■ Intracoronary adenosine is a useful adjunct toprimary angioplasty in patients with acute MI(myocardial infarction), say researchers in Italy. In thispilot study, 54 such patients were randomised toreceive adenosine 4mg (n = 27) or saline, distal to thecoronary obstruction prior to reperfusion. All patientsachieved angiographic success. Post-angioplasty, 27adenosine recipients had TIMI grade 3 flow, comparedwith 19 saline recipients (p < 0.05). Compared withsaline recipients, adenosine recipients had significantlylower incidences of inhospital mortality (5 vs 0patients) and Q-wave MI (23 vs 16 patients). Leftventricular wall motion improved in significantly moresegments, and deteriorated in significantly fewersegments, in adenosine, compared with saline,recipients.Marzilli M, et al.
