1 Introduction The Audiometry Module displays and stores pure tone and speech audiometry test results, recorded with the MA 25, MA 27, MA 41 and MA 42 audiometers. The Audiometry Module can either be used in combination with NOAH or the MAICO Database. Using the MA 25 and MA 27 the software is capable of transferring data to the corresponding database after recording. Using the MA 41 and MA 42 the software enables displaying the status of the device, the current level and frequency, the selected transducer and the test. As results are stored in the device, they are displayed immediately by the software and can be saved to the database. This operation manual is valid for software version 1.29 or higher.

1.1 PC Requirements

The software can run on PC’s with Microsoft Windows XP SP3, Windows 7 32/64bit Professional or Ultimate or Windows 8 32/64bit Professional or Ultimate operating systems. Minimum PC requirements: CPU: Pentium P4 Processor 2.4 GHz RAM: 512MB (XP) / 1 GB (Win7) Suggested Display Resolution: 1024*768 CD-ROM Drive USB-Interface (1.1, 2.0, 3.0) Accessories: MA 25 or MA 27 MAICO Audiometer (extended version) MA 41 or MA 42 MAICO Audiometer Supported patient management system: MAICO Database, NOAH

2 Installation of the Audiometry Module The Audiometry Module is used in combination with NOAH or the MAICO Database. Prior to installation of the Audiometry Module, make sure the needed software is already installed. Close all open programs before starting the installation. Make sure that you are logged in as administrator or executing the installation setup as the PC administrator. Insert the CD stick into the computer, and double click on the executable file setup.exe to start the setup.

Note: If the MAICO Database is not already loaded on your computer, this is available from the included CD or USB drive.

1. Open the CD or USB drive. Select the folder titled Database or MAICO Database

(Figure 1).

2. Select setup.exe from the Database or MAICO Database folder, to install (Figure

3. Follow the on-screen commands to complete the installation of the MAICO Database.

Before the Installation Wizard for the MAICO Audiometry Module is started, a pop-up window will require confirmation of which database to install the module in.

Select Noah or MAICO Database and then Install to initiate (Figure 3).

Note: When installing this into the MAICO Database a window will appear to modify the current installation. Select Repair and Next to continue with the installation (Figure 4).

Follow the steps through the installation progress by clicking the Next button in the Welcome screen (Figure 5).

In the next step, the destination folder can be selected. Please use the automatically selected default folder: C:\Program Files (x86)\MAICO. The Audiometry Module is a 32 bit software. Keep the selected destination folder and click on the Next button. On a 64 bit PC, it needs to be installed to the directory C:\Program Files (x86)\... (Figure 6).

Click the Next button again to start copying the files (Figure 7). When the installation of the software is finished, the installation of the driver will start automatically.

Click on the Next button in the welcome screen of the driver installation wizard (Figure 8) and the Finish Button when the installation is done (Figure 9 and Figure 10).

In the next screen, keep the check “always trust software from “MAICO Diagnostic GmbH” and click the Install button to continue the installation of the MA 41 and MA 42 device driver (Figure 11).

When the installation of the device driver is finished, click on the Finish button of the next displayed screen. In the last step, the Setup is complete screen is displayed, click on the Finished button. The installation process is complete.

3 Connecting the Device to the PC for the first Time Connect the audiometer to the PC via the USB cable and turn on the audiometer. After starting up, a new USB device is detected automatically and a dialog to copy the driver to the windows system directory is displayed. The process runs automatically, please, follow the procedure. Keep the selected item “Yes, this time only” and click on the Next button (Figure 12).

Select automatic installation in the next dialog window and click the Next button. Wait until the MAICO MA4x family driver is installed and the operating system is updated (Figure 13).

When the installation of the driver is completed, click on the Finish button. The new hardware is now ready to use. When the Audiometry Module is started the next time, the audiometer is detected automatically. No additional setting changes are required.

4 The Audiometry Module Make sure that the audiometer is connected and the device is running before starting the software. Start a new session on the audiometer, before starting the Audiometry Module.

4.1 Starting the Audiometry Module

The Audiometry module can be accessed through NOAH or through the MAICO Database software. Upon installation of the software, selection of where to install the module is prompted, NOAH or MAICO Database.

4.1.1 NOAH Users

If the Audiometry Module is used in combination with NOAH, a new icon in the module bar will automatically appear after the installation. To start the audiometer a patient must be selected within NOAH. Then click on the module icon. It will take a moment for the module to start. An additional window with no information will open. Please do not close this window. It is required for the connection between NOAH and Audiometry Module.

4.1.2 MAICO Database Users

If the MAICO Database is used, the Audiometry Module is automatically assigned to the audiometry module start button (Figure 14).

Note: If an audiometry module for a past MAICO device has previously been installed, the Audiometry Module needs to be added as new program under the menu Extras. See Extras Menu on page 11 for more information.

