Driven by our commitment to enrich lives by providing access to life beyond barriers, EZ- ACCESS, a dba of Homecare Products Inc., was founded in 1984 by Glenda Everard and her daughter, Deanne Sandvold. In 1988, son Don Everard joined the family business, bringing with him marketing expertise. This trio was the driving force behind the expansion of the modular aluminum walkway market, having pioneered the industry through the marketing and manufacturing of EZ-ACCESS portable wheelchair ramps and walkways.

EZ-ACCESS operates out of two locations. Headquarters and West Coast distribution and manufacturing (125,000 ) are located in Algona, Washington, 15 miles south of Seattle and East

Coast distribution and manufacturing (100,000 ) is located in Morganfield, Kentucky, where the

company reinforces its family-based philosophy with customers, employees and community alike. The two manufacturing facilities enable faster and less expensive deliveries across the nation to all their customers. Here is our corporate timeline:

Date Event 1984 EZ-ACCESS, a women owned company, is formed and begins providing bathing

products and portable ramps to the Durable Medical Equipment (“DME”) market. 1990 The Americans with Disabilities Act (“ADA”) becomes law. 1997 EZ-ACCESS is the first company to put portable ramps on the GSA-FSS contract.

EZ-ACCESS launches the tri-fold “Suitcase” portable aluminum ramp. 2003 EZ-ACCESS releases the “Pathway 2G” modular aluminum walkway system, which

it adds to GSA-FSS contract. The VA quickly adopts this as an alternative to wood. 2004 EZ-ACCESS changes the VA walkway system delivery model to provide installation

services quickly and directly to America’s warriors. 2005 – 2017

EZ-ACCESS obtains cooperative purchasing agreements from multiple VA locations, adopting a strategy of regional distribution and delivery, solidifying the manufacturer-direct delivery model.

2009 EZ-ACCESS opens its second manufacturing and distribution facility in Kentucky. 2010 EZ-ACCESS launches the “Titan” commercial modular aluminum walkway product.

EZ-ACCESS releases the aluminum OSHA stair. 2011 EZ-ACCESS launches the aluminum OSHA stair v.2.0.

EZ-ACCESS releases the “Passport” vertical platform lift product. 2014 EZ-ACCESS launches the aluminum OSHA “Fortress” stair. 2015 EZ-ACCESS releases the “Concierge” automatic door opener and the graphite

In many school districts and municipalities, deteriorating wood and asphalt ramps are used to access modular buildings and to provide stop-gap ADA access to their facilities. Our marketing and data analytics teams can review the buying patterns of TCPN members and proactively reach out to those who will soon need replacement walkway systems made out of long-lasting aluminum.

DATA ANALYTICS We collect and maintain a proprietary customer purchasing database that can serve TCPN. We have the ability to cross-reference the purchase orders of TCPN members and eligible agencies with TCPN’s internal purchasing data to identify any members who are purchasing aluminum modular walkway systems from sources outside of this TCPN contract. We can then target these members and educate them about the TCPN aluminum walkway systems contract, so that they can take advantage of TCPN’s nationally leveraged pricing. This will increase TCPN revenue and membership while serve the members. 13. What differentiates your company from competitors? EZ-ACCESS has grown to 175 employees, a world-class order processing center, and two state-of-the-art facilities with over 200,000 ft2 of manufacturing and distribution space. D. Financial Stability & Strength. We offer a stable partner to TCPN; Homecare Products, Inc., dba EZ-ACCESS, a Washington corporation, has been in business for more than 30 years, has an enviable financial foundation, and resultant strength to ably meet the requirements of this contract

i. ACCOUNTING.

The EZ-ACCESS accounting department operates in accordance with applicable federal and state rules and regulations as well as Generally Accepted Accounting Principles (GAAP), a collection of industry standards for financial reporting and statements (e.g., balance sheet, statement of income and retained earnings, statement of cash flows, notes to accounts, etc.). We utilize a state-of-the-art accounting software system allowing immediate and accurate periodic reporting.

