St. Ann Parish PREP Registration
2016/17
1. Please return the attached registration form with payment to the
PREP office at St. Ann’s Rectory, 502 S. Main Street, Phoenixville.
Please remember to fill out both sides of the form. You anly need to
fill out one form per family. Registration and payment may also be
returned at any of the remaining PREP sessions of the 2015-2016
PREP year. Please contact the PREP Director immediately if you need to discuss a payment plan.
2. It is essential to read all registration information included in this
letter in order to know how to proceed.
3. Make sure to clearly identify your choice of day. Know that your
choice is a commitment you are willing to make for the entire program
year.
4. If registering a child for the first time, you must include:
a. A copy of the child’s Baptismal certificate if the child was not
baptized at your current home parish
b. A copy of the child’s First Penance and/or First Communion
certificates.
c. Information from a prior Religious Education Program, Parish or
Regional Catholic school, or Catholic Academy if your child is
entering level two or above.
5. Tuition rates for the 2016/17 year are listed on the registration
form. Please make all checks payable to St. Ann Parish. Please be
advised that your check will not be cashed until 7/1 (the start of the
Parish’s fiscal year). You will receive an email confirmation when the
registration is received.
6. PREP registration will conclude on July 31st, 2016. After that date, a
$50.00 late fee per family will be assessed.
7. If you make the commitment to volunteer on a weekly basis as a
Catechist, Aide, or Office Staff, your family’s total PREP tuition will
be waived. Please contact the Director of PREP to discuss this
opportunity.
8. We are asking for at least one parent from each family to volunteer
to help on a rotating basis. Please fill out the Parent Participation
form and return with your registration. Rotating volunteers are
required to obtain both the State Police and Child Abuse clearances.
