and definitionsPREAMBLE

Ambulatory surgery is expanding throughout the world and it is considered that at least 75%, if not more,of all operations/procedures will ultimately be carried out in ambulatory surgery centres/units.The International Association for Ambulatory Surgery believes that it is important to prepare a list of basicdefinitions of ambulatory (day) surgery which will enable data to be collected for the comparison of am-bulatory surgery practice from one nation to another A multiplicity of terminology has gradually develo-ped which is causing confusion and problems in comparing ambulatory surgery practice in the differentnations.Fundamentally, ambulatory surgery should expand by the inclusion of operations on patients who are cur-rently admitted into acute bed hospitals as inpatients. Nevertheless, technological advances have stimula-ted the development of new techniques and modifications of existing techniques, which will further in-crease ambulatory surgery practice.The difficulty in producing a set of definitions which will apply to clinical practice and the health care sys-tem of each nation is appreciated. For this reason, member nations of the International Assocîation forAmbulatory Surgery have been invited to translate. the definitions into their own languages and includeany necessary interpretation applicable to their health care systems.An increasing number of operations/procedures, mostly minor but including some intermediate type, arecarried out as office based/out patient, surgery. This group should be recognised as a separate entity, dis-tinct from ambulatory surgery.The great majority of ambulatory surgery operations/procedures are elective, however appropriately selec-ted acute surgical operations, including trauma, can also be carried out in ambulatory surgery centres/units.It is emphasised that the ultimate choice of patient and procedure to be carried out in ambulatory surgerycentres/units must remain the responsibility of the attending surgeon and/or anaesthetist.

Terminology of the following Ambulatory Surgery definitionsmay be interpreted and expressed in more appropriate terms tosatisfy the specific requirements of healthcare authorities ineach nation.

IAAS – den internationale sammenslutning af dag-kirurgiske selskaber – tror på, at det er vigtigt atudarbejde en liste over basale definitioner ve-drørende dagkirurgi. Det vil gøre det muligt atindsamle sammenlignelige data fra den ene nationtil den anden. En mangfoldighed af benævnelserhar gradvist udviklet sig på området, og det harmedført konfusion og problemer i sammenlignin-ger af dagkirurgisk aktivitet i forskellige lande.

Grundlaget for den fortsatte udvikling af dagkirur-gien er en yderligere omlægning af operationer frastationære indlæggelser til dagbehandling. Tekno-logisk udvikling har stimuleret denne udviklinggennem udvikling af ny teknik og modificering afeksisterende teknikker, som medvirker til at ud-vikle den ambulante praksis.

Vanskeligheden ved at fremstille et sæt af defini-tioner, som kan anvendes inden for den kliniskehverdag og i sundhedssystemerne i de forskelligelande er indlysende. Af denne grund er med-lemslandene i IAAS blevet inviteret til at oversæt-te disse definitioner til deres eget sprog og an-vende nødvendig fortolkning så definitionerne kananvendes i de respektive landes sundhedsvæsener.

Størstedelen af de dagkirurgiske operationer/pro-cedurer er elektive, dog således at udvalgte akuttekirurgiske procedurer såsom traumer også kanudføres i dagkirurgiske enheder/centre.

Det understreges, at det endelige valg af såvel pa-tient som procedure til dagkirurgiske aktiviteter ide dagkirurgiske enheder/centre er den ansvarligekirurgs og anæstesiologs valg.

Forhåbningsvis kan de følgende definitioner dannebasis for nationale og internationale sammenli-gninger af dagkirurgiske aktiviteter.

PREAMBLEAmbulatory surgery is expanding throughout theworld and it is considered that at least 75%, if notmore, of all operations/procedures will ultimatelybe carried out in ambulatory surgery centres/units.

The International Association for Ambulatory Sur-gery believes that it is important to prepare a list ofbasic definitions of ambulatory (day) surgerywhich will enable data to be collected for the com-parison of ambulatory surgery practice from onenation to another A multiplicity of terminology hasgradually developed which is causing confusionand problems in comparing ambulatory surgerypractice in the different nations.

