Govt. Plans Centralised DigitalEntity For ‘Ayushman Bharat’

SchemeThe Indian government is in discussions to set up anew digital entity called the National Digital HealthMission that will act as a central digital repository for allthe data coming under its flagship health insurancescheme Ayushman Bharat. A report filed by a commit-tee set up by the Ministry of Health and Family Welfarehas identified the need to establish a specialized bodycalled National Digital Health Mission (NDHM) which isto be based on similar models as seen in the likes ofUIDAI and GSTN. The report is open for public com-ments till August 4. Some of the proposed provisionsunder the NDHM are – l Citizens should be able toaccess their electronic health records in a convenientmanner, preferably within 5 clicks; l Citizens shouldget integrated health services at a single point, thoughmultiple agencies, departments or services providersare involved; l NDHM shall assure continuum of careto citizens, across primary, secondary and tertiary careand across public and private service providers; l Aframework for unified communication centre will beprepared to facilitate voice-based services and out-reach; l NDHM shall support national portability forhealthcare services National Digital Health Mission

Objectives:- The report also recommends that theNDHB will be an open API-based ecosystem wheresecurity and privacy will be built into the design anddevelopment of the APIs, which will be audited forsecurity and privacy before deployment. The 14-mem-ber committee recommended that consultations shouldbe held with Unique Identification Authority of India(UIDAI), the Aadhaar regulatory authority, and theMinistry of Electronics and Information Technology(MEITy) in the design of the Personal Health Identifier(PHI). “Aadhaar cannot be used in every health contextas per the applicable regulations. The design of PHI,therefore, must allow multiple identifiers (chosen from the specified types of identifiers) for designing the structure andprocesses relating to PHI,” said a National Digital Health Blueprint (NDHB) created by a committee under former MeitYSecretary and UIDAI ex-chairman J Satyanarayana. The PHI will maintain a personal health record of patients and will havedetails about their immediate family, date of birth, gender, mobile number, email address, location, family ID and photograph.India’s Ayushman Bharat Plan:- Under the Ayushman Bharat, India is expected to have 40,000 Ayushman Bharat Health

and Wellness Centres (AB-HWC), meant for primary health care, by the end of 2019-20. The mega public health insurancescheme called Pradhan Mantri Jan Arogya Yojana (PM-JAY) and popularly known as the Ayushman Bharat, also has an appon Google Play Store. At the Union Budget 2019, the govern- ment announced that it will allocate INR 6,400 Cr for the imple-mentation of Ayushman Bharat. The allocation has increased by INR 4K Cr from 2017-18, while overall INR 10K crore has beenallocated to implement AB until 2020. Overall, the Indian health- care market stands at $100 Bn, according to IBEF and isexpected to touch $280 Bn by 2020, growing at a CAGR of 22.9%.

He had not even hopped off his bicycle when his staff informed him about a Nipah case being con-firmed in Kerala, which was followed a week later by nationwide protests and strikes by doctors and health professionals after twojunior doctors were assaulted at Nil Ratan Sircar Medical College in Kolkata. Dr Harsh Vardhan speaks to Sanchita Sharma onthe firefighting his ministry has done over seven weeks and its plans for the next five years. Edited excerpts: Your taking charge

was immediately followed by one of India’s biggest doctors’strikes. How can hospitals be made safer for doctors?Doctors often work under stressful and difficult conditions and Istrongly condemn assault against them. The violence in Kolkatathat led to doctors’ strikes across the country gravely affectedhealthcare services. The health ministry has set up a 10-mem-ber committee on violence against doctors with representationfrom the ministry, Indian Medical Association (IMA), Federationof Resident Doctors Association, Medical Council of India,AIIMS, and the Union ministry of home affairs and the lawdepartment. The first meeting was on July 10th, during whichthe home and law ministries concurred on the need for a centrallegislation. It was decided to have the first draft of the proposedAct readied within a week.I wrote to West Bengal chief ministerMamata Banerjee, urging her provide a secure working environ-ment to doctors. I also requested all chief ministers to considerestablishing specific state legislation or bring amendments in

