ABOUT THE PRACTICEHolland Park Surgery was established at the inception of the National Health Service in 1948..The practice premises at that time were located in Palace Gardens Terrace, just to the north of Kensington High Street. The practice relocated to larger premises at 73 Holland Park in November 1984,then moved into its new site at 12 Phillimore Walk on 23rd May 2016.

Holland Park Surgery provides Personal Medical Services (PMS) under the NHS.

PRACTICE OPENING TIMESMonday 07:00am - 20:00pm** Thursday 08:00am - 18:30pm Tuesday 07:00am - 18:30pm** Friday 08:00am - 18:30pm

Wednesday 08:00am - 20:00pm****Extended hours sessions take place between 7am till 8am in the mornings and 6.30pm to 8pm in the evenings. These appointments are only bookable in advance and are based on availability of doctors and nurses for the extended clinics. There are no phone access duringthe extended hours.

HOW TO REGISTER AS A PATIENT

Patients who live in our practice area may register with our practice (Proof of eligibility will be required if this is your first registration in the UK).

Proof of address i.e. a recent utility bill or a rental agreement is required for all registrations. Please contact reception first. You should bring your NHS number and the details of your previous GP with you. Although your registration will be with the practice, you can see the GP of your choice but for administrative reasons, you are registered under the name of one of the partners of the practice, Dr Ali Al-Rufaie or Dr Bavani Dharmawardene.In order to register with the practice, patients are encouraged to have a new patient health check. This will involve a 15 minute review of past medical history, family history, drug allergies, blood pressure, basic urine test, height and weight. Please bring in any current medication or information that you think may help.If you are female, please bring the date and details of your last smear test.If you have children less than 6 years of age, please bring their immunisation records with you.

OUT OF AREA REGISTRATIONSWITHOUT HOME VISITS

For patients who do not live within the practice boundary area and would still like to register with our practice, we offer the patient a chance to be registered as an 'out of area' registered patient WITHOUT HOME VISITS.Out of area registrations are discretionary depending on the complexity of your medical needs.

Dr Bavani has been at the practice since 1995 and her interests include dermatological surgery,

obstetrics

& gynaecology, paediatrics, psychiatry, undergraduate teaching and elderly medicine. She

also trains registrars.

Dr Ali Al-Rufaie

BSc, MBBS, MRCGP, MRCP (UK) Qualified from University College London in 2003

Dr Al-Rufaie has been a partner at Holland Park Surgery since 2016, following 5 years as a salaried GP.

His interests are in general medical specialties (especially respiratory medicine and cardiology), care of

the

over 65's, and medical education.

OTHER DOCTORS EMPLOYED BY THE PARTNERSHIP:.Dr Caroline Bloom MBBS, DRCOG, MRCGP Qualified in London 1982Dr Bloom was a former partner and has been with us since 1988. Her interests include, asthma, paediatrics, rheumatology, contraception, obstetrics & gynaecology, minor surgery, GP training andundergraduate teaching.

Dr Richard Hooker MBBS, MRCGP, DCH Qualified in London 1985 Interests include GP training, undergraduate teaching and research, cryosurgery, heart disease, diabetes,

hypertension and dermatological surgery. He is a former partner and joined us in 1989. In his capacity as working part time as a GP Principal at Holland Park Surgery, Dr Hooker works for two days per week as a GP member of the governing body of West London Clinical Commissioning Group (WLCCG). Heprovides minor surgery from the practice for one session per week and is the minor surgery lead for WLCCG. He has been seconded to support the development of My Care My Way (One of the national Whole Systems Integrated Care Service (WSIC)) which is looking at transforming the delivery of healthcare to patients aged 65 and over with chronic conditions at risk of emergency admission.

Dr Lara De Haas MBChb, MRCGP Qualified in Leeds 1995Here since 2002. Interests include obstetrics and gynaecology. Dr De Haas runs the weekly Mother and Baby Clinic.

Dr Deepa Bala MBBS, DRCOG, DCH, MRCGP, TTT Qualified in Newcastle Upon Tyne in 1998

Interests include women’s health, teaching, paediatrics, sports and exercise medicine. Dr Bala joined us

in June 2016.

Dr Gesthimani Misirli MD, MSc, PhD, MRCGP Qualified in Greece 2005 andcompletion of full 3 year GP training in the UK in 2016.Dr Misirli has been with us since November 2016 and her interests include ENT and Cardiovascular Medicine. She has completed her PhD in Epidemiology with a focus on Cardiovascular Medicine.

Dr Fatima Jivraj BSc, MBBS, DCH, DFSRH, MRCP Qualified in London 2006 Since completing her GP training in 2011 in North West London, Dr Jivraj has acquired specialist

Medicine (MRCPUK) and in Women's Health (DRCOG). She has further developed her teaching skills

by

completing a Postgraduate Certificate for Teachers in Primary Care. She joined Holland Park Surgery in June 2016.

