Permission to film, photograph and publish Consent form for parents/carers

The NSW Department of Education (department) endeavours to tell stories through films and photographs that inspire and engage the school community.

The department aims to help schools to share their work with their colleagues and the school community through the production of short videos and photographs.

What does it involve?Filming and photography allows schools to capture the magic of the classroom and beyond with a simple video camera, camera or device.

The films, short videos and photographs are shared on the department page, YouTube and other social media platforms.

What do you need to do?

If you are happy for your child to be filmed, interviewed and photographed as part of the iEntrepreneur project please complete the consent form overleaf and return to your Principal by the following date:

Permission to film, photograph and publishI authorise the department through its employees or agents to

1. make a video and/or audio recording, photographs of my child together with other audio visual material to be used in short films for the department to promote and share teaching and learning experiences

2. make use of or exhibit photos, and/or video/audio recordings, incorporating my child’s image, likeness and/or voice for the duration of the copyright which shall be the property of the department

3. make use of photos and/or video/audio recordings incorporating my child’s image, likeness and/or voice for the project which may be blurry, distorted or altered or used in composite form whether intentional or otherwise, that may occur or be produced in the processing of the project

4. use my child’s first name, age, class, school and video/audio recordings and/or photographs of my child, my child’s work and expressions of opinion in the short film for the project

5. provide or share any photos or video/audio recordings incorporating my child’s image, likeness and or/voice to professionally edit the recordings into short films.

I understand the places where this material may be published or disclosed include but are not limited to:

• public websites of the department including the school website and the staff intranet (staff only), blogs and wikis

• public websites where the videos will be uploaded such as YouTube

• department publications to promote the project including the school newsletter and school report, promotional material published in print, and electronically

• department and school social media networks such as Facebook, Twitter and YouTube

• local and metropolitan newspapers and magazines and other media outlets.

I am aware that when information is published on public websites and social media channels, it can be discoverable online for a number of years, if not permanently. Search engines may also cache or retain copies of published information. Published information can also be linked to by third parties.

I understand that consent to for my child to participate in filming or photography is entirely

voluntary. I have read this permission to film, photograph and publish form and fully understand the

meaning.
