The UK medical device market isvalued at over £8 billion, the thirdlargest in Europe after Germanyand France. Medical device com-panies represent an importantemployment sector directly pro-viding around 46,000 UK-basedjobs, and many more related jobsin ancillary services.

The UK has traditionally hadstrong business links with MiddleEast countries, particularly theGulf coast states of UAE, Qatar,Saudi Arabia, Kuwait, Bahrain andOman; but also Lebanon, Israeland Egypt in the wider region.

This issue looks at the strongbusiness relationship the UK en-joys with the GCC countries in thefield of medical technology, andsome of the British manufacturersdoing business in the Middle East;Bedfont Scientific, Durbin PLC,and Swann-Morton.

Great Ormond Street Hospitalfor Children NHS Trust(GOSH), a specialist tertiaryhospital situated in the heart ofLondon, is recognised as oneof the world's leading children'shospitals.

Founded in February 1852, it wasthe first children's hospital in theEnglish-speaking world, and hasgrown to become an internation-ally famous centre of excellencein child healthcare. Much haschanged in medicine over thattime but GOSH is committed todelivering the best and most up-to-date treatment now and in thefuture.

Clinical services

GOSH is a service within the Na-tional Health System. Children are

referred from other hospitalsthroughout the UK and from over-seas where treatment for highlycomplex or rare conditions is notavailable.

GOSH is renowned for innovativeand latest developments inhealthcare and has the widestrange of children's clinical special-ists in the UK. It is also the largestcentre for heart and brain surgeryin the UK, runs the largest paedi-atric intensive care unit, and is thethird largest centre for childrenwith cancer in the world. The hos-pital has carried out over a thou-sand bone marrow transplants,and research into how such trans-plants could be carried out betterwhen a partially matched parentaldonor is used. A non-invasivemethod of replacing pulmonaryheart valves developed at GOSH

is now being rolled out to othercentres. GOSH has also pio-neered the use of gene therapyfor treatment of complex condi-tions.

In a world first in children, Britishand Italian doctors have trans-planted a new airway (trachea)into a child and used the child’sown stem cells, in the body, to re-build it. The surgery was carriedout at Great Ormond Street.

The donated trachea was strippedof the donor’s old cells, down tothe inert collagen. The child recip-ient’s bone marrow stem cellswere collected, and applied to thegraft in situ in the body, to rebuildthe cellular component of the tra-chea. Thus the child’s own cellswill be used to make the new air-way sealed and effective.

This is the first time that this hasbeen performed in a child. It isalso the first time the entire lengthof the trachea has been trans-planted.

The application of this technologyshould reduce greatly the risk ofrejection of the new trachea, asthe child’s stem cells will not gen-erate any immune response.

Research and Education

Working closely with researchpartner, the Institute of ChildHealth, GOSH pushes the bound-aries of paediatric treatment, re-search and training. The hospitalis the UK’s only Specialist Bio-medical Research Centre in pae-diatrics.

The hospital is also at the fore-front of paediatric training in theUK. We train more children’snurses than any other hospitaland play a role in training paedi-atric doctors. Nursing practice is

advancing rapidly and with manynurses also supporting clinical re-search activity and leading spe-cific nursing care researchprogrammes

Gulf Office

As a hospital with a worldwidereputation, GOSH helps to pro-mote the best quality paediatrichealthcare management in theGulf through collaboration withother healthcare institutions.Treating more than 3,000 childrenannually from the Middle Eastalone, GOSH has a Gulf regionaloffice based in Dubai HealthcareCity The office is used for man-aging referrals, patient liaison, de-veloping consultant visitingprogrammes in the region andproviding management services.

GOSH has recently extended thevisiting programme to include clin-ical nurse specialists and dieti-cians. Consultants who havevisited cover a wide spectrum ofspecialities: radiology, respiratory,

endocrinology, gastroenterology,nephrology, anaesthesiology, car-diology, rheumatology, oncology,neurology, dermatology, neuro-surgery, haematology.

Children and their families benefitfrom the establishment of the Gulfoffice by having better access tosupport before and after treatmentat Great Ormond Street Hospital.This support ensures a better ex-perience for children and familiesduring treatment at GOSH, andbetter outcomes, by providing ap-propriate support when the childreturns home.

