Law Enforcement Identification Number Inspection Instructions If this form is used for identification number verification without applying for an assigned or reassigned number, this completed form should be submitted to the requesting county tax assessor-collector’s office or the Texas Department of Motor Vehicles (TxDMV), as applicable.

If this form is used to apply for an/a: • Assigned Number: This completed form must be submitted by mail or in person to your nearest TxDMV Regional Service Center

with evidence of ownership, photograph of the vehicle, and a $2.00 processing fee. • Reassigned Number: This completed form must be submitted in person to your nearest TxDMV Regional Service Center with

evidence of ownership, a court order requiring the department to reassign the original manufacturer’s identification number, a $2.00 processing fee, and the vehicle must be physically present at the time of application for installation of the number.

Mailed fees should be in the form of a personal check, cashier's check, or money order payable to TxDMV. Do not mail cash. Credit/debit cards and temporary checks are not accepted. Fees are non-refundable.

If an identification number is assigned or reassigned to your vehicle, you will be provided a copy of this form and an original Notice of Assigned or Reassigned Identification Number (Form VTR-68-N). The copy of Form VTR-68-A and the original Form VTR-68-N must accompany the title application when submitted to your county tax assessor-collector’s office.

Inspection Fee: If the inspection is conducted by an auto theft unit of a municipal or county law enforcement agency, a separate $40.00 inspection fee may be charged at the time of inspection. This $40 inspection fee will be waived if the individual presenting the vehicle for inspection is the current registered owner.

Important – Original Manufacturer’s Vehicle Identification Number A court order from a court of competent jurisdiction awarding the applicant ownership of the vehicle is required if the original vehicle identification number or component part number assigned by the manufacturer has been removed, changed, or obliterated, and law enforcement is unable to determine the original vehicle identification number or component part number.

If law enforcement is able to determine the original vehicle identification number or component part number, the applicant must obtain a court order from a court of jurisdiction ordering the department to reassign the manufacturer’s original vehicle identification number or component part number; otherwise, the department will only issue a new assigned number.

Trailer/Semitrailer Definitions Trailer: designed or used to carry a load completely on the trailer’s own structure and is drawn by a motor vehicle. Semitrailer: designed or used so part of the weight of the semitrailer and its load rests on or is carried by the pulling motor vehicle.
