Nurse Family Partnership
• An evidence-based nurse home visiting program for first time mothers with 37 years of research.
• A public health
nurse visits the mother from early pregnancy until the baby turns 2 years-old.
Babies First
• Focuses on child growth, development, health, safety, and parent child interactions, including attachment.
• Home visits for children up to age 5 that are considered high risk.• Based on Social,
Emotional, and Medical Risk Factors
CaCoon
• A home visiting program that focuses on serving children with special needs and supported by OHSU.
• Children ages 0 to 21
years of age are served by a public health nurse.
Maternity Case Management
• A home visiting program that focuses on helping prepare a pregnant mother for motherhood and pregnancy and labor and delivery.
• This program begins during the prenatal period and is completed up to 2 months after the baby is born.
Overall Goals
Decrease visits to the Emergency Department Increase rates of immunization, annual well-child, and
dental care visits. Get women into early prenatal care by a health care
provider Increase overall access to care through referrals and
resource education. Reduce early preterm births for high risk women Encourage overall personal growth and goal
achievement for women
How you can help?
Refer, Refer, Refer Shred all old referral form and replace with the new Central
Oregon Referral Form Make sure to get client’s signature on new form so we can
refer to other programs if needed. Provide the nurse with as much information to help us to
find the right fit for the client. Make no promises.
We will see as many as possible but, caseloads fill up fast