Our financial statements are prepared and reviewed by Edward L. Fietz, CPA, of Rhodes & Associates, PLLC, a full service CPA firm established in 1978 and serving the Seattle, Washington metropolitan area for over 30 years.

ii. BANKING. EZ-ACCESS primarily banks with Washington Trust Bank, a diversified financial services holding company headquartered in Spokane, Washington. It is the oldest and largest privately owned commercial bank in the Pacific Northwest. EZ-ACCESS has a long-established line of credit and excellent working relationship built over decades of mutual rapport and reliability.

iii. LIQUIDITY. The cost of funds borrowed by EZ-ACCESS is lower than the returns generated by assets which are controlled by the business, evidencing that EZ-ACCESS has used capital gearing efficiently to accelerate our profits.

iv. DEBT TO EQUITY RATIO. EZ-ACCESS has a favorable current ratio of 2:1, indicating low risk.

v. SHAREHOLDERS EQUITY. The shareholder’s equity or net worth account of EZ-ACCESS has a positive balance which is greater than the goodwill of the company.

vi. CASH FLOW: The EZ-ACCESS cash flow statement shows that the short-term assets are funded by short tenure borrowings, and long-term assets are funded by long tenure borrowings, reflecting long viability of the business.

E. Resources

i. WEBSITE. We offer TCPN a more robust website than the incumbent with an incline calculator and chart, ramp selector tool, online live chat, a comprehensive answer center, technical documentation, photo galleries, assembly and use instructions, warranties, an order page, and videos for each product we offer. Our website page views exceed 100,000 per month. This makes our online presence a valuable conduit to educate and serve the modular aluminum walkway market.

ii. PERSONNEL. EZ-ACCESS is staffed for a high annual volume of production for projects from coast to coast. Should the need arise, additional staff and additional shifts will be added quickly to process any additional work that may be created above the volume currently processed. Departmental staff are cross-trained to ensure the proper handling of project requests, orders, customer service answers, and related inquiries as required by the contract. The production and shipping departments are staffed to supply and deliver required products for the contract in the volume needed with no additional delay.

iii. MATERIALS. Extruded aluminum is the primary raw material used in manufacturing EZ-ACCESS ramp and walkway systems and is sourced from two large independent extrusion companies. With matching extrusion dies at both companies, EZ-ACCESS greatly reduces the risk of raw material supplier issues due to limited capacity or acts of God. Other raw materials are sourced from national suppliers with whom EZ-ACCESS has long-term relationships.

iv. DISTRIBUTION. EZ-ACCESS aluminum modular walkway systems are shipped from our distribution centers via commercial carriers directly to our customers/partners. Shipment and tracking information is retained in our proprietary project management system (EZ-Tracker) for each shipment. EZ-ACCESS has contracts with four national commercial

trucking firms as well as multiple parcel carriers, capable of shipping our high volumes without expansion and ensuring continuity of walkway system distribution in the event one carrier is unable to perform or its services are temporarily disrupted. We also use a, photographic shipping inventory system to guarantee that all components will be included in each shipment. Our reinforced packaging, not like others’ plastic banding, demonstrates our care and ensures that each modular aluminum walkway system arrives intact even if it is shipped long distances.

v. MANUFACTURING. EZ-ACCESS facilities in Algona, WA and Morganfield, KY have over 200,000 ft2 of manufacturing and distribution floor space. No additional capital expenditures or major equipment will be needed to perform the contract. EZ-ACCESS will seamlessly increase walkway system manufacturing and distribution from its current facilities as needed to meet increased contract demands. The geographic separation and redundant capabilities of our two manufacturing and shipping facilities ensure continuity of walkway production in the event of natural disaster or other disruption at one location.

vi. TECHNOLOGY.

Accounting Software. EZ-ACCESS uses the Microsoft Business Management - Great Plains software for corporate purchasing, inventory control, and related accounting. This software is developed and supported by Microsoft and additional licenses are available as needed for expansion. EZ-ACCESS incorporates system security and performs data backups on a daily basis.

EZ-ACCESS’ data analytics department uses Forecast Pro, which provides the ability to create accurate and credible data-driven forecasts, along with other tools for efficiently managing, monitoring, and improving our aggregate fulfillment process. Forecast Pro provides custom modeling options and a comprehensive set of diagnostic tools. Forecast Pro’s comprehensive methods accommodate seasonal demand, product hierarchies, projected growth, new products, slow-moving items, causal variables, and outlying factors. The EZ-ACCESS forecasts for 2015 was accurate to within one percent (1%) and 2016 was accurate to within three percent (3%).

Project Management Software. EZ-Tracker is a custom-built tracking software suite designed specifically by EZ-ACCESS for the documentation and tracking of the milestones that cover a project’s completion. This program allows for real-time input and storage of data, photos, and documentation for the purposes of monitoring the order process from beginning to end. EZ-Tracker currently holds data from over 80,000 projects that EZ-ACCESS has performed over the past ten years. The use of this software and the ability to track individual processes and service milestones is paramount to contract performance. The data tracked by this program is housed on a secure server located at the EZ-ACCESS corporate office and is backed up on a daily basis. Hardware. EZ-ACCESS information technology (IT) hardware systems include three (3) domain controllers, twenty-two (22) servers, 150 computer stations, and a Mitel IP phone system. Email and data systems are encrypted and backed up daily.