Fundamentally, ambulatory surgery should expandby the inclusion of operations on patients who arecurrently admitted into acute bed hospitals as inpa-tients. Nevertheless, technological advances havestimulated the development of new techniques andmodifications of existing techniques, which willfurther increase ambulatory surgery practice.

The difficulty in producing a set of definitionswhich will apply to clinical practice and the healthcare system of each nation is appreciated. For thisreason, member nations of the International As-socîation for Ambulatory Surgery have been invi-ted to translate. the definitions into their own lan-guages and include any necessary interpretationapplicable to their health care systems.

An increasing number of operations/procedures,mostly minor but including some intermediatetype, are carried out as office based/out patient,surgery. This group should be recognised as a se-parate entity, distinct from ambulatory surgery.

The great majority of ambulatory surgery opera-tions/procedures are elective, however appropria-tely selected acute surgical operations, includingtrauma, can also be carried out in ambulatory sur-gery centres/units.

La chirurgie ambulatoire se développe à travers lemonde et on considère qu'au moins 75%, si n'estplus, de toutes les opérations/actes chirurgicauxseront, à terme, réalisés dans des unités/centres dechirurgie ambulatoire.

Pour l'Association Internationale de Chirurgie Am-bulatoire (IAAS) il est important d'arrêter une listedes définitions de base de ce que recouvre la chi-rurgie ambulatoire (de jour, d'un jour). Ces défini-tions doivent permettre la constitution des bases dedonnées nécessaires à toute comparaison de prati-que ambulatoire d'un pays à l'autre. Un vocabu-laire diversifié s'est graduellement développé. Ilest source de confusions et pose problèmes dansles comparaisons internationales.

Fondamentalement, la chirurgie ambulatoire de-vrait se développer par inclusion de patients opéréslors d'hospitalisations dans des lits de court séjour.Néanmoins, des progrès techniques ont stimulél'apparition de nouvelles pratiques et permis la mo-dification des pratiques existantes, ce qui faciliteral'extension de la chirurgie ambulatoire.

L'IAAS a bien compris les difficultés qu'il y avaità produire un ensemble de définitions adaptéesaux pratiques et aux systèmes de santé de chaquepays. Pour cette raison, les associations nationalesmembres de l'Association Internationale de Chirur-gie Ambulatoire (IAAS) ont été invitées à traduiredans leurs langues les définitions retenues et à yinclure toute interprétation nécessaire à leur appli-cation dans l'organisation de leurs systèmes desanté.

Un nombre croissant d'opérations ou d'actes chi-rurgicaux, surtout mineurs mais incluant certainsactes de type intermédiaire, est réalisé en cabinetmédical. Ce groupe devrait être reconnu commeune entité séparée, distincte de celle de la chirurgieambulatoire.

La grande majorité des opérations ou des actes réa-lisés en ambulatoire l'est dans le cadre d'une chi-rurgie programmée. Cependant, convenablementsélectionnées, des opérations chirurgicales aigües,y compris traumatologiques, peuvent aussi êtreprises en charge dans des unités ou centres de chi-rurgie ambulatoire.

En conséquence, elle n'a paschoisi de distinguer les séjours am-bulatoires des "courts séjours" de"24 à 72" heures ni des séjoursd'hospitalisation traditionnellecomme le préconisait l'AFCA. The International Association for Ambulatory Sur-gery believes that it is important to prepare a list ofbasic definitions of ambulatory (day) surgerywhich will enable data to be collected for the com-parison of ambulatory surgery practice from onenation to another A multiplicity of terminology hasgradually developed which is causing confusionand problems in comparing ambulatory surgerypractice in the different nations.

Fundamentally, ambulatory surgery should expandby the inclusion of operations on patients who arecurrently admitted into acute bed hospitals as inpa-tients. Nevertheless, technological advances havestimulated the development of new techniques andmodifications of existing techniques, which willfurther increase ambulatory surgery practice.

