SAbout Michael White...Michael White was born in 1967 and grew up in the Blue Mountains, west of Sydney. A childhood spent close to the bush inspired his initial creative direction. After leaving school he completed an apprenticeship in the electrical trades. He then travelled and worked around Australia, the result of which deepened his interest in the landscape.

He is a self taught artist. His work has developed from impressionist studies of the landscape into more suggestive, sometimes abstracted interpretations of a variety of subjects. Whether painting outdoors or working in the studio, Michael continues to experiment and seek new directions for his art.

Michael's paintings hang in collections throughout Australia as well as the UK, USA, Canada, New Zealand, Germany, Swit-zerland, Holland, Denmark, Japan, Hong Kong, Singapore, Malaysia and Dubai.

The 10-AM CommandmentsAdmission for creation of an AV fistulaACS 1438 Chronic kidney disease, kidney replacement therapy provides the following classification advice:

1. Cases of chronic kidney disease with ongoing kidney replacement therapy, whether by dialysis or by transplant, which comply with ACS 0002, require a code from N18.- Chronic kidney disease to describe the current stage of disease, except in routine dialysis only admissions.

2. For routine dialysis only admissions it can be assumed from the assignment of Z49.1 Extracorporeal dialysis or Z49.2 Other dialysis that the patient has CKD – stage 5 (see also ACS 1404 Admission for kidney dialysis).

Does the same apply for patients admitted for creation of an AV fistula? Is a CKD code required, or should CKD only be assigned if it meets the criteria in ACS 0002 Additional diagnoses?When a patient is admitted specifically for creation of an AV fistula assign Z49.0 Preparatory care for dialysis as the principal diagnosis. An additional code for CKD should be assigned if it meets the criteria in ACS 0002 Additional diagnoses.

Alcohol poisoningWhen should the code for acute alcohol intoxication (F10.0) versus poisoning by alcohol (T51.0) be assigned?Clinical advice indicates that alcohol poisoning is a particularly severe form of alcohol intoxication. Typically, alcohol poisoning is characterised by major disturbance of conscious level, inability to rouse the patient and resultant threat to life requiring supportive treatment.

Coders should be guided by the documentation in the clinical record. Where acute alcohol intoxication is documented, assign F10.0 Mental and behavioural disorders due to the use of alcohol, acute intoxication following the index pathway:

Intoxication - alcoholic (acute) (with) F10.0

Where alcohol poisoning is documented, assign T51.0 Toxic effect of alcohol, Ethanol following the index pathway:

Poisoning (acute) (see also Table of drugs and chemicals)

and appropriate external cause of injury codes.

Anaemia in neoplastic diseasePlease clarify the following:

a. For a patient admitted with the following additional diagnoses:

Anaemia and melaena ? cause

Patient is transfused with 1 unit of packed red blood cells (Hb 9.2)

Patient is also noted to have prostate cancer, not treated or investigated at this admission and therefore does not meet ACS 0002 Additional diagnoses for code assignment.

Should the anaemia be coded to D63.0 Anaemia in neoplastic disease with additional codes for the neoplasm assigned or should the anaemia alone be coded?

The index pathway for ‘Anaemia, in neoplastic disease’ should not be followed in the scenario cited where the ‘anaemia’ and ‘melaena’ are clearly documented as due to an unknown cause. In this instance code the anaemia alone.

b. The following advice was issued in Coding Matters Vol 6, No 1:

‘Code D63.0* Anaemia in neoplastic disease should be assigned when anaemia occurs in, due to or with a neoplastic condition. The specific code for the neoplasm should be assigned when known, as indicated by the inclusion term:

We are now entering the final stages of preparation for the publication of ICD-10-AM/ACHI/ACS Seventh Edition. The Seventh Edition includes extensive input from interested parties and stakeholders. It also includes changes derived from the foundation classifications: WHO ICD-10 and the Medicare Benefits Schedule (MBS). Public submissions for changes to ICD-10-AM/ACHI/ACS are accepted all year round, via the public submissions link on the NCCH website. All classification users, for example: clinical coders, clinicians, epidemiologists and researchers who identify a need for improvement to the classification content (codes in the tabular, changes to the index, or perhaps clarification of a coding standard) are invited to submit their suggestion(s) through the NCCH Public Submission process. Public submissions are prioritised, with regard to the timing of the current updating work. NCCH staff constantly monitor the submissions and assess each proposal as it is received.

The NCCH acknowledges input from members of the Clinical Casemix Committee (CCC), Clinical Classification and Coding Groups (CCCGs) and particularly from the NCCH’s

Coding Standards Advisory Committee (CSAC). The CSAC has representation from Australian state and territory jurisdictions, New Zealand, the Health Information Management Association of Australia (HIMAA), the Australian Institute of Health and Welfare (AIHW), the Clinical Coders Society of Australia (CCSA), the private sector, CCC, the Commonwealth Department of Health and Ageing and the National Health Information Standards and Statistics Committee (NHISSC).

One of the primary aims of the work undertaken for Seventh Edition has been to review individual ACS to align them with the primacy of ACS 0001 Principal diagnosis and ACS 0002 Additional diagnosis. The content of a number of ACS has been incorporated into either ICD-10-AM or ACHI. This work minimises the number of standards required, allowing for ICD-10-AM and ACHI to stand alone where possible in the coding process.

The depth and scope of the scrutiny of changes emanating from both inside and outside the NCCH, should guarantee the relevance of the classification as the Australian Standard for the two years following 1 July 2010.

Brachytherapy planningIs it acceptable to assign Z51.4 Preparatory care for subsequent treatment as the principal diagnosis when a patient is admitted for brachytherapy planning or should the principal diagnosis be the cancer?

The NCCH advises that coders should assign the condition as the principal diagnosis for brachytherapy planning, as ‘planning’ is considered part of the treatment of the neoplasm. Z51.4 Preparatory care for subsequent treatment, not elsewhere classified is a non-specific code and the data collection is better served by coding the condition with the intervention code specifically describing the reason for admission.

ElectrochemotherapyWhat is the correct code assignment for electrochemotherapy?

Electrochemotherapy is a therapeutic approach which facilitates the delivery of non-permeant drugs directly into the interior of a cell. This is achieved by the local application of short and intense electric impulses that transiently allow permeation of the cell membrane; and transportation of molecules that otherwise would not be permitted by the cell membrane.