The establishment of a regularconsultant-visiting programmehas meant that specialists fromGOSH come to the region on aregular basis to see children in thecomfort of their own environmentrather than have the family travelto the UK. Consultant's insight tothe services provided locally is in-valuable when planning children'sdischarge back to the region fromUK and the teams work together

to improve the overall quality ofcare.

Support for Families in London

The Harris International PatientCentre is a dedicated wing of thehospital where we care for thechildren from overseas who re-quire specialist treatments un-available in their own countries.Some private patients from theUK are also seen at the Centre.The Division has 130 staff consist-ing of doctors, administrators, ad-vocates as well as nursing andspecialist nursing staff.

An advocacy service aims to sup-port the international patients andmaximizes the resources avail-able for families. Its core is to as-sist patients and their families tocommunicate effectively with

members of the multidisciplinaryteam within GOSH. The advo-cates bridge the linguistic and cul-tural gaps between theinternational patients and theteams involved in the care of thechild. Arabic TV channels, news-papers, clinical informationleaflets and parent informationbooklets are available and halalfood is served. The hospital em-ploys an Imam for spiritual guid-ance and support.

The Child First and Always

‘The child first and always ‘ hasbeen the hospital’s motto for al-most 100 years. A recent survey,conducted by Ipsos MORI,showed that 94 per cent of pa-tients and their parents were ei-ther ‘very’ or ‘fairly’ satisfied withtheir last visit to GOSH. The vast

majority (96 per cent) of thosequestioned had confidence andtrust in the doctors, and the sameproportion said they would belikely to recommend the hospitalto a friend or relative if their childneeded treatment.

That focus and commitment re-mains the same today with an em-phasis on not only looking afterthe child’s medical condition butalso their overall wellbeing andthat of their family. This character-izes GOSH’s approach today andinforms the vision of the future.

Mr WR Swann, Mr JA Mortonand Miss D Fairweatherfounded the business in August1932 to manufacture and sellrazor blades.

By 1957 Swann-Morton was man-ufacturing over 38 million bladeseach year. During the mid 40's MrJA Morton decided to leave thecompany to pursue other interestsleaving Walter Swann and DorisFairweather as remaining co-di-rectors. Mindful of the future secu-rity of the company, the workforceand its founding principles a trustwas formed to administer thecompany within which the em-ployees had a 50% share and theremaining 50% placed in a chari-table trust. It is this unique culturethat inspires quality and commit-ment, and Swann-Morton's staff isvery proud to be a part of this.

In the 1960's demand for ster-ilised surgical blades increasedand Swann-Morton developed,with the expertise of UKAE atWantage, a sterilisation processusing Cobalt 60 gamma radiation.It was one of the first commercialplants of its kind in the world andenabled Swann-Morton to becompletely self sufficient for all itssterilisation requirements.

Today the irradiation and microbi-ological facilities are offered undercontract to other healthcare man-ufacturers from Swann-Morton(Services) Limited and Swann-Morton (Microbiological Labora-tory Services) Limited.

At the turn of the new centurySwann-Morton marked itsprogress and achievements byembarking on an extensive ex-pansion programme involvingmajor construction works to in-crease despatch, sterile storage,laboratories ,production and officefacilities.

The further addition of CobbWorks across from the main sitein September 2006 providing pur-pose built space for the R& D andengineering departments andCygnet works in October 2008some 400 metres away providingmuch needed storage for raw ma-terial stocks underlines a policy ofongoing investment.

The desire for on-going improve-

ment, the hard work ethic of all itsemployees and a policy of promo-tion from within will sustain it as aworld leader and undoubtedlytake Swann-Morton to evengreater heights to the benefit ofthe company, its employees andtheir customers.

MEH: Please outline your busi-

ness (market share, exports etc)

in the UK and internationally.

