ENCHANTED Monitor Tips:
Just a reminder that for all Patients that are randomised in to Treatment arm [A], please ensure the vial number is entered onto the CRF and the package is stored with Patient File for review by monitor. To prevent mix ups please attach ENCHANTED label. Please request from your RCC if you have not re-ceived any. Scoring sheets are provided in the patient file for all Neurological/ Function-al & Quality of Life assessments, they are also downloadable off the EN-CHANTED database. Please record all assessments on these scoring sheets as they will form part of the patients source documents. All laboratory results/reports need to be assessed and signed and dated by a delegated ENCHANTED Investigator. If a result is outside of the normal range, the Investigator needs to document whether the result is clinically sig-nificant (CS) or not clinically significant (NCS).