The difficulty in producing a set of definitionswhich will apply to clinical practice and the healthcare system of each nation is appreciated. For thisreason, member nations of the International As-socîation for Ambulatory Surgery have been invi-ted to translate. the definitions into their own lan-guages and include any necessary interpretationapplicable to their health care systems.

An increasing number of operations/procedures,mostly minor but including some intermediatetype, are carried out as office based/out patient,surgery. This group should be recognised as a se-parate entity, distinct from ambulatory surgery.

DEFINITIONEN DEN BEREICHDES AMBULANTEN

OPERIERENSPRÄAMBEL

Weltweit werden immer mehr ambulante Opera-tionen durchgeführt, und es ist davon auszugehen,dass letzten Endes mindestens 75% aller Operatio-nen/Prozeduren, wenn nicht mehr, in ambulantenOperationenzentren / Operationseinheiten dur-chgeführt werden.

Die International Association for Ambulatory Sur-gery findet es wichtig, dass eine Auflistung dergrundlegenden Definitionen für ambulantes (tages-chirurgisches) Operieren erstellt wird, anhand de-rer es möglich ist, Ver-gleichsdaten für die ambu-lante Operationspraxis aus verschiedenen Ländernzu erfassen. Nach und nach hat sich eine Fülle vonFachbegriffen herausgebildet, die für Verwirrungund Probleme beim Vergleich der ambulantenOperationspraxis in den verschiedenen Ländernsorgt.

Grundsätzlich sollten ambulante Operationendurch die Einbeziehung von Operationen an Pa-tienten erweitert werden, die gegenwärtig stationärin Krankenhäusern mit Akutbetten aufgenommenwerden, wobei jedoch die Entwicklung neuer, dieambulante Operationspraxis unterstützender Ver-fahrenstechniken und Abwandlungen bestehenderVerfahrenstechniken durch die technischenFortschritte gefördert wird.

Wir wissen um die Schwierigkeit, Definitionenfestzulegen, die auf die klinische Praxis und dasGesundheits-wesen eines jeden Landes zutreffen.Aus diesem Grunde wurden die Mitgliedsländerder International Associa-tion for Ambulatory Sur-gery gebeten, die Definitionen in ihre jeweiligeLandessprache zu übersetzen und et-waigenotwendige, für ihr Gesundheitswesen zutreffendeInterpretationen mit einzufügen.

La Day Surgery si sta espandendo in tutto il mon-do e si considera che almeno il 75%, se non più, ditutti gli interventi chirurgici/procedure invasive sa-ranno a breve termine condotte in centri/unità dichirurgia ambulatoriale.

La IAAS ritiene che sia importante preparare unalista di definizioni di base sulla Day Surgery cherenda possibile raccogliere dati per il confrontodella pratica di Day Surgery tra un Paese e l’altro.Una vasta terminologia si è gradualmente svilup-pata e sta creando confusione e problemi nel con-fronto dell’ attività di Day Surgery tra i vari Paesi.

La difficoltà nel produrre una serie di definizioniche possano essere applicate alla pratica clinica edal Sistema Sanitario di ciascun Paese è notevole.Per questa ragione, ai Paesi membri dell’IAAS èstato chiesto di tradurre le definizioni nella lorolingua e di inserire qualsiasi necessaria interpreta-zione applicabile al loro sistema sanitario.

La grande maggioranza di interventi chirurgici/procedure invasive di Day Surgery sono elettive,anche se casi selezionati di particolari operazionichirurgiche, inclusi traumi, possono essere condot-ti in centri/unità di Day Surgery.

Va sottolineato che al chirurgo e/o anestesista res-ponsabile rimane la scelta ultima del paziente edella procedura da effettuare in centri/unità di DaySurgery.

PREAMBLEAmbulatory surgery is expanding throughout theworld and it is considered that at least 75%, if notmore, of all operations/procedures will ultimatelybe carried out in ambulatory surgery centres/units.

