The publisher regrets that a correction from the author was missed in the final
version and would like to apologise for this error. The opening sentence on page 350
should read:
In 2007 Peter Dean, then chairman of the Gambling Commission, commented that
‘We are a nation of gamblers’ (Gambling Commission, 2007).1 Three hundred years
ago, people were saying much the same thing.
