The mini‐lesson titled “Reevaluation with Assessment in EXCEED, Part 2”, covers the IEP Chair Responsibilities when completing a Reevaluation with Assessment.  This mini‐ lesson continues the Reevaluation with Assessment outlining the Evaluation Chair’s responsibilities.

In order to open the Reevaluation Event, you must first  go to the Current and Upcoming Events and click on the Meeting Manager.

If you click on the  Event Overview  you will see a list of the forms needed to complete the report – MOST of the forms are identical to what is in the Initial Eligibility Event (one exception – Instead of an Evaluation Summary – there is a Reevaluation Summary in the Eligibility Determinations Event Section).  The Evaluation Chair is responsible for completing the NOA and Areas to be assessed forms.

When the Evaluation Chair has been notified that Additional Testing is needed (for this example – it is additional testing that requires parent consent), the Notice of Action and Areas to be Assessed Forms need to be completed within the Reevaluation Event.  If additional consent is needed, an additional NOA/Areas to be Assessed is available.

This NOM is different in that it is being used for two purposes – by the IEP Chair to schedule the review of existing data and by the Evaluation Chair to schedule a review of the test results.  To view the NOMs sent by the IEP chair, you will only need to click on Previous Invites located on the bottom center of the screen when in the NOM form.  Deleting the information that is in the form when it is selected will not delete the information in the saved PDF that was created once Send Invite was selected.  It is important to ensure that the IEP Chair clicked Send Invite or the information will be lost.  This can be checked by clicking on Previous Invites to see if there is a saved PDF for the previous inviteclicking on Previous Invites to see if there is a saved PDF for the previous invite.

Use the templates provided by your department to  paste Background and Test Interpretations into EXCEED.  All text boxes will expand to hold all of your information.

It is important to note that an eligibility statement must be completed for any new area of eligibility and also for the disabilities the student continues to meet eligibility for as determined by the IEP team.

Remember to reenter areas of disability.

When the disability is confirmed by the IEP team the Evaluation Chair needs to include the statement provided by your EPS, in the Eligibility Determination text box for each area of disability.  

Click SaveClick Save.

The Professional Judgment response should always be “No” for previously determined eligibilities.

When completing the Reevaluation Event, complete any new areas of Disability – same as in the Initial Eligibility Event.

Click on the Record of Participation in the Events Section.  Enter the names of those participating in the meeting.  The yes/no boxes are only required if the student has a Specific Learning Disability.  Save the form and return to the event overview.

Remember: All forms, with the exception of the RED, are considered optional until any necessary information is updated on the Disabilities screen.  At that point, Eligibility Determination forms may be required depending on the data entered.  The other forms will not show as required, but will still need to be completed such as the Record of Participation, the Background, and the Evaluation Results.

In the Event Section you will find an NOA which is to be used to notify the parent/guardian of a Change in  Eligibility.

At the bottom of the Event Overview screen click check Compliance. 

Next, click Print, in order to view the printed copy as a PDF.  Proofread it carefully since it will not be able to be unlocked. Edit the document if necessary.  Attach ESI Assessment Report.

After you are satisfied click lock This will lock the Reevaluation EventAfter you are satisfied, click lock.  This will lock the Reevaluation Event. 

Notify Word Processing that report is ready to be sent out.
