Cardiographic Technician (230 Hours)The Cardiographic Technician program at Polaris Career Center is designed to prepare students to perform non-invasive cardiac testing procedures such as electrocardiograms, Holter and telemetry monitoring, and stress testing. Cardiographic technicians work in hospitals and cardiology offices to assist physicians in the diagnosis and monitoring of heart disease.Students will receive 130 hours of classroom and lab time. In addition, students will have the opportunity to gain practical experience through a 100-hour unpaid externship in a local clinical setting. Note: Externship hours must be completed during daytime working hours, Monday through Friday. Healthcare providers require immunizations for students participating in externships. The cost of the immunizations will be the student’s responsibility. Students will be required to have a background check. Upon completion, students will be eligible for credentialing as a Cardiographic Technician (CCT) with Cardiovascular Credentialing International. Prerequisite: Medical Terminology course or concurrent enrollment in a Medical Terminology course. Proof of successful course completion (transcript or certificate of completion) is required.

Dental Assisting (900 Hours)FINANCIAL AID ELIGIBILE PROGRAM – for those who qualifyA career as a dental assistant offers many opportunities and the field is growing faster than average occupations according to the United States Bureau of Labor Statistics. As valuable members of the dental care team, assistants greatly increase the efficiency of the dentist in the delivery of quality oral health care. In this course you’ll learn all the skills you need for entry-level employment.

Dental Assisting students will have the opportunity to gain practical experience through a 250-hour, unpaid externship (additional days may be required for the externship). Students will be required to have a routine physical, a criminal background check, and purchase their own uniforms. Immunizations will be required prior to participation in the clinical portion of the program. Students who complete the program will be eligible to take the Certified Ohio Dental Assisting Examination, and earn the credential CODA (Certified Ohio Dental Assistant). Students will also complete the Ohio Dental Assistant Radiographer initial training course. This initial training course satisfies the requirements of the Ohio State Dental Board and allows the student to earn the Ohio Dental Assistant Radiographer certification.

Cosmetology Academy (1500 Hours)FINANCIAL AID ELIGIBILE PROGRAM – for those who qualifyBe a cut above the rest in one of the fastest growing careers today!Do you enjoy working with people? Do you have an eye for beauty? Consider the tremendous career opportunities for students of cosmetology.Let the Polaris Career Center Cosmetology Academy prepare you for a challenging career in this exciting, cutting-edge field. Gain hands-on experience in the Polaris Career Center New Beginnings Salon while obtaining the industry knowledge necessary for your Cosmetology license.

Acquire skills commonly used in cosmetology in the following areas:

Graphic & Web Design Certification (600 Hours)FINANCIAL AID ELIGIBILE PROGRAM – for those who qualify

Graphic Design – 300 HoursA 300-hour program is designed to prepare the student to perform specific functions in the graphic design/production field. This will be achieved by studying the basics of design, typography, color theory and basic layout; then applying them in the computer disciplines: Adobe InDesign CS6 (page layout); PhotoShop CS6 (photo correction, retouching, etc.) and Illustrator CS6 (creating and modifying vector images). Advanced classes in electronic page layout and print production will round out preparation for jobs in the field.Skills developed include, but are not limited to:InDesign CS6

Web Design – 300 HoursA 300-hour course created to prepare the student for the web design field. This will be achieved by studying the basics of: HTML, Cascading Style Sheets (CSS), Content Management Systems, Social Media Networks, Flash animation and more, then application with Adobe Flash CS6 and Dreamweaver CS6. Design skills used from the Graphic Design section will be required before beginning the Web Design section.

*Adobe offers Associate Certification, nationally recognized for the above mentioned Adobe programs. The fee included in the tuition is for one exam. An introduction to the MacIntosh class will be included for those unfamiliar with the MacIntosh OS X v10.7 operating system. Prerequisite: General computer knowledge; keyboarding skills helpful.

Early Child Care Education/Child Development Associate (930 Hours)FINANCIAL AID ELIGIBILE PROGRAM – for those who qualifyThe Early Child Care Education/Child Development Associate (CDA) is the most widely recognized credential in early child care education (ECE) and is a key stepping stone on the path of career advancement in ECE. At Polaris Career Center, the CDA program is designed to prepare students to successfully complete the requirements for the nationally recognized CDA credential. The CDA credential is based on a core set of competency standards, which guide early care professionals as they work toward becoming qualified teachers of young children. CDA students will learn how to nurture the emotional, physical, intellectual, and social development of children. The CDA student will gain knowledge of how to put the CDA Competency Standards into practice and an understanding of why those standards help children move with success from one developmental stage to another.

