Description of the Company

1. Introduction PT. LUBRITAMA PACIFIC INDONESIA is a lubricating oil processing plant. Its main business activities are importing raw material base oil (Base Oil), Oil Lubricants (Lube Oil Blending) and packaging (packing) to meet the needs of Indonesian market.

Area of Operation: PT. PLI has 2 areas of operation are:

1. Plant located on Jl. Kingdom Bojonegara Km.6 Bojonegara Attack Sub. There is an entrance to the factory location in the southwest. (See Figure 1).

2. Terminal covering an area of tanks piled and Jetty 1.

2. Various Emergency Notification Procedures for Emergency Conditions 2.1. fire 2.1.1. Fire in factory The first person who saw the fire should:

• Turn on the nearest fire alarm and evacuation of all personnel. • Confirm the location of the incident to the Supervisor Place of Genesis (SIC) in

the Production Section (ext 15) • Trying to extinguish a fire if the situation allows

In the main Fire Alarm Panel at Security Post will be displayed to check the location where the alarm. Guides Safety Officer will announce the location of being emergency and PLI Emergency Response Team (PERT) will immediately jump to the scene. Supervisors Genesis Place (SIC) will confirm the type and condition of the emergency and instructed the Emergency Response Team members PLI (PERT) others to contact the local Fire Department (at the phone number 113).

2.1.2. Terminal Area Fire The first person who saw the fire should: • Ring the nearest fire alarm and evacuation of all personnel • Confirm the location of the incident to the Jetty Master Controller as Genesis

Place (SIC) (ext 21) and then contact the Jetty Master Fire Fighting Team SMI (Hadi at Tel No. 0852 6444 0121).

• Trying to extinguish a fire if the situation allows

Guides Safety Officer will announce the location of being emergency and the Emergency Response Team PLI and SMI will immediately jump to the scene. Supervisors Genesis Place (SIC) will confirm the type and condition of the emergency and instructed the Emergency Response Team members to contact PLI other local Fire Department (on phone number 113 )

2.2. Spill occurs 2.2.1. Spill Area Factory If a spill occurs, the person who first saw must:

• Trying to find the location of the source of the spill, stop the spill source, localize the spill and contain it without taking risks.

• Confirm the location of the incident to the Supervisor Place of Genesis (SIC) at the production office (ext 15)

• Supervisory Genesis Place (SIC) will determine the number of spills that exist and PLI contact the Emergency Response Team (PERT) to prevent the spread of spilled oil by using sand, Solvent, NEOS, or other material that can absorb the oil spill.

• Contact the Environment Agency emergency services (at telephone number 0254-200177).

2.2.2. Spills in the area of Terminal If a spill occurs, the person who first saw must:

• Trying to find the location of the source of the spill and contain it without taking risks.

• Confirm the location of the incident to the Jetty Master Controller as Genesis Place (SIC) (ext 21) and subsequently Jetty Master will inform the Spill Response Team SMI (Hadi at Tel No. 0852 6444 0121).

Supervisors Genesis Place (SIC) will determine the number of spills that exist and take the following steps:

• Stop pumping activity / loading and unloading • Call the Spill Response Team SMI to prevent the spread of spilled oil by using

sand, Solvent, NEOS, or other material that can absorb the oil spill. • Contact the Environment Agency emergency services (at telephone number

0254-200177).

2.3. Emergency relief 2.3.1. Emergency Aid in the Factory The first person who saw this condition shall:

• Conducting emergency rescue if understood • Contacting Supervisors Genesis Place (SIC) (ext 15) to tell the nature of the

medical condition and location of the incident. • After Supervisory Genesis Place (SIC) received a call, then with PLI Emergency

Response Team (PERT) will go towards the scene. Supervisors Genesis Place (SIC) will instruct all personnel rescuers to perform the following actions:

• Contacting P3K officer (if not available, contact the SMI P3K at telephone number 0254-5753172)

• Call an ambulance by calling an ambulance in the SMI phone number 0254-5753172.

If any unexpected needs, Supervisory Genesis Place (SIC) will instruct the employee to used private cars to transport the victim to a medical center.

3. Fire Response Procedures • Supervisory Genesis Place (SIC) will determine the location of safe places

gathered PLI Emergency Response Team (PERT) depending on the location of the fire.

• Members of PLI Emergency Response Team (PERT) others gathered at the Congregate Safe Place (Muster Point) and be prepared to receive further instructions to extinguish the fire or rescue victims.

• Activation of fire fighting facilities was conducted by the Chief or Assistant Chief in charge.

• Uniform Firefighters should be used by members of the PLI Emergency Response Team (PERT) which will enter the scene to conduct an emergency action or rescue victims. Rescue to be done in pairs.

• Supervisory Genesis Place (SIC) commissioned PLI Emergency Response Team (PERT) to record the number of employees who enter the emergency location or building.

• Members of PLI Emergency Response Team (PERT) others will be assigned by the Supervisor of Genesis Place (SIC) to assist with the evacuation of all employees and themselves to Assemble Emergency Safe Place.

• All victims should be brought to a Safe Place Congregate if possible.

• Once firefighters arrived, the Supervisory Genesis Place (SIC) will hand over this task and describes the actions that have been done.

• Supervisory Genesis Place (SIC) will be ready to assist firefighters if needed until the emergency is over resolved.