Make sure that the audiometer is switched on and fully booted before starting the Audiometry Module. Depending on the selection of the audiometer, either the pure tone or speech audiometry screen will be displayed.

4.1.2.1 MAICO Database Menu Items

The MAICO Database menu items are displayed at the top row of the software (Figure 15). Description of each of these items is explained below:

New: Clears the demographic fields to enter a new patient into the database. Load: Loads an existing patient from the database. Delete: Removes a patient from the database. Measurements: Display saved sessions for a particular patient. Same function as Measurements button described on page 14. Quit: Closes the database application Programs Menu MAICO approved modules are run within the database. These programs are then listed under the Programs menu for selection and use (Figure 16).

Options Menu Hardware-Configuration, PC-Configuration and Language are listed under the Options Menu (Figure 17).

Hardware-Configuration: Select communication/comports between personal computer and MAICO audiometry and impedance hardware. Select printer type (color/grey scale) (Figure 18).

PC-Configuration: Select if the database is networked. If so, enter the pathway for the database in the Device field and click OK (Figure 19).

Language: Select display language: English, German or French. Also select date format: day/month/year or month/day/year (Figure 20).

Extras Menu New Program and the Program selection are listed under the Extras menu (Figure 21).

New Program: Select to add a module into the database. The new program will then be selectable under the Programs menu. To add a new program, follow the steps below: 1. Click on New Program. The following window will appear (Figure 22):

2. Type name of program as it should appear in the Programs menu and click OK. 3. The following window will appear (Figure 23). Open the AudiometryModule folder

and select the “AudiometryModule.exe” file for the program that is being added to the database. Click OK (Figure 24).

4. The following window will appear. If an argument for the program is required, enter the code here and click OK (Figure 25).

Program Selection: This will display which programs are activated by the program icons located at the top of the database screen. A program file (i.e.: .exe file) may be typed into a field to activate the appropriate icon (Figure 26).

The program can then be selected by pressing the icon at the top of the screen instead of going to the Programs menu (Figure 27).

Help Menu The Help Menu offers the selection of the Help and About (Figure 28).

4.1.2.2 Selecting Patient and Opening a Module from MAICO Database

Once the MAICO Database is opened, the main screen of the database is displayed (Figure 29).

To conduct a test using one of the installed modules, first enter the patient’s demographic information and click Save. Next, click on the icon at the top of the screen that corresponds to the audiometry module or click Programs and select the module. To choose an existing patient, use the Search button. This will display a list of patients already in the database. The search can be narrowed by typing information into one of the demographic fields and then selecting a search category (i.e. Surname, Birthdate or Client-No.). Select a patient from the list by clicking on the name. The patient’s information will then be displayed in the fields on the main database screen. Click the Measurements button to view past tests for the selected patient. A list of stored tests will be displayed.

The first time you enter the Module a pop up will appear to select the audiometer to be connected (Figure 30).

Once you enter the module, the Device Type selection can be adjusted in the Settings (Figure 31).

If you connect the MA 41 or MA 42, the status of the connection to the device is displayed at the right lower corner (Figure 32) of the software screen. If the connection is established, it turns green and the word Connected is displayed. If the connection to the device is interrupted, it turns red and Not connected is displayed. Click on the red circle to reconnect to the device. If reconnection does not occur, the audiometer should be restarted.

If the MA 25 or MA 27 is selected the status bar turns grey and the word Evaluation is displayed (Figure 33).

4.2 Pure Tone Audiometry When the Audiometry Module is started, the tone audiometry screen is displayed (Figure 34). If the device is connected to the PC, the PC software will change its screen according to the currently selected test displayed on the audiometer. On the pure tone audiometry screen, two audiograms are displayed - one for the left ear results and the other for the right ear results. The current level, frequency and selected signal are displayed above each audiogram. The selected level and frequency of the audiometer are also indicated by the position of the red or blue curser in the pure tone audiogram (PTA). The left and right PTA value is displayed between the audiograms, once all required frequencies have been tested and a dB value has been stored.

Below the audiogram tables, the masking values or SISI1 test results are displayed. This is based on what is selected between these boxes (i.e. masking levels or SISI). Between the audiograms are buttons of the selected transducer, test and signal configuration. These buttons cannot be clicked by the user. They display the status of the selected transducer of the connected audiometer. If the MA 25 or MA 27 is connected the selected transducer, buttons between the audiograms appear flat. The Audiometry Module does not receive the information

about the transducer from the device (Figure 33).

4.2.1 Store Data with the MA 41 or MA 42 Audiometer

If the user presses the Store button on the MA 41 or MA 42 audiometer, this value will immediately be displayed on the audiogram. The patient response and stimulus presentation will also be displayed within the module by turning the icon green. Once the user switches the audiometer to the speech test, the screen of the PC software will also display the speech screen.