Completing the CDA program will assist you in meeting current job requirements and advancing your career and salary. The CDA will also increase your knowledge of early childhood education and understand developmentally appropriate practice. In turn, you will increase your confidence and reinforce your commitment to early childhood education.Students will receive 450 hours of classroom training and have the opportunity to gain practical experience in a 480 hour unpaid required field experience student teaching at an area child care center. Note: field experience hours will vary between 6 A.M. and 6 P.M. and will be determined by the child care center offering the student the field experience. Upon completion of the CDA program, the student will be eligible to apply for the CDA credential. A routine physical and a criminal background check are required prior to student participation in the field experience. Immunizations may be required and the cost of these immunizations will be the students’ responsibility.Learn the CDA Observation Method, R-O-R (Review, Observe, & Reflect) and plan for the verification visit.

Medical Assisting (946 Hours)FINANCIAL AID ELIGIBILE PROGRAM – for those who qualifyThere is no better time than the present to enter the healthcare field. The Medical Assisting program is a great way to gain access to this exciting profession. The Medical Assisting curriculum prepares you to perform administrative, clinical and laboratory procedures for employment in a physician’s office, clinic, or hospital. The course will cover, but is not limited to, the following:

Medical Assisting students will have the opportunity to gain practical experience through a 160-200 hour unpaid externship. Students will be required to have a routine physical, and criminal background check. Some healthcare systems and physician practices may require proof of current immunizations for students participating in externships. Students must also complete the Hepatitis B series prior to participation in their externships. The cost of the physical and immunizations will be the responsibility of the student.The Polaris Career Center Medical Assisting Program is accredited by the Commission on Accreditation of Allied Health Education Programs (www.caahep.org) upon the recommendation of the Medical Assisting Education Review Board. (MAERB). Commission on

Heating, Ventilating, Air Conditioning and Refrigeration (HVAC/R) (600 Hours)FINANCIAL AID ELIGIBILE PROGRAM – for those who qualifyAs the population grows, so does the demand for new residential, commercial, and industrial climate control systems. The highly trained technicians in the Heating, Ventilating, Air Conditioning and Refrigeration (HVAC/R) profession maintain the complex HVAC/R systems used in residential, light industrial, and commercial buildings. Trained technicians have job stability and high earning potential.The HVAC/R program at Polaris Career Center will help you access this growing field. This program takes place in a comprehensive classroom and practical learning environment.

Natural Resources/Ranger/Basic Peace Officer Academy (600 Hours)FINANCIAL AID ELIGIBILE PROGRAM – for those who qualifyThe Polaris Career Center Natural Resources/Ranger/Basic Peace Officer Academy prepares candidates for police and natural resource law enforcement officer positions and other careers that require peace officer training. This 600-hour course is conducted under the auspices of the Ohio Peace Officer Training Commission. The instruction will be provided by those who have experience and expertise in the field of law enforcement, natural resources, and parks and recreation.College CreditStudents can earn technical credits toward an Associate of Technical Studies Degree in Park Ranger Policing at Cuyahoga Community College. Additional credits can be obtained throughout the state.

Medical Billing & Coding (with electronic health records) (900 Hours)FINANCIAL AID ELIGIBILE PROGRAM – for those who qualifyThis 900-hour program is designed for those who wish to gain employment in medical billing and

health information management in physician practices, outpatient clinics, billing companies as well as a variety of other medical facilities. Students will develop a thorough knowledge of medical terminology, anatomy and physiology, electronic medical records, practice management software, Microsoft Office, medical office administration procedures, medical procedural coding, medical diagnostic coding, hospital billing, and reimbursement procedures. Graduates will be prepared for entry-level positions as medical records technicians, or billing and coding specialists. Students will be required to complete a 180-hour unpaid externship during daytime working hours at the end of the program. Students will be eligible to participate in the National HealthCareer Association Certified Medical Billing and Coding Specialist exam (CBCS),