Chris Taylor (CT): Swann-Mortoncurrently holds the surgical bladeand scalpel contracts with NHSSupply Chain, NHS NationalServices Scotland and WelshHealth Supplies exclusively. In ad-dition they also hold supply con-

tracts with many of the leading pri-vate hospital groups thus enablingthem to supply to all Private andPublic Hospital facilities through-out the UK. Swann-Morton have exportedsince the 1930’s when they soldlarge volumes of Razor blades tothe USA. They currently exportsurgical blades and scalpels ei-ther directly or indirectly to over100 countries around the Worldthrough an experienced and loyalnetwork of distribution partners.They have their own sales anddistribution office in Peynier,France looking after the Frenchmarket which is the No.1 exportdestination and also in Poznan,Poland. They are particularlystrong within Europe, Australiaand South Africa with a share ofthe Hospital market in excess of70%.

MEH: What business do you cur-

rently have in the Middle East,

and what are your plans for future

expansion?

CT: Swann-Morton has a relation-ship with the Middle East whichdates back to the 1960’s howeverit was in the mid 1980’s after thecompletion of an intense in-housemachine building program bySwann-Morton’s experiencedteam of engineers that they feltconfident enough to place a realfocus on the region and to expandtheir existing network of distribu-tion. The 1986 Arab Health showheld in Jeddah, Saudi Arabia wasthe launch pad for this initiativeand was followed with attendanceat the Saudi Medicare and subse-quent Arab Health shows in Dubaiwhich still forms the main meeting

room for the distributors and cus-tomers.

Swann-Morton’s sales to the re-gion increased by some 15% lastyear even against a backgroundof Global recession and this fol-lowed a 16% increase in the pre-vious 12 months.

Current sales to the region con-tinue to move in the right direction(in fact the company recently se-cured a $33,000 USD con-tractwith the MOH in Bahrain) asnew opportunities develop outsideof the traditional MOH and MODtender system and with the recog-nition of the quality embodiedwithin the Swann-Morton products

supported by reliable delivery andexcellent customer service.

Although parts of the region arestill very price conscious making itdifficult to secure some of themore volume based contractsSwann-Morton aims to increasethe awareness and availability ofthe more specialised items in theirrange which at the moment onlyrepresent a small proportion oftheir Middle East sales.

MEH: How can the GCC states

benefit from your products?

CT: Since several of the leadinghealthcare facilities in the Worldare now based within the Middle

East the market has becomemore receptive to some of themore specialised products withinthe Swann-Morton range.

As surgical procedures becomemore specialised in order to ben-efit the patient, reduce recoverytimes and improve quality out-comes it sometimes places astrain on the traditional instru-ments available to the surgeon.Swann-Morton have an ongoingrelationship with the surgeons andare constantly developing therange with devices which we feel

will assist in meeting the chal-lenges of this ever changing envi-ronment.

Whether it is safety devices tohelp in the reduction of accidentalsharps injuries and the fightagainst cross infection, or smallerfiner scalpels suitable for en-doscopy and arthroscopy proce-dures or the new “Cygnetic” rangefor Orthopaedic surgery, Swann-Morton aim to stay at the leadingedge of surgery.

MEH :Who are your target cus-

tomers, and how do you intend to

reach them?

CT: Traditionally with the assis-tance of our locally based distribu-tion partners we have tenderedfor contracts for the MOH, MODand private hospital groups. Ourmain end user groups are the Sur-geons and OR nurses who haveto handle the product on a regularbasis. More recently a number ofcustom procedure tray manufac-turing units have been establishedwithin the Middle East and wehave supplied our scalpels or

stitch cutters as a component partof these kits.

Our local distribution partnershave sales teams who visit theHospital facilities on a regularbasis and who are available to an-swer any product related ques-tions the end user may have. Wealso have a very informative web-site at www.swann-morton.comwhich carries not only in depth in-formation on each product withinthe range but also has an interac-tive section which contains train-ing films, product handling andcleaning information and alsoPDF copies of the main productliterature which can be down-loaded.In addition to this we have a pres-ence either directly ourselves orindirectly through our distributorsat Arab Health in Dubai and otherregional shows.

MEH: Have you recently launched

any new products for the Middle

East market?

CT: Recent investments in newtechnology and ongoing processimprovements have resulted inthe launch of a new range ofStainless Handles introduced toreplace the traditional Nickel han-dles over the last 12 months andthe new improved Sterile Dispos-able Scalpels to be phased in dur-ing the a latter part 2010.

