Flow Chart — Admission through Completion Certificate in Clinical and Community Outcomes Research
The student completes the Certificate application process
described at <https://ictr.wisc.edu/certificate-
research-programs/>.
If qualified, the student is admitted and notified.
The Certificate Advisory Committee reviews the student's
research interests and recommends an adviser match.
The adviser name and contact information is sent to the student.
The student meets with Certificate adviser to plan
Certificate program. Student completes the curriculum-plan-
and-tracking form and sends copy to Sharon Schumacher.
The student meets with Certificate adviser to discuss project plan and develops a
project proposal for review and approval.
When the student completes all course and project requirements,
the student notifies Sharon Schumacher.
The ICTR student services coordinator will inform the UW Registrar that the student has
completed the Certificate program.
The ICTR student services coordinator will generate and award the formal certificate document to the student.