existing state legislative acts to ensure doctors and clinical establishments function without fear of violence. What is the long-term solution to end violence against doctors? Ensuring a safe environment for doctors and patients and involves multiplestakeholders. Apart from taking all possible preventive and deterrent measures, state governments need to investigate all casesof violence promptly and ensure that the criminals get prosecuted under the legal provisions available. The IMA’s draft act, calledThe Protection of Medical Service Persons and Medical Service Institutions (Prevention of Violence and Damage or Loss ofProperty) Act, 2017, which includes clauses on penalty and recovery in case of loss or damage to property, has now been sharedwith the states for their consideration. Nipah was confirmed in Kerala the day you took charge even as Muzaffarpur was strug-gling to contain child deaths from acute encephalitis syndrome (AES). Kerala contained Nipah, but Bihar floundered. How can theCentre ensure containment and treatment protocols are followed across states? You see, health is a state subject. The IntegratedDisease Surveillance Programme (IDSP) does surveillance to quickly detect and respond to disease outbreaks, providing addi-tional manpower, training rapid response teams to investigate outbreaks, strengthening laboratories, strengthening ICT equipmentfor data entry, analysis and data transfer, and providing funds for operationalisation.The Centre also supports states in implement-ing communicable disease programmes, such as National Vector Borne Disease Control Programme that covers six vector-bornediseases, including Japanese encephalitis and AES, which is of unknown etiology. So, what is causing the AES deaths in Muzaffarpur? AES describes a disease with symptoms of fever and alteredconsciousness and has a very complex etiology. Studies were done on the possible association with litchi toxins in Muzaffarpur during 2013-14, but no definitive evidence linking AES to litchi toxins wasconclusively established.In the present outbreak, Indian Council of Medical Research (ICMR) has done research studies covering various aspects of the disease and I am hopeful we will arrive at sub-stantial findings soon. Soon after taking charge, you requested all states that haven’t joined the Ayushman Bharat-Pradhan Mantri Jan Arogya Yojana (AB-PMJAY) to launch the scheme.Rajasthan has announced merging the state healthcare scheme with AB-PMJAY. Did you hear from Delhi, Telangana, Odisha or West Bengal? The implementation of AB-PMJAY across the coun-try is undoubtedly the top priority. The scheme is going to be rolled out in Rajasthan and Punjab very soon. As a follow-up action, CEO, National Health Agency (NHA), visited Odisha and the state is pos-itive about implementing AB-PMJAY. The NHA is working on the convergence plan. I expect other states to get on board sooner than later.How many people have been treated under AB-PMJAY? Which are the best and worst performing states? The budget forAyushman Bharat went up 154.87% this year. Under the health protection scheme, there have been 32,07,000 hospitalisations and 8.9crore cards issued till July 12.It is difficult to rate states as there are different parameters for performance, such as access, quality etc.Some states that are doing better are Chhattisgarh, Maharashtra, Jammu & K a s h m i r ,Daman and Diu, Gujarat, Kerala, Himachal Pradesh etc. The NHA continually monitors per-formance of all states on different parameters and issues guidelines to improve performance.How is primary health care, which can treat 80-90% illnesses, being s t r e n g t h -ened? As you know, delivering Comprehensive Primary Health Care (CPHC) through healthand wellness centres (HWCs) is a key component of Ayushman Bharat, which is the fulcrumprogramme being implemented to achieve the proposed goal of universal health care.Ourvision is to transform 150,000 sub-health centres (SHC) and primary health centres intoAB-HWCs in a phased manner. The target for 2018-19 was 15,000, and the cumulative tar-get for 2019-20 is 40,000 and so on, till we achieve our goal by 2022. We are on track andhave at least 19,000 operational AB-HWCs. Family History Of Heart Disease Is A Significant Risk Factor In Acquiring Coronary Artery Disease (CAD)More than 52% of deaths caused by heart diseases in the country are in people below 70 years of age. People with a family history are susceptible and therefore, the need of the hour is toraise awareness on following a healthy lifestyle and undergoing timely checkups. AD impacts the flow of oxygen-rich blood to the heart, which can, over time, put a person at an increased riskof a heart attack. Cardiac ailments can put a massive dent in the lives of people, especially those who are susceptible to it. While timely diagnosis and treatment are essential, it is also imper-ative for people to start making healthy changes to their lifestyle from a young age. Dr Manoj Kumar, Director and Head - Cardiac Cath Lab, Max Super Specialty Hospital, Patparganj, NewDelhi, said, “People who have a family history of heart diseases are more likely to get CAD and have high build-ups of calcium in their arteries. It is thus important that they get their coronaryartery calcium levels checked at regular intervals. When the arteries stop supplying blood to the heart, CAD strikes. In this case, the muscles in and around the heart start narrowing. This hap-pens due to cholesterol and plaque build-up on the inner walls of the artery. As the build-up increases, supply of oxygen toheart is reduced, leading to angina or heart attack. Blockage in thearteries can develop from a young age. While some amount of it is common, wrong lifestyle choices can worsen the situation. If you have a parent or a sibling with a case of CAD, you musttake precautions right from a young age to avert the risk.” Adding further, Dr Manoj Kumar, said, “The tendency of premature CAD in Indians can be attributed to a genetic predisposition includ-ing high levels of low-density lipoprotein (LDL-C) or ‘bad’ cholesterol and metabolic syndrome. When combined with an unhealthy lifestyle, these can wreak havoc on the heart leading to earlymortality and morbidity. It is, therefore, essential to cultivate healthy habits right at the outset to reduce the risk of acquiring CAD and other heart problems.” CAD Treatment:- For CAD, angio-plasty is one of the treatment options available. This is done to restore the flow of blood in the body. In this process, a drug-eluting stent is coated with time-release medicine. Some tips to

