Nerves of the upper limbMedian nerveThe nerve can be damaged by:
●● penetrating injuries of the arm and forearm;●● lacerations at the wrist;●● wrist dislocation;●● carpal tunnel compression.
Sensory loss The median nerve is sensory to the palmar aspect of the thumb, index and middle fingers, and the dorsal aspect of the distal phalanx and half of the middle phalanx of the same fingers. It also supplies sensation to the radial side of the palm.
Motor malfunction If the injury is at wrist level, there is:
●● wasting of the muscles of the thenar eminence;●● loss of abduction and opposition of the thumb
(Fig. 3.37).
If the injury is above the antecubital fossa (elbow level), there is:
●● wasting of the muscles of the forearm and thenar eminence;
●● loss of flexion of the thumb and index finger;●● holding of the hand in the ‘benediction’
position with the ulnar fingers flexed and the index finger straight).

●● a ‘claw’ hand, in which the ring and little fingers are hyperextended at the metacarpo-phalangeal joints and flexed at the interphalangeal joints (Fig 3.38).
●● an absence of both abduction and adduction of the fingers, with a positive Froment’s sign (inability to hold a sheet of paper between thumb and index finger) (Fig 3.39).
If the injury is at elbow level, there is:
●● wasting of the intrinsic muscles of the hand and hypothenar eminence;
●● a claw hand without the flexion of the terminal interphalangeal joints because of paralysis of half the deep flexors of the fingers;
●● a positive Froment’s sign.
If the injury is well above the elbow, there is:
●● wasting of the intrinsic muscles of the hand and hypothenar eminence;
●● loss of flexion of the little and ring fingers;

Ulnar nerveThis nerve can be damaged by:
●● compression or injury at the elbow;●● penetrating injuries of the arm or forearm;●● fractures of the medial epicondyle;●● lacerations at the wrist;●● long-standing marked cubitus valgus.