Nursing Assistant (STNA) (90 Hours)State Tested Nursing Assistants gain employment caring for diverse patient populations including the elderly in long-term care settings, assisted living facilities, home care environments and hospitals. Our program is state-approved and consists of mandatory attendance for classroom/clinical hours. The STNA exam is included in the fee and is offered to our students after successful completion of the program. Medicare approved Home Health Aide Certification and American Heart Association Basic Life Support (CPR) certification is also included. The 2-step Mantoux TB test is required before class begins. Immunizations may be required and the cost of the immunizations are the responsibility of the student. Employers may require a background check. Phlebotomy Certification (252 Hours)Students will be trained to perform a variety of blood collection methods using proper techniques and precautions including: vacuum collection devices, syringes, capillary skin puncture, and butterfly needles on adults. Both classroom instruction and clinical experience during a 100-hour externship at a healthcare facility will provide an opportunity for

the student to gain the skill and knowledge necessary to take the Certified Phlebotomy Technician (CPT) exam through the National Healthcareer Association (exam fee included). Note: Clinical hours will be scheduled during the day, and students will be placed in clinical assignments as space is available. Students will be required to purchase their own uniforms and have a criminal background check. Healthcare providers and labs require immunizations and may require blood titres (at the student’s expense) for students participating in externships. Students will be required to complete the three-part Hepatitis B vaccination series (at the student’s expense) prior to starting their clinical hours. Financial Aid (for those who qualify)A variety of financial aid options may exist to cover all or part of your training tuition. Your financial aid must be confirmed with Pat Carter, Financial Aid Coordinator 440-891-7668, before registration can be initiated. Students are responsible for submitting a Federal Application for Federal Student Aid (FAFSA). The FAFSA application is processed by the U.S.

Department of Education. Students should add the Polaris Career Center “School Code” (016745), which will then enable the financial aid office to have access to view student eligibility. Once the student applies, they should notify Pat Carter that they have submitted an application. It is recommended that the student start the application process prior to registering for a program.The following Federal financial aid options exist to cover all or part of your training tuition:

Pell Grant • The maximum award for 2013-2014 is $5,645 for a 900+ clock hour

program and $3,764 for a 600-clock hour program.

• Award amounts are based on the student’s EFC (Expected Family Contribution), calculated from the previous year’s income information submitted on the FAFSA (Free Application for Federal Student Aid).

• Pell Grant awards are applied to the student’s tuition account when enrolling in a full-time job training program meeting 600, 900, 930, 946, and 1500 clock hours.

• Students are required to self-pay the balance of the tuition not covered by the Pell Grant or other type of financial aid. A payment plan is available (see page 10).

• All Pell Grant funds are applied directly to tuition. The Pell Grant does not have to be repaid.

• To remain eligible for Pell Grant funds, a full-time student must maintain satisfactory academic progress (C average), in addition to a minimum of 90% attendance at all times. Students not in compliance will be responsible for paying the portion of financial aid expected to be received.

Step 2 – Go to www.pin.ed.gov and apply for a PIN number. If you are a dependent student, a parent must also apply for a PIN #. You may create your own PIN number and use it the same day to sign your FAFSA.

Step 4 – If you provided an email address on your application, you will receive within 3 – 5 days a SAR (Student Aid Report) from the U.S. Department of Education. If you did not provide an email address, the SAR will arrive by mail within 3 weeks. If you would like to check the status of your FAFSA, go to www.fafsa.gov.

William D. Ford Student Loan • Student loans must be repaid. Repayment starts six months after completing

school.

• Subsidized Loans are available for students with demonstrated financial need, as determined by federal regulations. No interest is charged while you are in school. The maximum amount that can be borrowed is $3,500 (less the processing fee).

• Unsubsidized Loans are not based on financial need. Interest is charged beginning with the first disbursement. The maximum amount that can be borrowed is $6,000 (less the processing fee).

Step 9 - Complete “Exit Counseling” before the last loan disbursement.