To reassemble place the lever into the ‘half moon’indentation, located on the graduated bottom section,identified with the Cygnetic® mark. The lever should bein the forward position. Take the top section and slide itinto the forward guides of the bottom section while atthe same time moving the lever back to the vertical andplacing gentle downward pressure on the Swann-Mortonlogo. Listen as the 3 parts click into place. Continue tomove the lever back towards the handle body to lockthe component parts into place. The handle is now readyto use for the next procedure.

The UK has traditionally hadstrong business links with MiddleEast countries, particularly theGulf coast states of UAE, Qatar,Saudi Arabia, Kuwait, Bahrain andOman; but also Lebanon, Israeland Egypt in the wider region.

Saudi Arabia is the UK’s 23rdbiggest trading partner worldwide,and one of the most important inthe Middle East accounting for

£1.7 billion of UK exports overall,and exporting £819 million of oil tothe UK in 2007 . Saudi Arabia isthe largest market, and largesthealthcare market in the MiddleEast, with the market for medicaldevices alone valued at £433 mil-lion for 2007 with the UK one ofthe leading suppliers, and the sec-tor is expected to continue ex-panding, particularly if privatesector investment reaches thelevels hoped for in the Eighth De-velopment Plan.

Kuwait is also a market of tremen-dous opportunity for UK compa-nies. The total UK exports toKuwait in 2007 were around £426million . The market in Kuwait forthe healthcare and medical sectoris strong and increasing steadily,

particularly buoyed up by the sur-plus funds coming out of oil profits. US manufacturers dominate thismarket with 30% share followedby Germany, France, then the UK.Kuwait is one of the largest in-vestors in the UK in the areas ofliquidity, investment portfolios,and property. As with Saudi Arabiathe UK is a major importer ofKuwaiti oil.

Relative to its size the UK is anunusually fruitful source of med-ical technology innovation;Britain’s 2000 medical technologycompanies employ nearly 50,000people, making up 14 per cent ofthe European medical technologyemployment total.

ABHI Chief Executive, PeterEllingworth, told MEH, “ ABHI isconstantly working to make theUK a fertile environment for inno-vative technologies to be rapidlyadopted. We are working closelywith Government and Departmentof Health associated agencies tohighlight the critical issues affect-ing our sector, including work to

improve sector metrics which willallow both Government and in-dustry to accurately quantify thecosts and benefits of early adop-tion of medical technologies.

“As the NHS continues to treat anaging population and an obesitycrisis the role of the medical tech-nology sector will become evermore important. Treatments suchas orthopaedic implantables thatenable people to lead a full,healthy normal life into their oldage, and gastric bands which canhelp cut the level of obesity canhelp deliver better treatment forpatients and keep costs, both tosociety and the NHS, down in thelong term.”

An international outlook

ABHI’s work in promoting theirmembers abroad encompassesseveral different strands. ABHI co-ordinates the UK pavilion at anumber of trade fairs, includingMedica and Arab Health. Thisyear they took around 100 com-panies to Arab Health and will betaking the same number in Janu-ary next year.

ABHI works closely with UK Tradeand Investment, the Governmentbody charged with promoting UK

companies abroad, to help coor-dinate trade missions to key mar-kets around the world. Theyrecently worked in conjunctionwith UKTI to take several compa-nies on a trade mission to SaudiArabia. ABHI has strong links withUKTI’s Commercial Officers allaround the world and they con-tinue to have a significant inputinto the UK Life Science market-ing strategy.

Peter Ellingworth told MEH, “TheMiddle East represents a keymarket for most of ABHI’s mem-bers. ABHI have personnel on theGovernment’s working groups forthe Middle East, most notablyIraq, and in order to support ourmembers wishing to access thismarket we are in the process ofestablishing a ‘Middle East Work-ing Group’. ABHI have stronglinks with The Middle East Asso-ciation in London and have held anumber of joint events aimed atpromoting the Middle East to UKmedical device manufacturers. “

Durbin PLC is a specialist med-ical supply company thatsources and distributes med-ical equipment, pharmaceuti-cals and consumable suppliesto healthcare professionals inover 180 countries.

Established in 1963 in Londonand with an expected turnover inexcess of £40m this year, theyhave set up their operation to actas a one-stop-shop able to dealwith healthcare supply needs fromlocal project level to national scaleprojects.

