Lately I fell in love with procedure!

Surprisingly, I have been called by the contractor with the request that RDK asked him to

call me to handle the process of PR-PO for the work activities of Safety Consultancy &General Contractors. This is outrages!

I can consider this request by the contractor, and not by RDK, as a trick for maintenanceto fall in, because since last week I requested for the one who started this event, to send mea written request to proceed with these activities.

First of all, I have been requested to send a memo regarding a PO which the contractor hadreceived. This is a fraud according to me.

Secondly, a PR had been made for purchasing two containers to remove the objects from

the wooden office, namely Jurong Building.

Apparently, the PO had been cancelled, although one container had been delivered.

If sending me a written request is not possible, I would like to know the real reason why forthis.

Henny sorry for the late response!

It all started since 2011, when the backlog team of RDK did throw documents for the USS offices, into the USS

control room!

During the departure of this backlog team, last year,they requested to clean up the control room, and brought in this

contractor to do this job with 2 labor, within 6 months. The process had been handled at RDK, and the PO had been

made. This PO had never been approved, which caused the problem that the invoices could not been received!

Times after times I requested for the PO to be approved in MAXIMO. Last week I have been requested to make a

memo for changing the description of the PO, whereupon I asked for a written request from upper level. I am still

waiting for this request to proceed with the PO change. The 2 ladies who were working for this same company, have

been used for filing system USS and this also out of my scope and will ask the one responsible for bringing in these

to ladies to do the administration and filing work. I would like to help solving this issue but need written request to

process what is needed.

I have no idea whatsoever about the cleaning/clearing of the Control Room as this washandled by Rignald Mauricia (Mou). These days Im seeing Eugene talking to the

persons doing the cleaning so I suppose he knows something about the process. Myrecommendation is for you to consult Eugene and/or Mou to find out more.

We have not yet paid SCGC a cent as we are still awaiting the receipt of the services byMaintenance.

About the starting/stopping dates I can tell you that we had no agreement on the starting

date, and now we do not have an agreement on the stopping date. In October 2013, I

talked to Shenill and requested her to find us someone to help us out with the invoice

control and filing. I came in one day and found these ladies here. Shenill informed methat they came to help us with the activities that we discussed. I requested her to send me

the contract and the CV of the ladies, but no response till today.

Yesterday I understood from themselves that today Friday February 21, 2014 is their last

day here with us and I agree with this.

We have a PO numbered 16208 value ANG 118,251.36 with this Contractor for the

cleaning/clearing of the old USS Control Room under the custodian of Mr. RignaldMauricia (Mou) of RdK. Currently there are five persons working under this contract.

Three persons working on the cleaning/clearing of the old control room and two, namely

Mrs. Ermigia Hensen and Zahaira Olaria helping us (A/P) out with invoice verification

Up to now three invoices of ANG 19,708.56 and one for ANG 9.854.28, for a total ofANG 68,979.96 were paid to SCGC via Blue Factor. No payments made to Blue Factor

as yet.

Outstanding are one invoice of ANG 9,854.28 and two invoices of ANG 24,175.35 for a

total of ANG 58,204.98. This means that a total of ANG 127,184.94 still to be paid to

SCGC. No receiving and no payments could be made as the PO is still WAPPR. Thus notapproved by Mr. Hanst.