IT Personnel. EZ-ACCESS maintains an IT staff of experienced technicians to support all EZ-ACCESS’ technology needs. IT audits are routinely performed to assure that there is no interruption to the data processing or communication between EZ-ACCESS and our customers/partners.

vii. VENDORS. EZ-ACCESS has strong working relationship with primary vendors that assures uninterrupted supply of raw materials and the capability to grow without placing a strain on financial or material partners.

We are committed to helping to build a cleaner future! As our business grows, we want to make sure we minimize our impact on the Earth's climate. So we are taking every step we can to implement innovative and responsible environmental practices throughout Region 4 ESC to reduce our carbon footprint, reduce waste, promote energy conservation, ensure efficient computing, and much more. We would like vendors to partner with us in this enterprise. To that effort, we ask Offerors to provide their companies environmental policy and/or green initiative.

28. Please provide your company’s environmental policy and/or green initiative. We hold environmental stewardship as a high value. The aluminum we use in our modular walkway systems exceeds 67% recycled material to reduce our environmental impact. We also recycle all aluminum trimmings, shavings, and scraps. Our modular walkways are built tough, so they can last many years allowing them to be reused for many different projects, instead of ending up in a landfill. Once our walkway system is no longer needed, it can be fully recycled. We use transition mats made from 100% recycled rubber for exact fits around thresholds. We reduce our paper waste by minimizing use of printed documents and storing most files electronically. We also seek transport partners who use light-weight trailers to reduce total carbon emissions.

To be considered for evaluation, companies must provide relevant responses to all these sections. Availability and Quality of Resources Describe the various resources from your company that will be made available, to assist the districts in the execution of their mission. Indicate the availability of each resource, e.g., fulltime, part-time, or as required.

Over the last 30 years, we have created policies, procedures, and entire departments devoted to assist our customers/partners accomplish their goal of safe access. We have three customer service teams who are all cross-trained in our commercial walkways, residential walkways, and powered products; they are available for questions via email or phone from 8 a.m. – 7 p.m. Central Time Monday to Friday. We also have a technical team that can troubleshoot the most complex issues with every single product line and even past products; they are available for questions via email or phone from 10 a.m. – 7 p.m. Central Time Monday to Friday. All our customer service groups abide by the 1-day rule, where any missed call or email is responded to within one business day. Additionally, our website features an incline calculator and chart, ramp selector tool, online live chat, a comprehensive answer center, technical documentation, photo galleries, assembly and use instructions, warranties, an order page, and videos of all our products. Our technical team is working diligently on a comprehensive electronic knowledgebase for all products; this resources will be made available upon completion and testing. Our full product catalog, specifications, photos, and literature can be found in Section XII. Installation Capabilities Installation services are available in most areas through EZ-Access’ nation-wide network of certified independent ramp technicians. We can coordinate with both the technicians and TCPN members, but a separate installation agreement is required. Installation fees are variable and dependent upon scope, location relative to installer, and site conditions. Training Capabilities Training of staff in the use of and maintenance of the modular aluminum walkways is critical. An understanding of your capabilities in training staff both through skills transfer and classroom models is desired. Educating our customers/partners is a high value for EZ-ACCESS. We provide electronic copies of technical documentation, online live chat, a comprehensive answer center, detailed assembly and use instructions, and installation videos on our website, but sometimes that is not enough. Project Management Skills TCPN requires a project management function as part of the services delivered by the vendor. Service providers must describe their competencies to manage and coordinate project activities, resources, and communications. Service providers must address their capabilities in areas to include but not limited to:

1. Project management methodologies and tools. We use well-documented policies and procedures, software, and regular reviews/audits to assure that each project is managed well with direct inter-team collaboration. We have invested tens of thousands of dollars to create EZ-Tracker, a custom tracking software suite. It is designed specifically for EZ-ACCESS to document and track the milestones that cover all project processes. This program allows for the real-time input and storage of data, photos, and documentation for the purposes of monitoring the order and implementation process from beginning to end. EZ-Tracker currently holds data from over 80,000 projects that EZ-ACCESS has performed over the past ten years. The use of this software and the ability to track individual processes and service milestones is paramount to contract performance. The data tracked by this program is housed on a secure server located at the EZ-ACCESS corporate office and is backed up on a daily basis. All data input and retrieval is encrypted to ensure the security of the confidential information contained in each project (see sample of EZ-Tracker screen, below). For more information on policies and procedure and quality assurance methods, see the following questions.

