Is a refractive correction procedure from Carl Zeiss Meditec AG, which is carried out in a single step with a femtosecond laser, without the need to cut a flap.

The femtosecond laser creates a refractive lenticule (a thin round section of corneal tissue formed by creating a lower then upper femtosecond laser pass) in the centre of the cornea.

A small incision, less than 4mm, is then made in an otherwise intact cornea. The lenticule is then released from the cornea by using a fine spatula (6-835, 6-836 or 6-836-1) and then removed through the small incision using a fine pair of forceps (2-836E).

The removal of the lenticule has now permanently reshaped the cornea to correct the refractive error.

Additional / Optional Instruments

This technique involves inserting the dissecting forceps into the pocket created by the femtosecond laser, which on parting, completes dissection of each dissection plane in turn. The forceps can also be used to extract the lenticule, minimising instrumentation in and out of the pocket.Patient comfort is improved as eye movement is minimised. There is no need for fixation forceps, when there is counter traction provided by the opposing arms of the dissection forceps. The surgery is more consistent from the surgeon’s point of view, even when the dissection is suboptimal, as a result, day 1 vision is more constant. Dr Ian Chan, Perth, Australia

This surgical instrument developed with Professor Dan Reinstein has enabled a surgical technique for SMILE to be performed with one instrument, entering the eye only once through one incision, using only one sweep per plane, and taking about 30 seconds to separate and withdraw the lenticule. This technique minimizes the risk of infection by using only one instrument, inserted only once into the stromal interface. This technique also minimises trauma to the stromal interfaces, improving day 1 uncorrected vision over other lenticule extraction techniques that require more corneal manipulation.
