ORDER
IT IS ORDERED that LaKe Village Water Association,
Inc., ("Lake Village" > shall file an original and 3 copies
of the following information with the Commission with a copy
to all parties of record by May 1, 1985. Lake village shall
also furnish with each response the name of the witness who
will be available at a public hearing for responding toquestions concerning each area of information requested. Ifneither the requested information nor a motion for an exten-
sion of time is f i led by the stated date, the case may be
dismissed.
1 ~ Provide copy of Preliminary Engineering Report on
the project proposed in this case. Report to include a
narrative description of proposed construction and an
enumeration of the facts relied upon to show that the
proposed new construction is or will be required by public
convenience or necessity.2. Provide hydraulic analyses, supported by computa-
tions and actual field measurements, of typical operational
sequences of the existing water distribution system. Compu-
tations are to be documented by a schematic map of the sys-
tem that shows pipeline sizes, lengths, connections, pumps,
water storage tanks, wells, and sea level elevations of key
points, as well as allocations of actual customer demands.
Flows used in the analyses shall be identified as to whether
they are based on average instantaneous flows, peak instan-
taneous flows, or any combination or variation thereof. The
flows used in the analysis shall be documented by actual
field measurements and customer use records. Justify fully
any assumptions used in the analyses.
3. Provide a summary of any operational 6eficiencies
of the existing water system that are indicated by the hy-
draulicc
analyses or that are known f rom experience.
4. Provide hydraulic analyses, supported by computa-
tions and field measurements, demonstrating the appropriate-
ness of the engineering design of the proposed construction.
Justify fully any assumptions used in the analyses.
5. Provide a pressure recording chart showing the
actual 24-hour continuously measured pressure available at
Lake vilLage's 47,000-gallon water tank in the vicinity of
Curdsville. Identify the 24-hour period recorded, the exact
location of the pressure recorder and the sea level
elevation of the recorder.
6. Provide a pressure recording chart showing the
actual 24-hour continuously measured pressure available atthe City of Harrodsburg's water transmission main in the
vicinity of the intersection of Highway 33 and Chatham Road.
Identify the 24-hour period recorded, the exact location of
the pressure recorder and the sea level elevation of each
recorder.
7. Provide a pressure recording chart showing the
actual 24-hour continuously measured pressure available on
the discharge side of Lake village's pressure reducing valve
on Highway 33 near Danville. Identify the 24-hour period
recorded, the exact location of the pressure recorder and
the sea level elevation of the recorder.
8. Provide a pressure recording chart showing the
actual 24-hour continuously measured pressure available on
the existing 6-inch water line on Highway 33 in the vicinityof Hughley Road. Identify the 24-hour period recorded, the
exact location of the pressure recorder and the sea level
elevation of the recorder.
9 ~ Explain how the operation of the proposed pump
station is to be controlled. Provide a copy of the pump
manufacturer's characteristic (head/capacity) curve for the
proposed pumps.
10. Explain the purpose of the pressure reducing
valves in the proposed master meter vault on Highway 33 atChatham Road. Explain how the pressure reducing valves are
to be monitored and maintained after they are sealed in the
proposed master meter vault.
ll. Provide a detailed breakdown with a full descrip-
tion of major expenses included in the following maintenance
accounts for the test period. (Items less than 850 may be
grouped and repetitive or like items may be consolidated and
shown as a single or combined amount.)

12. Provide a detailed breakdown of the transportation
expense in the amount of 82,300 for the test period.
13. What criteria does Lake Village use in capitaliz-ing expenditures?
14. Provide the name(s) of all employee(s), a ful1
description of the duties performed, estimated time per week
or test period performing those duties, and the hourly wage
and/or base salary paid for their duties. (List the
employee(s) by account in which his (their) salary isincluded and provide a total of that account. Any allocation
of salaries between accounts should also be provided ~ )
15. Is Lake Village current in its payments to the
FmHA? If not, then does the FmHA consider Lake Village to be
delinquent?
16. Provide an explanation of and interest rate forthe following test period liabilities:
