Introduction to Portfolio

This portfolio is a culmination of my graduate-level Practicum I Training through

Grand Canyon University with 150+ hours completed at Hill Crest Behavioral

Health Services during the 2015 academic year. This portfolio contains experi-

ence gained through a sample case study of a client with identifying information

marked out. Entries include the client’s Psychosocial Assessment, Treatment

Plan, Progress Notes, Suicide Assessment, and Discharge/Safety Plan. It also in-

cludes a Professional Resume as well as professional certificates and member-

ships acquired.

Professional Goal Statement

I plan on becoming a Mental Health Counselor in the state of Alabama after obtain-ing my Masters Degree of Professional Counseling through Grand Canyon Univer-sity. After completion of Masters Degree, I plan to obtain a NCE Licensure as well as an ALC/LPC Licensure in the state of Alabama. Methods of practice that I plan to implement would be Dialectic Behavior Therapy and Cognitive Behavioral Ther-apy as well as incorporate an eclectic style therapy that combines several different practices in order to best suit the particular client.

Case Study I

Pt is a 29 year old African American Male that has been referred by JBS Probate

Court for admission. Pt arrived via transport from Birmingham City Jail. Patient

was arrested for THC Possession. Patient states that he and his mother got into an

altercation because she could not understand or agree with his view of the Bible.

Patient was pleasant and cooperative during intake as well as hyperverbal speech

at times. Patient has a diagnosis of schizoaffective disorder with Bipolar Type.
