Position Description
Position Title: Aged Care Team Leader Level: 4 Department: Community Services Position Reports to: Community Services Regional Manager Position objectives: This position is responsible for managing the day to day operations of Aged
Care & Family Care services of the Shire Council in the community and surrounding ward.
Program objectives: Operate under the direction of the Shire Services Manager and in
accordance with Shire Councils corporate plans, policies and relevant legislation.
Division objectives: To provide a high standard of community services, supporting the health,
well being of the community.

Key Responsibilities
Responsible for implementing aged care programmes within the community. Responsible for the delivery of nursing care to aged care recipients. Comply with information requirements of the aged care and community care sectors Work effectively in a community care environment Provide support to aged care recipients and assist with self medication Provide personal care in a home and community care environment Support the aged care recipients to meet their emotional and psychosocial needs Provide care support which is responsive to the specific nature of dementia Facilitate cooperative behaviour and communicate appropriately with clients and colleagues Participate in safety procedures for direct care work Maintain a safe working environment and ensure Occupational Health & Safety regulations are
adhered to. Ensure the policies and procedures of the Shire are adhered to and all resources effectively
deployed.
Selection Criteria
Skills and Knowledge: Proven ability to work well under pressure. Strong administration skills including knowledge of Microsoft Word & Excel Well developed oral and written communication skills. Strategic and decision making skills with an analytical and innovative approach to problem solving. Understanding of community development. Sound knowledge of the principles of Occupational Health & Safety. Qualifications and / or Experience: Knowledge & experience of nursing & first aid A current driver�s licence. An appropriate tertiary qualification or experience.
Appointment Conditions
This appointment is a full time permanent position and the appointed applicant will be required to undergo a Police check. The position holder must be willing to adhere to Councils Code of Conduct, Conditions of Employment, Policies and Procedures.
