ESTEEM– Self Care & Self Management Services, are brought to you by a
partnership of health and social-care organisations working in the London
Borough of Tower Hamlets. The lead organisation on all services is
Community Options, an award winning mental health service provider.
These services have been commissioned by the Clinical Commissiong Group in
Tower Hamlets. They are pilot services and are specifically designed to meet the
physical health and mental well-being needs of people living with long term
conditions such as Diabetes, COPD and mental illness in the local community.

What is the service?
The Harmony Singing Group is a community based singing for well-being
group that meet on a weekly basis for a 2 hour session in Tower Hamlets.
The group is led by a professional musician.
There is no audition to join and no previous experience of singing in a
group or choir is required. The group will have the chance to sing a mix of
popular tunes.
During each choir session there will be a short group discussion led by an
experienced Mental Health worker to look at strategies and tools that will
help participants improve their mental well-being.
Who is the service for?
The service is for people living in Tower Hamlets with either or
Chronic Lung Disease.
Mental ill-health issues.
What are the expected outcomes for people taking part?
An increase in participants self confidence and self esteem.
An increase in general wellbeing.
An improvement in participants (with Chronic Lung Disease) breath
control and diaphragmatic breathing.
Reduced social isolation & increased social networks.
Is there a cost for taking part?
No there is no charge to participants to take part in the singing group.

Where and when does the choir meet?
The singing group meets from: 4-6pm every Wednesday at: Toynbee Hall, 28 Commercial Street, London , E1 6LS
How can people take part? Joining the Harmony singing group is easy.
Individuals may refer themselves by requesting an application form
by calling: 0208 313 9725 or emailing:
Self referrals—applications should be supported by a Health Professional i.e. GP, Care-Coordinator etc.
Health Professionals may also make a referral directly to Community Options by using the same email address and phone number to request a referral form. The proposed participant will need to also sign this form.

LIVING WITH ALCOHOL USE Individual 1-1 Support

LIVING WITH ALCOHOL USE
What is the service?
This service provides up to 40 hours of individual, 1-1 flexible support, to
people whose alcohol use increase their risk of self-neglect, social exclusion
and isolation.
It provides practical support to help safeguard people from the hazards
within the home as a result of self-neglect.
Who is the service for?
This service is for people living in Tower Hamlets who have issues around
alcohol use which impacts on their mental and physical wellbeing
Please note that this is not a cessation service but aims to support the
individual to keep as well as possible and to set up systems to help them
self manage.
What are the expected outcomes for people taking part?
For the individual to have improved strategies and systems in place to
deal with day to day life such as: eating well, paying bills, personal
budgeting& finances, personal hygiene.
To have improved mental and physical wellbeing.
To be more independent and have reduced the need for formal ser-
vice to intervene .
To have increased their contact with the wider community and have
access to a wider network of support.
To have improved their quality of life.
To have increased their sense of personal worth and self-esteem

What is the service?
This service provides up to 36 hours of individual, 1-1 flexible support, to
help people who hoard manage the personal and community risks which
affect them.
It provides practical support to help safeguard people from the hazards
within the home or as a result of self-neglect.
Who is the service for?
This service is for people living in Tower Hamlets who have issues around
hoarding which impacts on their mental and physical wellbeing

COPING OPTIONS Esteem Booster Course

COPING OPTIONS What is the service?
Coping Options is an eight session course running for 2 hours per session.
It is led by a mental health professional and is designed to help people
explore the impact of their mental well being on their physical health.
During the course participants will explore and test coping strategies and
tools to help improve well being. People will be encouraged to share their
experiences and progress with others and look at the benefits of
empowerment on their well-being, both physical and mental.
Who is the service for?
The service is for: people living in Tower Hamlets who:
Want to improve their physical and mental well-being.
Who may feel under confident, unable to cope and may be
socially isolated.
What are the expected outcomes for people taking part?
Participants will benefit from developing coping strategies to help
them manage their day to day life.
Will have improved mental and physical well being.
Experience an increase in self-esteem and confidence.
Have a better understanding of how taking care of physical and
mental health in an integrated approach can improve general
well-being .
Is there a cost for taking part?
No there is no cost to participants to take part.
When and where are the courses taking place?
How can people take part?
Individuals need to be referred to the service by a Health Professional from
the 9 South East locality GP surgeries taking part in the pilot.
Health Professionals can access the referral form on EMIS under
“ESTEEM”.
Health Professionals can also request a referral form by calling:

The referral form must be co-signed by the individual wishing to
access the service.
