Attachment A - STUDENT INTERN RESPONSIBILITIES (Please initial all applicable)

Agree and accept the academic work assignments outlined in this agreement.

Meet with the Supervisor at least one hour each week, or as agreed.

Complete the Evaluation of Organization Form as requested by professor.

Keep all information concerning the Organization confidential.

Use any such information only for the purposes necessary for this internship.

Familiarize and adhere to the organization’s policies and procedures and standards of ethical conduct.

Adhere to workplace safety rules and procedures.

Inform my supervisor of any problems or concerns.

Attend the internship site during scheduled work dates and times.

Assume the risks and any other hazards that may arise in relation to this internship.

Agree to indemnify, release, waive, discharge, and covenant not to sue William Jessup University (WJU), its Board of Trustees, officers, employees, agents and volunteers for all costs, damages, and awards, including attorneys fees, arising from or related to any demands, causes of action, claims, counterclaims, or cross-claims brought by any other person or entity against William Jessup University arising from or related to this internship for my acts or omissions, whether intentional or negligent.

Understand that William Jessup University reserves the right to make cancellations, changes or substitutions with the internship placement in cases of emergency or changed conditions or in the general interest of this internship program.

Receive and agree to the terms of William Jessup University’s internship registration procedure.

Receive and agree to all the policies and terms of the William Jessup University Student Handbook which is in effect as of this date.

Communicate with the Faculty Advisor and the Director of Career Development, if necessary, about questions/comments.

I understand and acknowledge that there are risks associated with this internship, some of which may arise from (a) the location of the internship, (b) the amount and type of criminal activity at or near the location of the internship, (c) the hours when I will be present at the Internship Site, (d) the educational, criminal, mental and social backgrounds of the individuals I will be working with or serving, (e) my assigned tasks and responsibilities and (f) the amount of supervision I will receive. Attachment B - SUPERVISOR DUTIES (Please initial all applicable)

Orient, train and supervise the Intern.

Provide Intern with Organizational Policies and Procedures, including harassment prevention training.

Meet with the intern at least one hour each week, or as agreed.

Provide feedback and constructive criticism to the student.

Complete the Evaluation of Intern Form each week, or every _____ hours.

Enforce workplace safety rules and procedures.

Comply with US Dept. of Labor policies on paid/unpaid internships.

Maintain any information provided by William Jessup University or the Faculty Advisor as “confidential” in accordance with FERPA.

Destroy or return to WJU any student information necessary to protect student confidentiality.

Agree that Intern acts as agent of the Organization and not William Jessup University when executing any documents on behalf of the Organization.

Attachment C - FACULTY ADVISOR DUTIES

Agree to meet regularly with the Intern to discuss the internship experience and do an on-site visit, if possible.