In addition to the export business,they have divisions that handleclinical trial supplies, importedmedicines, and family planning

supplies. They also handle stor-age and distribution for a numberof third-parties.

Company history

Durbin PLC was founded as a re-tail pharmacy in Harrow, Londonin 1963. The company later beganexporting medicines and medicalsupplies abroad.

Leslie Morgan joined the com-pany in 1976 as a pharmacist anddeveloped the export side of thebusiness soon after as interna-tional sales director. He travelledwidely, sourcing medicines andmeeting new customers fromaround the world.

In 1999, Leslie Morgan bought the

company and took on the role ofManaging Director, heading up asmall team which has now grownto an 80-strong workforce of phar-macists, nurses, medical engi-neers, commercial executives andwarehouse/freight staff. In No-vember 2002, Durbin bought thetrading arm of the charity ECHOInternational Health Services -one of the leading UK suppliers ofmedical equipment and pharma-ceuticals to charities and NGO’saround the world.

In the same month, they launchedtheir unlicensed/'named-patient'medicine service - helping UKhospitals, retail chemists and dis-pensing GPs find medicines thatare not available in the UK.

Our logistics expertise will alwaysensure that medicines will be de-livered safely and adhering to theconditions of storage. For exam-ple, Durbin has recently been ap-pointed by Biogen Idec tocoordinate the ex-US early ac-cess program of a US approvedmedicine Ampyra (USAN: Dalfam-pridine, INN: Fampridine). Ampyrahas only recently been licensed inthe USA to improve walking in pa-tients with Multiple Sclerosis. Itsanticipated licensing in Europe isexpected in early 2011. Addition-ally we are able, on request, tosupply and make 'specials' medi-cines (bespoke unlicensed medi-cines).”

Durbin PLC’s customers includepharmaceutical wholesalers andtraders, hospitals, retail chemists,GPs, multilateral organisations,NGOs, charities, religious healthfacilities and government agen-cies. Their team exhibits at theArab Health show in Dubai everyyear, and Mr Morgan is a regularvisitor to the region.

During 2004, Durbin PLClaunched two new services - Clin-ical Trials and Sexual Health Sup-plies. Their Clinical Trials teamsource medicines for comparatorstudies and multi-centre delivery,and the Sexual Health Suppliesdepartment provides UK GPs andfamily planning clinics with thecomplete range of reversible con-traceptives.

Mr Morgan told MEH, "Initially mygoal was to find out what prob-lems healthcare professionalswere having in getting hold ofmedicines and supplies and tosource whatever they needed asquickly and cost-effectively aspossible," says Leslie. "The wholeoperation still revolves aroundthat philosophy and it works. Ourcustomers call with a request andwe find what they need and get itto them when they need it."

A strong presence in the region

Asked about Durbin’s business inthe Middle East, Mr Morgan toldMEH, “We deal with a variety ofcustomers in the Middle East, par-ticularly in Saudi Arabia and theUAE. We supply a large range ofpharmaceuticals to this region,and are well known for supplyingoncology drugs, narcotic anal-gesics including controlled drugs,and anti-infectives including TBdrugs.

Mubadala Healthcare, a businessunit of Mubadala DevelopmentCompany (Mubadala), has an-nounced plans to expand the ex-isting services of Imperial CollegeLondon Diabetes Centre (ICLDC)through the establishment of asecond facility in Al Ain. This newCentre will provide world-classmedical care for local residents inAl Ain and the surrounding regionfrom the middle of next year.

The first Imperial College LondonDiabetes Centre facility opened inAbu Dhabi in 2006, following anagreement between Mubadalaand Imperial College London tocollaborate in the fields of health-care, education and research. Itwas one of the first facilities to beestablished by Mubadala as partof its strategy to stimulate the de-velopment of a robust privatehealthcare infrastructure in the re-gion.

The International Diabetes Feder-ation has estimated that 18.7 percent of the UAE population is liv-ing with Diabetes and accordingto the Centre's figures, an aver-age of 15 per cent of their patientsreside outside Abu Dhabi in Al Ain."It is testament to the excellentwork carried out at the Centreover the past four years that weare now able to replicate thesame model of care for patientsliving outside the capital.
