    What is person centred planning?

    Person Centred Planning is a process of

    learning how a person wants to live and

    then describing what needs to be done to

    help the person move toward that life

    This is a fundamental Person Centred Thinking Skill. What we have seen

    over the years is that nearly anyone in need of long term services who is in

    circumstances where others exercise control, has what is 'important for' them

    addressed while what is 'important to' them is largely ignored or seen as what

    process of person centred thinking around the

    person. These are the very people who needgood person centred thinking the most.

    There are lots of tools now that the big whole life planshave been broken up into smaller and simpler to

    use tools of planning.

    The variety of tools makes it possible to select one thatmakes most sense to think about the most

    pressing issue in the personslife at a given time,and to build on and develop the person centred

    information.

    Part of the skill in person centred thinking is to selectthe right tool for the job.

    We have developed guidance and competencies tohelp people see how the tools fit with the 025

    Plan.

    Bringing people together with the person toshare and gather information is one very

    useful technique in person centredplanning, but there are lots of other useful

    ways of involving the person withoutenforcing participation in a meeting. With

    creative and imaginative thinking ways canbe found to involve the person which keeps

    them at the centre of planning withoutdoing things that we know will be

    unproductive.

    that is person centred?

    To help you think about your life and how you would liketo improve it

    You want people and services to start listening to what

    you want You have a big decision to make that you want to think

    about

    You are facing a big change in your life, like moving

    house or leaving school There is something important that you want to achieve

    You want the people who care about you to know aboutwhat matters to you

    LUCY'S STORY

    Lucy first had a one-page profile in Year 4, going into Year 5. She is now about

    to go into Year 8.

    Lucy talked about what made agood and bad day for her at school and whathelped her to have more good days. Lucys family shared their thoughts and

    said what they love about her. Time was spent with her teacher and with the

    teaching assistant who had supported Lucy for two years and had lots of

    information to share.They said what they like and admire about Lucy and whatthey thought was important to her at school and how best to support her. When

    her profile was written everyone checked it.

    Lucys profile was updated each year and when she was in Year 6 a profile for

    Year 7 was drawn up. It was shared with all of the teaching team at her new

    school so they immediately knew what worked for Lucy at school. Lucysprofile contributed to her Individual Education Plan.