4.2.2 Transfer Data from the MA 25 or MA 27 Audiometer

If the MA 25 or MA 27 is connected, the Request Measurement button appears (Figure 35). To transfer stored data from the device, click on Request Measurement and the tone audiometry values are displayed. Once the values are transferred, they are deleted from the device.

4.2.3 Editing Results

Recorded results on the PC can be edited if the Edit button is pressed. Select the transducer for the results that need to be edited (i.e. transducer button between the audiograms). To change the dB value, click on the audiogram at the level and frequency where the change needs to occur. If no result at this frequency and level has been stored, a new value will be created. Click the Edit button again when editing has been completed.

4.3 Speech Audiometry

If the user changes to the speech test on the MA 41 or MA 42 audiometer, or if the Speech button is pressed, the speech audiometry user interface will be displayed on the PC monitor. A speech audiogram will be displayed on the left side of the screen and a speech table is displayed on the right side. All results will be stored in the corresponding column of the table, and WRS2 results will be stored additionally in the speech audiogram. The current level of the speech signal and masking noise, if masking is activated on the device, is displayed. Also, the selected ear is displayed above the channel information and is indicated by the red and blue color of the channel information for the right and left ear (Figure 36).

The buttons below the speech table are available to select the transducer, the speech test (i.e. SRT3, WRS, etc.) or preview of the pure tone audiogram. For the SRT test, the level of the speech recognition threshold and the condition of the transducer and speech source (microphone, CD or Wave) is stored in the table. When WRS test is selected the level, percentage and condition will be stored. This occurs when the store button is pressed on the MA 41 or MA 42 audiometer. The level and percentage value are displayed in the speech audiogram and the speech table.

4.3.1 Edit Speech Test Results

Click on the Edit button in the speech test screen to start the edit mode, and click on R or L to select either the right or left ear results. To change the

value of a stored result, click on the entry in the column Condition/Word List of the speech test result table. Use the up/down arrow of your PC keyboard to change the signal level, page up/page down for masking level and right/left arrow for % of WRS. Make sure the correct speech test icon is selected when editing (i.e. SRT, WRS, MCL4 or UCL5. To add a new result, click on a blank line under the Condition/Word List column and use the appropriate keys listed above for entry. To delete a line, right click on the measurement and select delete. Click the Edit button a second time to terminate the edit mode.

4.4 Advanced Tests

If the MA 42 is used, the Audiometry Module can display and store results of advanced tests like the SISI (Short Increment Sensitivity Index), Decay or ABLB6 test. If an advanced test is selected, the required information is displayed by the PC software. E.g. if the SISI or Decay test is selected, the audiogram of the contra lateral ear disappears and the selected test is displayed. The results are transferred to the Audiometry Module if they are also stored in the device (Figure 37).

as a ladder diagram in the tone audiogram, and the tested frequency will be indicated by a green circle. Please note that for the ABLB test results for only one frequency can be stored.

4.5 Save and Load Results

Select the Save button if the session is completed and the results need to be stored in NOAH or the MAICO Database. To start a new session and clear the results in the audiograms, select the New button. In the appearing dialog box, there it is required to select Store or Do not Store the results before a new session is started. To load a previous session, press the Load button to load a stored session or to add a previous session to the currently displayed results. A dialog to load or add a session is opened; the sessions are organized by date (Figure 38). Select a session in the displayed dialog, select New or Add to load only the selected session or to add this session to the already displayed results.

The functions Store, Load or Delete audiogram are available under the menu bar item Audiogram. If the user selects Delete Audiogram, it is possible to select which part of the measurement should be deleted from the screen (tone, speech or other additional results). The Previous Sessions! Item in the menu bar is another place to load or add stored sessions.

4.6 Notes

The tone and speech screens display a line to add notes to the current measurement. These notes will be saved as additional information and displayed when the session is loaded. Additionally they will be added to the printout.

4.7 Settings

Within the menu bar is a Settings menu. This selection allows Diagram Settings, Printer Settings, Device Type and Connection to the Device to be modified (Figure 39 and Figure 40).

Diagram Settings allows for selection of Degrees HL, Speech Sounds and Symbol selection. Degrees HL allows for selection of Children, Adults or Off (Off is the default selection). This will display a severity scale on the audiogram chart. Speech Sounds provides the frequency specific information of consonants and vowels to be displayed on the audiogram. Lastly, regional symbol settings can be changed by clicking on the item Symbols. International Symbols according the norm IEC 60645-1, US Symbols, UK symbols or German symbols can be selected. Printer Settings: Printer Settings allows clinic and tester information to be displayed on the printout. A window is displayed to enter the name, address and phone of the site. In addition, a picture or logo can be added to the printout by selecting the Load icon. This allows the selection of a picture/logo to be loaded from the computer. The Examiner field allows names to be entered for the printout. To enter the name, put the cursor in the box and type in the name. When there is more than one name to enter, select No Examiner so the field becomes blank and type in the next name. Selection of a default name on the printout is available by using the drop down arrow and selecting a name within the list. A checkmark will be shown beside the default name selected. When more than one examiner is available for selection, a pop up can be incorporated in the module start up for the examiner name to be selected. Select the radio button beside Ask at module start to activate this option. When all settings are established, select Ok. To leave without saving, select Cancel (Figure 41).

