    Please see the 2020 Student Medical Examinations article of Medical questionnaire on website. It will be posted on the GSIS H/P, when we get the details from the Student Health Care Center. so please check this site periodically. 5.Requirements as below ・Please fill in the documents before coming

    April New Students

    *「Medical Questionnaire」We sent the letter of Medical Examinations for New Graduate Students notice with orange color envelop. (You could keep it.)

    *「Research paper of mental and physical health for Graduate Students」(gray color envelop)Please get it at the GSIS office before check-up. It’s available to get it at your lab instead, in that case, please let us know by email. (is-kyom@grp.tohoku.ac.jp)

    October New Students (Eligible for check-up)

    We will send you the research paper of mental and physical health for Graduate Students (gray color paper) with orientation document and the medical questionnaire to your lab.

    AY 2020 Medical Examinations for New Undergraduate/Graduate Students

    Student medical examinations will be conducted in October and November at the Student Health Care Center as

    follows. Eligible students are asked to make an appointment using the online reservation system. All medical

    examinations will be conducted by reservation only to avoid crowding. You will NOT be able to fill out your exam

    form at the venue. Please fill it out before coming.

    1. Dates: October 12 (Mon.), 13 (Tue.), 15 (Thu.), 20 (Tue.), 22 (Thu.), 23 (Fri.)

    November 9 (Mon.), 10 (Tue.), 11 (Wed.)

    To avoid crowding, please come at your appointed time (you can check in 5 minutes early). Please be

    punctual.

    2. Location: Tohoku University Student Health Care Center Kawauchi 41, Aoba-ku, Sendai-shi

    ☏022(795)7836・7829 http://www.health.ihe.tohoku.ac.jp/ Note: There are no parking spaces, so please use public transportation.

    Note: In the event of an emergency (natural disaster etc.) during the examination period, please see the

    Student Health Care Center website.

    3. Eligibility: New students (undergraduate, graduate) entering Tohoku University in AY 2020 who have not yet

    undergone a medical examination in this academic year are eligible (excludes students who underwent

    chest x-rays during the June examinations). Includes new non-degree students (research students etc.)

    entering the university this academic year. Adult students who underwent a medical exam at their workplace

    or another off-campus facility may submit a copy of their results to the Student Health Care Center instead

    of undergoing the university's medical examination. (However, in this case the center will be unable to issue

    a Medical Examination Certificate.)

    4. Making a Reservation (start from September 23 and you can be booked until 24:00 two days before)

    Please make an appointment using the reservation website. (QR code at right)

    Note: See the attached manual (資料3) for instructions on using the reservation system.

    (An email confirming your reservation will be sent from kenshin@star7.jp. Please make sure you are able

    to receive it.)

    5. Please bring (required): ① Tohoku University Medical Examination Form. Fill out the front of the form in pencil, and be sure to bring

    it to your examination. Please enter your Student ID Number, not your Examination Number, on the form

    (資料2).

    *The Medical Examination Forms have already been delivered to new undergraduate students or the

    relevant graduate schools/faculties.

    ② Student ID card, or document (such as enrollment notice) that shows your name and student ID number.

    ③ A device on which you can display your confirmation email, or a printout of the email.

    ④ A black ballpoint pen.

    ⑤ Mental and Physical Health Questionnaire for Students.

    6. Type of exam: chest x-ray

    Note: Please wear a thin, plain t-shirt for the chest x-ray. (Do not wear clothing/bras/tops with printed

    patterns, embroidery, metal parts, buttons, etc.) Also, please do not wear jewelry or accessories (if you have

    items that you cannot remove for religious reasons, please contact us). If you have long hair, please wear it

    up so that it doesn't touch your neck or shoulders.

    7. Request

    ◇Please wear a mask. ◇◇Please follow the instructions on the notice (moving from another prefecture etc.) from the administration

    or university. Only eligible students may undergo the exam.

    ◇◇◇If you are not feeling well, or if any of the following apply, do NOT undergo the exam. ■You have COVID-19 symptoms, or such symptoms subsided less than 72 hours ago. ■You have been diagnosed with COVID-19 and still have not recovered from COVID-19. ■You have learned that you were in close contact with somebody who has COVID-19 within the last two

    weeks.

    ■Those who have not been in Japan for 2 weeks. 8. Issuance of Medical Examination Certificate

    -Medical Examination Certificates reflecting the results of the exam will be issued by the Student Health Care

    Center starting Dec 7 (Mon), 2020.

    -A report of the medical examination results can be issued by automatic certificate machines starting Dec 7

    (Mon), 2020.

  • Measles Screening

    If you have never contracted, or received vaccination for, measles, please visit any convenient medical facility and undergo vaccination (at your own expense).

    ★Many students enter their Student ID Numbers incorrectly! Please check your number on your Student ID Card before entering it.★
