programme, PROVIDED THE PARENT / CAREGIVER HAS CONTACTED THE PROGRAMME

BY 8.30AM ON THE ORIGINAL DAY THEY HAVE BOOKED IN FOR, TO EXPLAIN

WHY THEIR CHILD IS AWAY

and provided there are spaces available on another day.

SORRY!

IF YOUR CHILD IS AWAY ON A TRIP DAY. WE UNFORTUNATELY CANNOT

TRANSFER YOUR PAYMENT TO ANOTHER DAY.

Payment can also be made at the Edendale School Office either by cash or cheque.

Payment can also be made by posting your payment along with

the completed enrolment form to the address below.

* DROP OFF AND PICK UP ARRANGEMENTS * FOR CHILDREN

If you arrange for an alternative Caregiver apart from yourself or the Caregivers listed in the

Parent / Caregiver Column on the previous page, to pick up your child from the Programme,

you must contact the staff at the Programme in advance to authorise the pick up.

Please note that the people you have listed as Emergency Contacts have authorisation only to

pick up your child in cases of illness or injury where the staff have been unable to contact the

Parents or Caregivers.

All Caregivers and Emergency Contacts must be over 16 years of age.

If you have not contacted the Programme prior to arranging an alternative person to pick

your child up, we unfortunately WILL NOT BE ABLE to let your child

leave the Programme with that person.

If you wish your child to walk home by themselves at 3.00pm when the Programme finishes,

your child must be 10 YEARS OR OLDER.

No child will be allowed to leave the Programme by themselves BEFORE 3.00 pm.

No child will be allowed to leave the Programme with any siblings or other children

who are YOUNGER THAN 10 YEARS OF AGE.

No child who attends Post Care, REGARDLESS OF AGE, will be allowed

to leave the Programme after 3.00 pm unless they are accompanied

by an authorised adult ( who must be over 16 years of age ).

* HOLIDAY PROGRAMME CONSENT FORM *

I hereby authorise the Edendale School Holiday Programme to seek medical help

if the need arises for my child while they are attending the Programme.

I understand a Staff Member will try to contact me and then the Emergency Contacts listed

before they seek medical help if there is enough time to do so but should they fail to make

contact or if it is an emergency, they will act as caregiver and sign on my behalf.

I authorise the Edendale School Holiday Programme to move my child

to a safe place if the need arises.

I give authority for my child to attend any trips / outings I have booked for them

in this Term’s Holiday Programme.

The Organisers of this Programme have adopted the Auckland City Council’s

Holiday Programme Standards and Guidelines for Holiday Programmes.

I acknowledge that in signing this form that neither the Organisers of this Programme

nor their Management Staff will be held liable for any loss or damage arising

by way of theft, injury, accident or otherwise out of attendance at this Programme.
