Major Research Interests

Improving the Mental Health of Vulnerable Populations . The theme guiding my research has been the interplay between formal care giving systems (e.g. medical, mental health and social welfare) and the vulnerable populations they serve. I have examined the latent functions of the medical care system and the barriers that prevent primary care pediatricians from diagnosing and treating prevalent emotional and behavioral issues in their young patients. I currently work with a team funded under the AHRQ Quality Initiative to develop quality indicators for the diagnosis and management of adolescent depression in primary care. Having developed a model of barriers to care for emotional/behavioral problems in pediatric primary care, I have come to realize that changing practice will necessitate system, institution, and physician level modifications.

That children are nurtured in families and that US social services systems can serve as resources or barriers for families has prompted two areas of investigation, one on the long term consequences of adolescent pregnancy, and the other on the impact of child welfare services on children’s well being. The child welfare work began with an evaluation of a comprehensive set of medical and social services for foster children in Connecticut, moved to research using data from the first nationally representative sample of children investigated by US Child Welfare agencies, the National Survey of Child and Adolescent Well-being, and most recently has focused on the development and testing of a theoretical model for the exploration, adoption, implementation and sustainability of evidence based practices. I also have collaborated on the design and execution of two studies to improve family environments for children. With my San Diego colleague and project PI, Cynthia Connelly, Ph.D., I am examining whether a community based telephone intervention for depressed, low income Latinas improves maternal depression and mother-child attachment. With my Stanford colleague and Co PI Richard Shaw, M.D., Chief of the Medical Consultation Liaison Service, I am examining the

effectiveness of a six session trauma focused intervention for mothers of preterm infants for reducing maternal depression/trauma and improving mother-child interactions.

Mental Health Services Research. My final area of research is mental health services. I was an investigator on both of the NIMH-funded multisite studies examining the prevalence of mental health problems and services use in children and adolescents. With my colleague, Robert Findling, M.D., Chief of Child Psychiatry at the Case School of Medicine, I developed a multisite study examining the positive predictive value of early symptoms of mania and the role treatment plays in symptom evolution. This observational study has identified four distinct trajectories of manic symptoms and suggests that treatment is a critical predictor of course. The importance of and variation in treatment quality reinforces the need to insure the adoption of and fidelity to evidence-based practices, key components of the research I am now conducting in conjunction with the NIMH-funded Advanced Center awarded to my colleague Kimberly Hoagwood.
