A. Facility (For all health facilities) 1. Is the health facility generally clean and orderly? 2. Is the health facility well-ventilated and well-lit? 3. Does the health facility have a permanent and adequate potable water supply? 4. Does the health facility have a functioning and accessible toilet and sinks for use of patients and staff? 5. Does the facility have enough space to ensure privacy ? (i.e. clients during physical/internal/counseling) 6. Does the health facility have the basic functioning equipment, supplies and essential drugs/medicines at all times?

Note: This survey questionnaire shall be filled up by the patient/client upon his/her exit during consultations in RHUs & OPD jn hospitals (out-patient)and after discharge during hospital confinement (in-patient).
