Mini Grant Overview
Purpose of Mini Grants
• An opportunity to involve community members to build upon existing community resources consistent with the Commission’s Strategic Plan.
• Fund projects benefiting pregnant women, children prenatal through age five and their families in Yuba County.
• A total of $30,000 for fiscal year 2015-16 has been allocated to mini grant projects, $5,000 is the maximum allowed for individual projects.
• Individuals, groups, public funded agencies, faith-based, non-profit and for-profit organizations are all eligible to apply.

Submitting your Application

Funding ProcessWhat Next?
1. In early April, applications will be reviewed by the Application Review Committee and selected applications will then be referred to the Commission.
2. Each Commissioner will read and score all referred applications.
3. Commission staff will calculate the average score for each applicant and present the average scores to the Commission on Thursday, April 23rd at 3:30 p.m. The Commission will review the scores and approve what applications to fund and the amount to fund.
4. If the total amount of complete applications received is more than the allocation amount for mini grant funds, the Commission can vote to increase the amount or consider the recommendation provided by the Application Review Committee to provide partial funding or no funding to low scoring applicants.
5. Applicants will be notified of Commission decisions by May 1st .

What to Expect if yourProject is Awarded
Responsibilities of Awarded Grantees
Timeframe: Grantees will have approximately a year to complete the project. The term of this mini grant funding is for fiscal year 2015 to 2016 and the project must be complete by end of the fiscal year.
Contract: Authorized signature of the terms of the agreement is a legal contract. To fully execute the agreement grantees must submit the certificate of insurance and endorsement page.
Budget: Upon contract execution 75% of approved mini grant award will be issued. Mini grant funds will not be used to replace or supplant funding from other sources. Keep careful records, including receipts to document expenses that are in accordance with the approved application narrative and budget.
What to Expect if yourProject is Awarded (cont.)
Responsibilities of Awarded Grantees
Delivery: Participate fully in Commission evaluation efforts and site visits applicable to the mini grant. Add the statement, “IN PARTNERSHIP WITH FIRST 5 YUBA” and include the First 5 Yuba logo to any materials written, publicized, produced or distributed by the approved project.
Compliance: Submit the Final Report by Thursday, June 30, 2016. The Final Report must include the following:
An invoice with receipts that reconcile the original 75% received or a check of any mini grant funds not expended *
An invoice with receipts that reconcile the remaining 25%
A post evaluation survey30JUN
FINAL REPORT
* After the reconciliation of the original 75% received, the balance of 25% will be paid to the organization/group upon receipt, review and approval of final report and all expenditure backup documentation. Final payment amount will not exceed the 25% balance or the amount actually expended for the project.