2. Project planning and communications. Our sales and customer service teams are amazing, and

they are trained to map out and communicate the planning and execution process from the customer/partner’s first inquiry until final delivery. Through their expertise with EZ-Tracker and tens of thousands of successful projects, our sales and customer service teams can receive all project details, ask pertinent questions, answer customer/partner concerns, and create a successful design that will provide a safe and positive outcome. We document all customer communications in EZ-Tracker, so that all projects can be accurately supported, even if the originating team member is out of the office. In general, our process for a project is (i) receive project information from the customer/partner, (ii) create a design to meet the project requirements, (iii) design approval by customer/partner, (iv) review by engineering if there is a custom component, (v) send to logistics to pull and package the appropriate system components from the closest distribution center, and (vi) ship the system to the customer/partner. Other departments will also be involved to handle invoicing, payment, etc.

3. Quality assurance procedures.

See Question 6 below.

4. Project management experience with large school districts (80,000+ full time students) and

small school districts (less than 1,000 Full time students). This year we signed a multi-year contract with the Jordan School District to provide turn-key district-wide aluminum walkway services. We have provided walkway systems to dozens of school districts and charter schools over the last 10 years. Universal safe access for all students is a high honor and priority for EZ-ACCESS. Our Titan™ commercial modular walkway system is the ideal solution for all school applications including portable classrooms, modular buildings, elevated walkways, bleacher access, temporary construction access, viewing platforms, and even stages. We have served school districts of varying sizes, for example: Metro Nashville Public Schools, TN (85,000), Jordan School District, UT (52,000), Auburn School District, WA (15,800), Tahoma School District, WA (7,900), and Comanche Independent School District, TX (<1,000).

5. Project management experience installing new walkways and complying with various regulations and requirements. We have manufactured and installed (through our dealers or certified independent ramp technicians) over 100,000 modular aluminum walkway systems, in every State and nearly every County in the U.S. Each municipality has different standards for walkway systems (i.e. specific ramp widths, curb rail heights, end loop configurations, etc.). Our staff is highly knowledgeable about state-level requirements, the federal standards of the Americans with Disabilities Act, Architectural Barriers Act, and OSHA, as well as the international standards of IBC.

Installation services are available in most areas through EZ-Access’ nation-wide network of certified independent ramp technicians. We can coordinate with both the technicians and TCPN

6. Quality Assurance programs during the manufacturing processes and construction / installation process. Our service quality plan is built on the Total Quality Management (“TQM”) model and implements our quality program with regard to service. As with the TQM model, our goal is to embed this proven quality management system within EZ-ACCESS to increase productivity, reduce unnecessary costs, and ensure quality of all processes and products.

We model to TQM to ensure EZ-ACCESS is consistently improving all aspects of our supply chain, manufacturing structure, distribution paths, product quality, and customer service from raw material procurement throughout the walkway system distribution flow.

When quality management is being undertaken effectively in a project, there will usually be areas identified for improvement. By building regular reviews of both output development processes and project processes, quality improvement can be carried out throughout the life of the project. • Real-time analytics are run to measure the input and output processes to track for

compliance to the timelines set forth in this contract. • Reviews of the analytic data are conducted by each manager on a periodic basis on both

their team’s input and output processes to assure quality of service performance of EZ-ACCESS. This includes customer service, managers, logistics, and operations teams.

• Periodic summary review reports are generated for each output stage of the program and are measured against our targeted outcomes.

Aluminum Docks and Gangways (through a partner). We have used our Titan™ walkway

system for several marine dock applications, including for a commercial water taxis’ ADA passenger loading ramp (see below). We work with a subcontractor for all our over-water Aluminum fixed and modular docks and gangway projects.They are fully capable of creating custom dock and gangway projects with short lead times. Their products fully-comply with the Aluminum Association Specifications and Guidelines for Aluminum Structures, IBC, and local codes. Freight will be charged on all docks and gangways.

TRANSITIONS® MODULAR ENTRY MAT. Designed for doorways and raised landings, the TRANSITIONS® Modular Entry Mat can be used by itself or in a pair to make entry convenient and safe. The mat simply sits against the doorsill (inside or outside) to provide a smooth ground-to-sill transition. The Modular Entry Mat is made of 100% recycled material, is slip-resistant, and is designed for years of continued use. To accommodate wider entrances two mats can be placed side-by-side.