Device Type The Device Type can be changed as described in Section 4.1.2.2 Figure 30. Connection to the Device: In the settings menu, the connection to the device can be deactivated or activated by selecting Connection to Device Off or On (Figure 42).

4.8 Transferring a Sessions from the MA 41 or MA 42 Audiometer

Full sessions stored in the patient list of the MA 41 or MA 42 can be transferred to the Audiometry Module. Connect the audiometer to the PC and make sure that a new session is started in the Audiometry Module. Enter the Patient List of the Audiometer, select a Session and load the session. All the results of the session will be transferred to the PC and also displayed in the Audiometry Module. The results can then be stored in the PC Database. If there are already results stored in the MA 41 and MA 42 Audiometry Module, a message is displayed on the PC screen to store the current results before loading the new results from the device into the Audiometry Module.

Select Store and click the OK button, to store the current results of the Audiometry Module in the connected database (Figure 43). A new session will be started and the results from the device will be transferred and displayed. Select Do not store, to delete the currently displayed results without saving. 5 Ownership, Warranty and Disclaimer

5.1 Ownership

The Audiometry Software (hereinafter the “SOFTWARE”) is solely owned by MAICO Diagnostics GmbH, Sickingenstr. 70-71, D-10553 Berlin, Germany. By purchasing the SOFTWARE the buyer is entitled the right of usage, but not ownership of the SOFTWARE. The SOFTWARE is to be used in accordance to the agreed terms of usage provisioned by MAICO.

5.2 Copyrights

MAICO’s ownership of the SOFTWARE covers worldwide and is therefore, protected against any unauthorized copying of the SOFTWARE. Non conformity of use of the SOFTWARE is strictly prohibited.

5.3 Limited Warranty

MAICO warrants that any physical media and physical documentation provided by MAICO are free of defects in materials and workmanship. This limited warranty is effective for a period of ninety (90) days from the original purchase date. If MAICO receives notification within the warranty period of defects in materials or workmanship and determines that such notifications are correct, MAICO will replace defective media or documentation. Do not return any product until you have obtained authorization to do so from your supplier. The entire and exclusive liability and remedy for breach of this limited warranty shall be limited to replacement of defective media or documentation supplied by MAICO, and shall not include or extend to any claim for or right to recover any other damages, including but not limited to, loss of profit, data, or use of the SOFTWARE, or special, incidental or consequential damages, or other similar claims, even if MAICO has been specifically advised of possibility of such damages. In no event will MAICO’s liability for any damages to you or any other person ever exceed the lowest list price or the actual price paid for the license to use the SOFTWARE, regardless of the form of the claim.

5.4 Disclaimer

MAICO covers; including but not limited to; all warranties, representations and terms and conditions, either expressed or implied; under specified terms of use ans application of the SOFTWARE for its specific purpose. All other terms and conditions shall not apply. Furthermore, MAICO does not guarantee that the SOFTWARE or Documentation is free of bugs, or fulfill the relevant standards, requirement or needs of a user. In this case, all the warranties, guarantees and terms and conditions on all MAICO delivered physical disk and documentation shall be limited to the 90 days warranty period. MAICO is not liable for any third party’s product, disks, software or documentation that is used in conjunction with MAICO’s SOFTWARE or programs, but is not directly manufactured or supplied by MAICO.

5.5 General Terms and Conditions

Any change made to this Agreement shall be notified in writing, agreed and signed between both parties, namely the purchaser of the SOFTWARE and a representative of MAICO. In the event that the essential purpose of the above remedy (limited warranty) is

not fulfilled, all other limited liability including the liability limits and exclusions of damage claims shall continue to apply This Software License Agreement shall be interpreted and construed according to, and governed by, the laws of Jurisdiction of Federal Republic of Germany. In the event that any legal or commercial dispute or controversy arising out of, or relating to this agreement; provided MAICO is in all case violated of the rights, to the SOFTWARE or other intellectual property protection right related to the SOFTWARE; shall be presented under the Jurisdiction of Federal Republic of Germany in the court of Berlin. The SOFTWARE is protected under both Copyright Law and the International Copyright Treaties. Copying of the SOFTWARE is strictly prohibited except for copies made of the SOFTWARE for backup purposes to protect data loss. The SOFTWARE can be used by any number of users, on any number of computers, and in any place, provided that they are not on more than one display screen at the same time.