Dr Sophie Le Gros BSc (Hons) 2008, MBBCh (Hons) 2012, MRCGP Qualified as aGP in London 2017. DOHNS 2018.Qualified as a GP in London in 2017. Was formerly a register at Holland Park Surgery and is now working as a salaried GP since October 2017. Dr Le Gros works as a GP with a specialist interest in ENT and has successfully completed the Diploma in Otolaryngology and Head and Neck Surgery (DO-HNS).

Dr Hannah Lowi Jones MPH (Manchester 2010), MbChB (Manchester 2011),DFSRH, MRCGP Qualified as a GP in London 2017.Formerly registrar and now a salaried GP since August 2017. Interests include palliative care andelderly medicine and sexual health.

Janet Ellis RN (1987), Specialist Practice District Nursing (1990), CommunityPractitioner Nurse Prescriber (2000), BSc (Hons) Nurse Practitioner Primary HealthCare (2002) and Nurse Independent /Supplementary Prescriber (2006).Janet has a special interest in Type 2 Diabetes. She is in charge of the management of the nursingteam. Also able to treat minor ailments e.g. ear, nose and throat infections, chest infectionsetc. She will be working closely with the doctors triaging 'on the day' problems. She runs weeklydiabetic clinics.

Maria Luzon BSN (Phillipines 1987), RN (2000) and Registered SpecialistCommunity Public Health Nurse - Occupational Health (2008).

Maureen Layoken Bachelor of Science in Commerce (1989), RN (2010),Community Adult Nurse (2010) and Practice Nurse (2017).

Maria Luzon and Maureen Layoken, work closely with the Nurse Practitioner offering similarservices: cervical smears, ear irrigation, family planning (Pill review and contraceptiveinjections), dietary advice, 24 hour blood pressure monitoring, travel consultation and immunisations, childhood immunisations, wound care, anti-coagulation monitoring (INR), chronic disease management (Hypertension, Asthma and COPD reviews) and spirometry. For IUD and implants, please contact your local family planning clinic.

They are supported by trained health care assistants, Anna James-Baker, Karolina Odrosek andClaire Babb, who can carry out blood tests, NHS and new patient checks, blood pressurereviews, ECGs, simple wound care, suture removals and selected immunisations andinjections.

HOME VISITSWe do ask if possible that you try and come to the surgery as we are able to offer more comprehensive care here. However, if you are too ill to come to us, a doctor may visit you at home. We do ask if you could ring before 11.00am if possible. The receptionist may ask for some information about your problem and the on the day duty doctor will telephone you to decide if a visit is necessary.

ACCESS TO THE SURGERYHolland Park Surgery is on one level and fully accessible for wheelchairs from the main road.There are several consulting rooms all accessible on ground level. There are disabled toilet facilities.

MAKING AN APPOINTMENTConsultations are by appointment only. Appointments can be made either by calling into the surgery on 020 7221 4334 or online using the SystmOnline facility (requires registration). We have three types of GP appointments: routine appointments that you can book in advance, 48 hour embargoed appointments that are available to be booked 48 hours prior to the requested date andurgent on the day appointments that are booked from 8am on the same day. Nurses appointments are booked in advance. Urgent medical problems which you feel cannot wait until the next routine appointment will always be dealt with on the same day by booking you on the same day or getting the doctor on duty to ring you. If you do not need to see a GP but would like to speak to them, we can pre-book telephone consultations with the doctor of your choice. Our aim is to offer appointments for non-urgent conditions with your usual doctor within ten working days of therequest. Please inform us as soon as possible if you are unable to attend your appointment as someone else may need it. Inevitably this means we need to focus on a single problem per appointment. If you have multiple medical problems, it may require more than one appointment to deal with the problems adequently.

We really do make every effort to run on time and therefore if you are more than 10 minutes late, you may be asked to re-book your appointment (in order to avoid delaying the patients who arrived on time) or wait until a free slot is available. This may not be until the end of surgery. Thank you for your co-operation.

TEST RESULTS AND SPECIMENSPlease telephone the practice between 10.00am and 4.00pm Monday to Friday to obtain the results of your investigations. You should telephone one week after the test has been carried out. The doctor will usually advise the receptionist of your results, allowing information to be passed on to you over the telephone. Sometimes the doctor may want to discuss the result with you and you may be asked to make an appointment. The reason for this is that some laboratory results are complicated to explain and are, therefore, better dealt with face to face. The reception team are not medically trained and can only tell you if your results are normal or not. Specimens may be left with the reception desk before 3pm Monday to Friday. Please ensure that specimen containers are properly labelled as investigations are not carried out on un-labelled specimens.

REPEAT PRESCRIPTIONSYou may order a repeat prescription by one of the following methods:1. Tick the items you require on the detachable counterfoil from your last prescription in person orput the request in writing including your name and date of birth

2. By fax to 020 7792 85173. By post to the practice address4. You can also request your medication online by logging into our website. You will have tocome to reception with a proof of ID to get your log-in details.