prevent CAD:- l Eat a low fat, high-fibre diet (including plenty of fresh fruits, vegetables and whole grains). Limit your salt consumption to 6 grams a day. l Indulge in regular exercise to main-tain a healthy weight. Exercise will also help strengthen the heart and increase blood circulation; reduce cholesterol levels, and keep blood pressure under check. l Smoking can cause CADand must be avoided. It is also imperative to reduce your alcohol consumption. l Ensure that your BP and cholesterol levels are under control. l Limited consumption of alcohol is suggest-ed. l Proper sleep is essential to recover from daily wear and tear in the body.

Cipla Eyes Chinese Respiratory Market Through JV With JiangsuAcebright

Respiratoryillnesses are a major cause of death in the country. Cipla (EU), a UK-based wholly owned sub-sidiary of Cipla, and Jiangsu Acebright Pharma will set up a JV in China, where the formerwould hold an 80 per cent stake and Acebright would hold the remaining for a combinedinvestment of $30 million. The JV will set up a manufacturing facility of respiratory products.The plant is expected to be commissioned in 2020. “While our core home markets remain ourcurrent growth anchors, we see China as a crucial part of our future road map. In May, weinaugurated our office in Shanghai. We have a long-standing relationship with Acebright, andthis partnership to build a manufacturing facility in China is a significant step for us,” said

Umang Vohra, managing director and Global chief executive officer of Cipla. Cipla has already started clinical trials ofa dry-powder inhaler (Fluticasone Propionate Salmeterol or FPSM) in China. It had launched FPSM in eight Europeancountries and also in Australia. In an earlier interaction with Business Standard, Vohra had indicated that in two to fouryears, the company expects to see good traction from the Chinese market. “In two to four years, we will see good trac-tion from the Chinese market. It is essentially a branded market and the size is huge. While it takes patience to enterthe market, it does offer great potential,” Vohra had said. In fact, after establishing a stronghold in the domestic mar-ket, Cipla is eyeing the next leg of growth in the segment to come from global markets. The company enjoys a 67 percent share across the Indian subcontinent’s respiratory market and sees potential for a 14-15 per cent growth as themarket is under-penetrated. In India, respiratory constitutes a 31 per cent share in the overall revenue. Most of the res-piratory products that Cipla has lined up for the international markets have a market size of around $2 billion or so. Thecompetition is expected from three to four players. It sees $100-million revenue from each of the key products (gener-ic Albuterol, FPSM, and generic Advair) depending on how many other players get approval. The $100-bn Chinese mar-ket too has potential for respiratory drugs. Cipla thus sees a potential in the market which is largely dominated by multi-nationals like GlaxoSmithKline, Novartis, Boehringer Ingelheim among others. “We are keen to take our well-estab-lished expertise in the respiratory segment to patients in China. Simultaneously, we will explore various routes to builda portfolio of products in other therapeutic segments such as oncology,” Vohra added. The company does not have sig-nificant exports to China at the moment “We do not have a significant export presence to China right now. The focus isbroadly respiratory products through local manufacturing and other therapies through import and other potential routes.We are looking at the oncology segment for instance,” said a Cipla spokesperson. Jiangsu Acebright PharmaceuticalsCo is a subsidiary of the Shanghai Acebright Pharmaceuticals Group. Acebright Group, established in 1996, is mainlyengaged in anti-viral, oncology products and vitamin ingredients. It is a supplier of anti-viral and oncology active phar-maceutical ingredients (APIs) APIs for global market and a manufacturer of finished dosage products to meet HIV/AIDSand cancer patients’ needs for China market. Shengping Xu, chairman of Acebright Group, said, “We have a long-stand-ing partnership of more than 20 years with Cipla which shares our vision and approach towards patients.