The International Association for Ambulatory Sur-gery believes that it is important to prepare a list ofbasic definitions of ambulatory (day) surgerywhich will enable data to be collected for the com-parison of ambulatory surgery practice from onenation to another A multiplicity of terminology hasgradually developed which is causing confusionand problems in comparing ambulatory surgerypractice in the different nations.

Fundamentally, ambulatory surgery should expandby the inclusion of operations on patients who arecurrently admitted into acute bed hospitals as inpa-tients. Nevertheless, technological advances havestimulated the development of new techniques andmodifications of existing techniques, which willfurther increase ambulatory surgery practice.

The difficulty in producing a set of definitionswhich will apply to clinical practice and the healthcare system of each nation is appreciated. For thisreason, member nations of the International As-socîation for Ambulatory Surgery have been invi-ted to translate. the definitions into their own lan-guages and include any necessary interpretationapplicable to their health care systems.

An increasing number of operations/procedures,mostly minor but including some intermediatetype, are carried out as office based/out patient,surgery. This group should be recognised as a se-parate entity, distinct from ambulatory surgery.

The great majority of ambulatory surgery opera-tions/procedures are elective, however appropria-tely selected acute surgical operations, includingtrauma, can also be carried out in ambulatory sur-gery centres/units.

It is emphasised that the ultimate choice of patientand procedure to be carried out in ambulatory sur-gery centres/units must remain the responsibilityof the attending surgeon and/or anaesthetist.

INLEIDINGHet aantal dagchirurgische ingrepen neemt wereld-wijd toe, verwacht mag worden dat tenminste 75%of meer van alle operaties/ingrepen te zijnertijd indagbehandelingcentra zal worden verricht.

De International Association for Ambulatory Sur-gery is van mening dat het noodzakelijk is een lijstvan basale definities met betrekking tot dagchirur-gie op te stellen, teneinde het mogelijk te makenom gegevens te verzamelen voor de vergelijkingvan de dagchirurgische praktijk in de verschillendelanden. Een veelvoud van benamingen is langza-merhand ontstaan waardoor verwarring wordt ve-roorzaakt en de vergelijking van de dagchirur-gische praktijk in de verschillende landen bemoei-lijkt wordt.

In principe zal het aantal dagchirurgische ingrepentoenemen wanneer patiënten die nu klinisch wor-den behandeld in dagbehandeling worden geope-reerd. Een verdere toename kan worden voorzienwanneer door technologische vooruitgang nieuweoperatietechnieken worden ontwikkeld en bes-taande technieken worden aangepast.

Erkend wordt dat het moeilijk zal zijn definities opte stellen die recht doen aan de klinische praktijken het gezondheidszorg systeem in ieder land.Daarom zijn de deelnemende landen van de Inter-national Association for Ambulatory Surgery uit-genodigd de definities te vertalen, waarbij reke-ning wordt gehouden met de mogelijkheden bin-nen het eigen gezondheidszorg systeem.

Een toenemend aantal operaties, meest kleinere in-grepen, worden uitgevoerd als poliklinische ver-richting. Deze groep van ingrepen moet wordenonderscheiden van dagchirurgie.

Het merendeel van de dagchirurgische ingrepen iselectief, echter goed geselecteerde acute ingrepen,ook traumatologische, kunnen worden uitgevoerdin dagbehandeling.

Benadrukt moet worden dat de uiteindelijke selec-tie van de patiënt en de ingreep voor dagbehande-ling tot de verantwoordelijkheid blijft behoren vande behandelende chirurg en/of anesthesioloog.

The International Association for Ambulatory Sur-gery believes that it is important to prepare a list ofbasic definitions of ambulatory (day) surgerywhich will enable data to be collected for the com-parison of ambulatory surgery practice from onenation to another A multiplicity of terminology hasgradually developed which is causing confusionand problems in comparing ambulatory surgerypractice in the different nations.

