  • Welcome to UnitedHealthcare Health & Home Connection!UnitedHealthcare Health & Home Connection is a program for people 18 or older who have met CARES long term care eligibility. The goal of UnitedHealthcare Health & Home Connection is to help you live at home for as long as possible. We want to help you be as healthy and independent as you can while staying in your home. UnitedHealthcare Health & Home Connection will help you and your family choose and arrange the right kind of care in your home. You can read more about the care you get from the plan by reading the UnitedHealthcare Health & Home Connection Enrollee Handbook.

    We offer many benefits: Personal care to help you bathe and dress. Homemaking and assistance with chores around your home. Home-delivered meals or meal preparation. Adult day services for social interaction and well-being. Personal care incontinent supplies not covered by Medicare. Respite Care. Assisted Living Services. Personal emergency help system. Occupational, physical, speech and respiratory therapies. Home modifications to support mobility. Non-emergency transportation to doctor appointments. Management and assistance with taking medications. Assessment, education, and guidance to you and your family about your nutritional needs. Services to address mental health or substance abuse needs.

    Definitions Primary Care Doctor When you join UnitedHealthcare Health & Home Connection, you can

    keep the doctor you have now. Your Doctor and your Case Manager will help arrange for all the health care you need. Your Doctor and Case Manager will set up a Case Management Team. It is your job to call your Primary Care Doctor and Case Manager when you need care. If you have an emergency, call 911, or go to the nearest emergency room. You may continue to use any physician or hospital you have been using for the services that Medicare covers.

    Case Manager Your Case Manager will work with your Doctor and other providers to help plan all your care. Your Case Manager will help plan for home care services. Your Case Manager may be a Registered Nurse or a social worker.

    Care Management Team A team made up of your Primary Care Doctor, your Case Manager, and you and/or a family member. Your Doctor may talk with your Care Management Team to help plan your care.

    Handbook A booklet that explains your covered health care services.

  • About Your Provider Directory This directory lists services you can get at home. It also lists services you must get at a health care

    office or hospital. If you do not find your service provider listed, please call the Customer Service Department at 1-800-791-9233, or TTY 711 for hearing impaired, between the hours of 8:00 a.m. and 7:00 p.m. Eastern time, Monday through Friday.

    Emergency Care A medical emergency happens when you are sick or injured you feel your health might be in

    danger unless you get medical care right away. Any time you think you have an emergency, call 911 or go to the Emergency Room. In an emergency, it does not matter if the hospital is in our network or not. It does not matter if you are in our Service Area, or not. We will always cover your emergency health services.

    You should call your Case Manager as soon as you are able after you get emergency care. Then, your treatment can be authorized and coordinated. All follow-up care must be provided or arranged by your doctor.
