Nigeria Family Planning Commitment

The FP2020 commitment for Nigeria is to achieve a contraceptive

prevalence rate (CPR) of 36% by 2018.

Nigeria also committed to increasing reproductive health

commodity funding, improving equity and access to family planning,

and training at least 3,700 community health workers.

Contraceptive market share is determined based on Couple-Years of Protection (CYP) : The CYP is calculated by multiplying the quantity of each method sold or distributed to clients by a conversion factor, to yield an estimate of the duration of contraceptive protection per unit of the method. The CYP for each method/outlet type are then summed over all methods to obtain a total CYP figure.

1. What types of outlets in the public and private sectors are carrying modern contraceptive methods?

1. PPMVs account for nearly three-quarters of outlets providing at least one modern contraceptive method. This ranges from 60% in the NC Zone to 81% in the SW Zone.

2. After PPMV’s, public health facilities (13%) and private health facilities (9%) are the next most common outlets providing at least one modern contraceptive method.

3. Public sector outlets were a considerably smaller proportion of the contraceptive market composition in Southern Zones compared to Northern Zones.

4. CHWs stocking modern contraceptives were common only in the North Central Zone.

5. General retailers did not typically stock modern contraceptive methods except in the SE zone, where this outlet category comprised 6% of the contraceptive market.

2. What proportion of public and private sector outlets are stocking selected modern contraceptive commodities and providing a range of methods?

1. LARC availability is largely restricted to public health facilities and private for-profit health facilities. However, even in these outlets, availability is below 25%.

2. Over 40% of CHWs were found to be stocking injectables. However, an additional 20% were out of stock when surveyed but had injectables available in the past three months and about 40% neither had injectables available on the day of the survey nor stocked the method in the last three months.

3. About half of public sector outlets did not have injectables currently available, with about 8% of those currently out of stock having carried the method in the last three months. Over 80% of private sector outlets did not stock injectables, with about 8% of those of having carried injectables in the last three months.

4. Emergency contraceptives were only available in the private sector.

6. Only the NC, NE and SS Zones had one-third or more public sector outlets stocking three or more modern methods. No private sectors had more than 20 percent of outlets stocking three or more modern methods.

7. More than 80% of pharmacies and PPMVs stock at least one modern contraceptive method; three-quarters of pharmacies stocked three or more methods and only 10% of PPMVs stocked three or more methods.

3. What is the relative market share for each contraceptive method and for each outlet type?

1. About three-quarters of the contraceptive market share was from private outlets and one-quarter from public outlets. Private health facilities contributed 44%, public health facilities contributed 26% and PPMVs contributed 26% of total volume of CYP distributed in Nigeria.

2. Contraceptive implants accounted for 42%, male condoms accounted for 23%, IUDs accounted for 15% and contraceptive injectables accounted for 9% of the total volume of CYP distributed.

3. By Zone, the NE and NW Zones had larger market shares for the public sector (about 60%) compared to the private sector. All other Zones had at least three-quarters of the contraceptive market share accounted for by the private sector.

4. LARC methods accounted for 69% of the total volume of CYP distributed in the North Central Zone, 68% in the South South Zone, 42% in the North West Zone, 27% in the North East Zone, 24% in the South East Zone and 21% in the South West Zone.

4. What is the consumer price of modern contraceptive methods among private sector outlets?

1. There was little price variability among contraceptive methods between private outlets.

2. The median price for equivalent methods tended to be higher in the SE and SW Zones.

3. In terms of CYP, male condoms and implants were almost seven times the cost of IUDs and injectables and oral contraceptives were more than five times the cost of IUD.

5. What is the availability and readiness of selected outlet types for performing contraceptive services?

1. Over half of public and private facilities and CHWs offer contraceptive injection procedures. Only one-fifth of public facilities and one-third of private facilities offer contraceptive implant and IUD insertion procedures.

2. More than half of public health facilities and private health facilities were service ready to provide contraceptive injection procedures according to service readiness criteria. Many public facilities did not have trained and credentialed staff available and many private facilities did not have injectable commodities available.

3. More than two-thirds of public health facilities and private health facilities were service ready to provide implant insertion procedures according to service readiness criteria. Many of these outlets did not have the implant commodities available.

4. About 47% of public health facilities and over half of private health facilities were service ready to provide IUD insertion procedures according to service readiness criteria. Many of these outlets did not have the IUD commodities available.
