BREAST CANCER SURVIVORSHIPPANEL DISCUSSION

The Institute of Medicine in its 2005 report: Lost in Transition:

From Cancer Patient to Cancer Survivor

The Four Components of Survivorship Care recommended include:

1. Prevention and Detection of New Cancers2. Surveillance for cancer recurrence or new cancers

and interventions for consequences of cancer and its treatment.

The 2005 Institute of Medicine Report3. Psychological Distress Management4. Coordination between Specialists and Primary

Care Providers to Ensure that Survivors’ Health Care Needs are Met

To address these components of care it was recommended that Cancer Care Specialists provide patients, care givers and PCPs with a Survivorship Care Plan.