Federal student loan funds will be applied first toward outstanding tuition costs for the current loan disbursement period. Once student accounts are made current with loan funds, remaining loan monies due to students will be mailed within 10 business days in the form of a check to the student’s address of record.The Pell Grant and William D. Ford Federal Student Loans are each disbursed in two separate payments. Low Pressure Boiler License Preparation (60 hrs.)This course is intended for operators of boiler systems used in hotels, apartment buildings, schools, and other large institutions. The following topics will be covered: Boiler Operation Principles, Boiler Fittings, Feedwater System, Steam System, Fuel System, Draft System, Boiler Water Treatment, Boiler Operation Procedures, Hot Water Heating Systems, Cooling Systems, and Boiler Operation Safety. Ghost Hunt at the Mansfield Reformatory (6.5 hours)Here is your chance to do your own investigation of one of the most haunted buildings in America. The Ohio State Reformatory (OSR) in Mansfield is claimed to be one of the top haunts in America. It has been featured on TV’s “101 Things To Do Before You Die” on the Travel Channel. This is truly an opportunity not to be missed! Joining us on this ghost hunt will be an expert on the facility, and an experienced ghost hunter. He will give us a personal tour of the prison, pointing out details, and sharing his personal stories about the facility. After our tour, we will then start our own investigations. You are encouraged to bring any “tools of the trade” that you wish to work with: camera (digital or disposable), video camera, tape recorder (with extra tape and batteries), flashlight (with fresh batteries). We will see if we can photograph anything unusual (things not seen by the naked eye - for example: mists, streaks of light, orbs, faces, or body images). We can see if we can tape record sounds, voices, words that cannot be heard by the human ear. This building contains steep steps and uneven surfaces. Proper footwear is necessary for your safety. Basic Rider Course - Beginning Rider (BRC) (Approximately 16 hrs.)This class is your first step in learning to ride a motorcycle. It is intended for the true beginner who has never ridden a motorcycle and for the experienced rider looking to refine their skills and obtain a motorcycle endorsement. If successful, you will receive a completion card which you will take to the driver’s license bureau, and after paying the necessary fee, a motorcycle endorsement will be added to your driver’s license. You do not have to take another riding test at the driver’s license bureau. Classes consist of 4 hours of classroom training, followed by approximately 12 hours of actual riding on a controlled course. Courses are offered on a variety of days and times, so make sure you know your class dates and times when registering. For the first class, bring your driver’s license along with either your motorcycle endorsement or a valid temporary riding permit, which you obtain by passing the written test for the permit at the driver’s license bureau. Otherwise, you will not be admitted into the class and your tuition will not be refunded. Also, bring an ink pen, and dress casually. Basic Rider Course 2 – Experienced Rider (BRC-2) (5 hrs.)This course is designed for experienced riders who want to improve both their mental and physical riding skills to lower their risks while riding.The course consists of training and practice on your motorcycle on a controlled course under the knowledgeable and helpful eyes of trained instructors. If you ride two-up, your passenger may also enroll in this course. Street Rider Course – On Road (6 hrs.)If you desire individual on-road instruction then this course is for you!This Motorcycle Safety Foundation Course is ideal for riders wanting additional help interacting with traffic on actual streets. This course is limited to four students and is highly personalized based on participant skill and experience. Helmet communication with the instructor is provided. Polaris is the ONLY location in the United States to offer this training! Full protective riding gear must be worn as defined below. $199

NOTE: Full protective clothing is required for ALL riding courses as follows: DOT Approved Helmet, eye protection, long sleeved shirts and long pants without holes, full fingered gloves, and sturdy over the ankle footwear.

Skills Enrichment Program Learning LabThis self-paced computer lab offers students a chance to work in a non-graded setting on review for GED testing, WorkKeys testing, basic English and Math skills, and English language skills. AccreditationPolaris Career Center is accredited by the North Central Association Commission on Accreditation and School Improvement and is a member of the University System of Ohio. Polaris Career Center faculty meets the Ohio Department of Education’s requirements for career and technical instructors including experience in business and industry.

Canceled CoursesThe Adult Education Department must cancel courses that do not attain the required minimum enrollment. The Adult Education Department reserves the right to cancel, discontinue, postpone, or combine courses before or at the first class meeting. Attempts are made to contact all students whose courses have been canceled. In the event a class is canceled, a full refund of the registration fee will be given.

Disabled StudentsPolaris Career Center is totally accessible to disabled students with mobility impairments. To save time contact the Adult Education Office at (440) 891-7600 prior to arrival for parking instructions, reasonable accommodations, and/or elevator location information.

Senior Citizens DiscountSenior citizens will receive a 10 percent discount on the published tuition fee (discount does not apply to material fees) except where indicated. Note: Senior Citizens include persons over the age of 65 who can produce proof of age and identity.

School ClosingClosing of the daytime classes at Polaris Career Center DOES NOT necessarily mean that the evening classes will be cancelled. The decision to cancel evening classes will be made by the Adult Education administration by 3:00 PM. Students should listen to the radio or TV or call the school at 440-891-7600 for further information.

Berea and Strongsville School ClosingWhen the Berea and Strongsville City School District’s classes are cancelled or dismissed due to inclement weather, evening classes will not be held regardless of weather changes. Classes missed will be added at the end of the session.

Withdrawals and Refunds• Full refunds will be issued when a class is canceled by the Adult Education

Department, or fees may be applied to another class.• No refunds will be issued after the first class meeting. The Motorcycle

Basic Rider, Basic Rider-RR Course, and Experienced Rider Course tuition fee is non-refundable.

• A $10 cancellation fee will be withheld from all refunds except for Job Training Programs which have a $100 non-refundable fee.

• Books and supplies are non-refundable.