Fundamentally, ambulatory surgery should expandby the inclusion of operations on patients who arecurrently admitted into acute bed hospitals as inpa-tients. Nevertheless, technological advances havestimulated the development of new techniques andmodifications of existing techniques, which willfurther increase ambulatory surgery practice.

The difficulty in producing a set of definitionswhich will apply to clinical practice and the healthcare system of each nation is appreciated. For thisreason, member nations of the International As-socîation for Ambulatory Surgery have been invi-ted to translate. the definitions into their own lan-guages and include any necessary interpretationapplicable to their health care systems.

An increasing number of operations/procedures,mostly minor but including some intermediatetype, are carried out as office based/out patient,surgery. This group should be recognised as a se-parate entity, distinct from ambulatory surgery.

Rozwój chirurgii ambulatoryjnej na całym świecieupoważnia do stwierdzenia, że co najmniej 75% -jeśli nie więcej – spośród wszystkich zabiegówoperacyjnych i innych procedur medycznychbędzie można ostatecznie wykonywać w ośrod-kach (oddziałach) zajmujących się tą dziedziną.

Międzynarodowe Stowarzyszenie Chirurgii Am-bulatoryjnej (International Association for Ambu-latory Surgery) uważa, że konieczne jestsporządzenie listy podstawowych definicji do-tyczących chirurgii ambulatoryjnej (chirurgii jed-nego dnia). Umożliwi to gromadzenie danych,które będą służyć do międzynarodowychporównań praktyki prowadzonej w tej dziedzinie.Stopniowo narastająca mnogość terminów iokreśleń spowodowała bowiem pewne zamiesza-nie i takie porównania sprawiają istotne trudności.

W zasadzie rozwój chirurgii ambulatoryjnej powi-nien odbywać się dzięki włączaniu do niej za-biegów dokonywanych na pacjentach, którzyobecnie przyjmowani są do leczenia stacjonarnegow szpitalach opieki krótkoterminowej (z tzw.łóżkami ostrymi). Niemniej jednak sam postęptechnologiczny jest bodźcem do rozwoju nowychtechnik zabiegowych oraz modyfikowania już ist-niejących, a to w jeszcze większym stopniuprzyczyni się do rozszerzenia zakresu praktyki chi-rurgii ambulatoryjnej.

Zrozumiałe są trudności, związane z utworzeniemzbioru definicji, które będą dotyczyć praktyki kli-nicznej i obowiązywać w systemie opieki zdrowot-nej każdego państwa. Z tego powodu zwrócono siędo krajów członkowskich IAAS z prośbą oprzetłumaczenie definicji na swoje języki, zuwzględnieniem wszystkich niezbędnych interpre-tacji, mających zastosowanie w ich systemachopieki zdrowotnej.

Rośnie liczba operacji (procedur), przeważniemałych, ale w części przypadków również niecobardziej złożonych, które wykonywane są w gabi-netach lekarskich, a także w trybie ambulato-ryjnym. PREÂMBULOA expansão da cirurgia ambulatória por todo omundo tem sido tal que se considera que pelo me-nos 75% de todas as intervenções cirúrgicas / pro-cedimentos podem ser realizados em Unidades /Centros de Cirurgia Ambulatória.

A Associação Internacional de Cirurgia Ambu-latória (IAAS) acredita ser importante a prepara-ção de uma lista de definições básicas sobre cirur-gia ambulatória que permita, no futuro, coligir da-dos e comparar a prática da cirurgia ambulatórianos diferentes países. A multiplicidade da termino-logia que se foi gradualmente desenvolvendo temsido responsável pela confusão e pelos obstáculosque surgem quando se pretende comparar a práticada cirurgia ambulatória entre países.

Em princípio, a cirurgia ambulatória deve expan-dir-se à custa da inclusão de intervenções cirúrgi-cas / procedimentos em doentes habitualmente ad-mitidos em camas de hospitais de agudos comodoentes internados. Contudo, avanços tecnológicostêm permitido o desenvolvimento de novas técni-cas ou de modificações de técnicas existentes, queconduzirão a um aumento da prática da cirurgiaambulatória.

