SENIOR MANAGEMENT ROTATION OBJECTIVES FORM Purpose of Rotation: This rotation is the penultimate rotation for a resident nearing the end of their training (PGY5). As such, the major emphasis is to provide the resident with significant responsibilities and latitude in decision-making, including strategic and tactical relationship development, relationship management, application of judgment, delegation and monitoring of performance. The rotation will also provide experience to achieve a number of specific management objectives which the resident has not yet accomplished in previous rotations, through project management or direct management of staff. Residents will play leading and collaborative roles in the maintenance and improvement of the health and well- being of the community through interdisciplinary and inter-sectoral partnerships. Instructions: The form is to be completed, reviewed and signed off with your rotation supervisor before the start of each rotation. A signed copy is to be submitted to the Program Director no later than the first day of the rotation. Keep a copy for review at mid-unit evaluation and at final evaluation. At the end of each rotation, please complete the “Evidence of Achievement of Objectives” column of the form and review with your supervisor. Consider this information in the resident, faculty and site evaluations. Then, sign the form along with your supervisor and resubmit to your Program Director. Purpose of Rotation:

This rotation is the penultimate rotation for a resident nearing the end of their training (PGY5).
