•Enroll, cancel or change all benefits except the State Health Plan

•Changes made during annual enrollment are effective January 1, 2010

•Enrollment packets will go out to Payroll Coordinators throughout campus on September 24. Please distribute materials.

•Dental rates are increasing by 9%

•Vision benefit in the State Health Plan ends 12/31/09. Consider vision plan enrollment.

•No Evidence of Insurability (EOI) needed for the Critical Illness plan

•No EOI needed for low option Cancer Plan or $20,000 in NCFlex Life.

•Must enroll each year for Flexible Spending Accounts

•Applies to anyone on the State Health Plan (SHP) when it is the primary coverage.

•July 1, 2010, tobacco cessation component begins.

•July 1, 2011, weight management component begins.

•All SHP participants are placed in the 70/30 PPO plan effective July 1, 2010.

•During the annual enrollment in March 2010, participants will “attest” that they and covered dependents are non-smokers OR are actively participating in a tobacco cessation program to move into the PPO 80/20 plan.

State Health Plan Comprehensive Wellness Initiative

•All SHP participants are placed in the 70/30 PPO plan effective July 1, 2011.

•During the annual enrollment in March 2011, participants will “attest” that they and covered dependents are non-smokers OR are actively participating in a tobacco cessation program AND must attest they have a BMI <40 OR in a weight loss program to move into the PPO 80/20 plan. (Physician exemptions)