É notória a dificuldade existente na elaboração deum grupo de definições que se aplique à práticaclínica e ao sistema de saúde de cada nação. Poresta razão, membros das Associações Nacionais daIAAS foram convidados a traduzir as diferentesdefinições para a sua própria língua, incluindoqualquer interpretação necessária à adaptação aoseu próprio sistema nacional de saúde.

PREAMBLEAmbulatory surgery is expanding throughout theworld and it is considered that at least 75%, if notmore, of all operations/procedures will ultimatelybe carried out in ambulatory surgery centres/units.

The International Association for Ambulatory Sur-gery believes that it is important to prepare a list ofbasic definitions of ambulatory (day) surgerywhich will enable data to be collected for the com-parison of ambulatory surgery practice from onenation to another A multiplicity of terminology hasgradually developed which is causing confusionand problems in comparing ambulatory surgerypractice in the different nations.

Fundamentally, ambulatory surgery should expandby the inclusion of operations on patients who arecurrently admitted into acute bed hospitals as inpa-tients. Nevertheless, technological advances havestimulated the development of new techniques andmodifications of existing techniques, which willfurther increase ambulatory surgery practice.

The difficulty in producing a set of definitionswhich will apply to clinical practice and the healthcare system of each nation is appreciated. For thisreason, member nations of the International As-socîation for Ambulatory Surgery have been invi-ted to translate. the definitions into their own lan-guages and include any necessary interpretationapplicable to their health care systems.

An increasing number of operations/procedures,mostly minor but including some intermediatetype, are carried out as office based/out patient,surgery. This group should be recognised as a se-parate entity, distinct from ambulatory surgery.

The great majority of ambulatory surgery opera-tions/procedures are elective, however appropria-tely selected acute surgical operations, includingtrauma, can also be carried out in ambulatory sur-gery centres/units.

It is emphasised that the ultimate choice of patientand procedure to be carried out in ambulatory sur-gery centres/units must remain the responsibilityof the attending surgeon and/or anaesthetist.

PREAMBULOLa Cirugía Ambulatoria se esta extendiendo y seconsidera que al menos el 75%, si no más de todaslas operaciones / procedimientos se llevaran a caboen régimen de CMA.

La IAAS considera importante preparar una listade definiciones básicas en CMA que permita reco-ger datos para establecer comparaciones entre unpaís y otro. Progresivamente se han desarrolladomúltiples terminologías, creando confusión y pro-blemas al comparar la practica de la CMA entrelos diferentes países.

Fundamentalmente, la CMA se expandiría con lainclusión de operaciones en pacientes que actual-mente son ingresados en camas de agudos de loshospitales. Sin embargo, los avances tecnológicoshan favorecido el desarrollo de nuevas técnicas ymodificaciones de las técnicas existentes, que in-crementaran los procedimientos de CMA.

Se aprecia la dificultad para producir un conjuntode definiciones que se apliquen a la practica clíni-ca y a los sistemas sanitarios de cada país. Por estarazón, los miembros de la IAAS han sido invitadosa traducir las definiciones a su propia lengua e in-cluir cualquier interpretación aplicable a su propiosistema sanitario.

Un numero cada vez mayor de operaciones / pro-cedimientos, principalmente menores pero in-cluyendo algunos intermedios, se llevan acabocomo cirugía en consulta. Este grupo debe ser con-siderado como una entidad separada, diferente a laCMA.

La gran mayoría de operaciones / procedimientosde CMA son electivos, aunque algunas operacio-nes urgentes, por ejemplo, en traumatología, pue-den también realizarse en los centros y unidades deCMA.

De cualquier manera la decisión ultima sobre si unpaciente o procedimiento es seleccionado paraCMA debe ser responsabilidad del cirujano y/oanestesiólogo.
