RCPA Format
Instructions:
Keep 2 Carbon papers in order to have 2 copies for TM
and BM
Fill personal information as your name / HQ, TM name/TM
HQ, date/ chemist name and place of chemist.
Write name and units/mth against it for our brand
and top 2 competitor Brands (prescribed at that counter)
Repeat same thing for all our Brands at the chemist.
Put stamp of Chemist and name of contact person, in the
space provided.
Put your signature in the space provided
Qty in strip / bottle / vial
Competitor brand name