The prescription will be ready for collection within two working days. If you would like it postedback, please enclose an SAE allowing for possible postal delay.We are unable to accept repeat prescription requests over the telephone.

PRIVATE MEDICATION REQUESTS

The initial prescription from your consultant should be taken to the chemist for dispensing in the normal way. Please book an appointment with your GP or put in a written request if you need to continue with this medication. This will only be issued when we have received a letter from your consultant informing us of the treatment plan. We prescribe according to our local CCG formulary and current cohort.

Prescriptions from private GPs cannot be converted into NHS prescriptions.

TEACHING AND TRAINING

The practice is committed to training and education. We are a fully approved training practice for fully trained doctors completing their training. Occassionally, we may have a medical student sitting in during your consultation as part of their training, but if you do not prefer this, your request to see the doctor alone will be accepted without question. We may video-record consultations for teaching purposes and to provide us with feedback on our consultation skills. However, this would only be done with your prior knowledge and consent.

MINOR SURGERY

There is a minor surgery clinic run by Dr Hooker and Dr Dharmawardene, who have been approved to carry out certain minor surgical procedures. These include minor skin lesions and cysts. You willneed to initially discuss the skin issue with your usual GP, they will then refer you to the service ifappropriate.

CONFIDENTIALITY & MEDICAL RECORDS

Holland Park Surgery holds your Personal Data including your personal information, medical history, vaccination records, pathology and radiology results electronically. The practice complies with data protection and access to medical records legislation. We are complying with the new General Data Protection Regulation (GDPR) on how we obtain, process and store your personal data. GDPR comes into effect on 25th May 2018. We will strictly use your personal and clinical information as required for delivering professional health care.

Identifiable information about you will be shared with others in the following circumstances:•To provide further medical treatment for you e.g. from district nurses and community services.•To help you get other services e.g. from the social work department. This requires your consent.•When we have a duty to others e.g. in child protection cases.

We do not pass your details on to other third parties for unsolicited marketing purpose.

Summary Care Record will contain key information about the medicines you are taking, allergies you suffer from and any adverse reactions to medicines you have had in the past. Authorised healthcare staff can only view your SCR with your permission. Having a SCR can help by providing healthcare staff treating you with vital information from your health record. This will help the staff involved in your care make better and safer decisions about how best to treat you. You have the choice of what information you would like to share and with whom.

COMPLAINTS AND SUGGESTIONSIf you have a comment, suggestion or complaint, please either speak to a member of our reception team or one of the partners or put it in writing to our Practice Manager, Raj Sharma. A complaint will be acknowledged within 72 hours and a full response aimed within 10 working days.

YOUR RIGHTS AND RESPONSIBILITIES Patients will be treated with respect and courtesy at all times and the practice staff expect the same in

return. We support the NHS policy of zero tolerance with regard to abusive behaviour. Anyoneattending the surgery who is abusive, be it verbal or physical, will risk removal from the practice list.

CHARGES

Charges may be made for services to patients which are not available on the NHS such as certain travel vaccinations, private medical cerificates, private insurance claim forms and various types of medical examinations. The receptionist will inform you of these charges, which follow BMArecommendations. A list of charges is displayed at reception for your information.

PRIVATE INSURANCEWe are always happy to make referrals into the private sector if clinically required. However, please note that we are unable to sign insurance claim forms if we have not been previously involved in the referral. As it is non-NHS work, there is a charge for this service in accordance with BMArecommended guidelines.

CARE QUALITY COMMISSION

The Care Quality Commission (CQC) is the independent regulator of all health and social care in England. They monitor, inspect and regulate all hospitals, care homes, home-care agencies, GP practices and Dental practices.

Patients have the opportunity to tell CQC about the experience of care that they receive at any of the detailed environments. You can view our latest CQC report on the practice website.

CHAPERONING POLICY

Our Medical Centre is committed to provide a safe, comfortable environment where patients and staff can be confident that best practice is being followed at all times and the safety of everyone is ofparamount importance. Patients are able to request the presence of a chaperone if required for any intimate examination or if they feel they require the support of another individual during any examinationby the doctor or nurse.

This chaperone policy adheres to local and national guidance and policy.

Patients are encouraged to ask for a chaperone if required at the time of booking their appointment wherever possible.

MY CARE MY WAY"My Care, My Way" is a new integrated care service for anyone aged 65 and over. It has beendesigned by local patients and GPs to improve healthcare for over 65s.

Integrated care teams include your GP. It may also include a My Care, My Way CaseManager or Health and Social Care Assistant based at the practice.

Patients are asked about their care needs and a holistic approach is taken to organise that care plan.

The My Care, My Way team at Holland Park Surgery consists of Jane Blackburn and Japhet Tombo, the Case Managers and Michael Parra and Paula Morrell, the Health and Social Care Assistants. The team has various health and social backgrounds.

SMOKING CESSATIONWe run a Smoking Cessation clinic every Wednesday. The clinic is carried out by Anna Rozyk who is trained to help you stop smoking. This NHS service and is free of charge. Appointments are booked via reception.
