Re: Cultural Arts Financial Assistance Grant
Purpose of the Organization: The Mission of the Community Chorus of Pakn Coast is to build a tradition of communit}' pride through choral excellence, by providing performances that enrich, educate, and entertain audiences and singers. The chorus (CCPC) sings a variet}' of musical styles, including Broadway, pop, folk, sacred, gospel and classical music. Founded in 2010, the Chorus currently has 66 members f rom Palm Coast and the surrounding area.
Pfogtam to be considered for funding—Concert Series: CCPC is beginning its seventh season with the first rehearsal on September 20, 2016. The concert schedule this season includes a Holiday Concert on Friday, December 9 and Sunday, December 11, and a Spring Concert on Friday, Apri l 21 and Sunday, April 23, 2017. The concerts will be held at Trinity Presbyterian Church, Palm Coast. Approximately 900 people attended the performances last year. The concerts are offered free to the public, and we hope to be able to continue this practice so anyone interested in attending will be able to do so. The audience is given the opportunity to make a donation to support the chorus i f they wish.
CCPC performed by invitation at the following communit}^ celebrations during the past year: Veterans Day Ceremony at Fleroes Park in November 2015; and Memorial Day ceremony at Heroes Park in 2016. Members are happy to be part of these important and meaningful American remembrances.
How the City of Palm Coast's financial assistance will be used: The Cultural Arts Financial Assistance Grant wil l provide partial support for production expenses including: sheet music, advertising, printing marketing materials and programs, postage for marketing mailings, honoraria for our professional director, accompanists, audio technicians and guest musicians, and Flagler County Sherriff s office for assistance with traffic control at concerts (please see Budget Worksheet footnotes addendum for further details). The Chorus markets concerts in a variety of media to reach a wide range of people. Concerts are promoted through local newspaper and neighborhood magazine advertisements, Flagler Auditorium events brochure, press releases, web sites, flyers, emails and direct mail. Public service announcements are made on local radio stations. Marketing staff sends short articles about the concerts and brief listings for community events calendars to newspapers, communit}^

weeklies, T V stations, and local churches. Concert information also appears in online calendars of events, such as the ones for the Chamber of Commerce, the Cit\r of Pakn Coast,
the count)^, newspapers and radio stations. The Chorus maintains its own web site, where
performances are publicized on several different pages. Flyers promoting concerts are posted around the community and mailed to people who attended previous performances.
We hope to continue to receive a grant to help support our activities as we seek to identify and develop new sources of funding. Our fundraising activities wiU continue to center on soliciting donations f rom residents and businesses in Palm Coast and the surrounding area.

Your Consumer's Certificate of Exemption is to be used solely by your organization for your organization's customary nonprofit activities.
Purchases made by an individual on behalf of the organization are taxable, even if the individual will be reimbursed by the organization.
This exemption applies only to purchases your organization makes. The sale or tease to others of tangible personal property, sleeping accommodations, or other real property is taxable. Your organization must register, and collect and remit sales and use tax on such taxable transactions. Note: Churches are exempt from this requirement except when they are the lessor of real property {Rule 12A-1,070, F.A.C.).
It is a criminal offense to fraudulently present this certificate to evade the payment of sales tax. Under no circumstances should this certificate be used for the personal benefit of any individual. Violators m?ill be liable for payment of the sales tax plus a penalty of 200% of the tax, and may be subject to conviction of a third-degree felony. Mike has been an educator for the past 41 years and currendy senses as associate facult)', University o f Phoenix, North Florida, Jacksonville, FL (2012-2016). Prior to this position Mike was the Superintendent of Schools, Bethlehem Central School District, Delmar, N Y (2007-2011) and Assistant Superintendent of this school district (2005-2007) Additionally, he served as the High School Principal, Bethlehem Central High School, Delmar, N Y (2000-2005) and Elementary Principal, Glenmont Elementary School, Delmar, N Y (1999-2000) and Music Educator, various school districts in NYS, 1975-1999. Besides his exemplar}' leadership skills he is also our chorus website developer and administrator.
The President shall be responsible for overseeing all activities of the chorus and shall preside over all Board Meetings; set the date for all meetings, with the concurrence of the Board; oversee the preparation of the annual budget; preside over the election of members to the Board and appoint chairpersons of standing and ad hoc committees.
Sectetaty: Doug Martin, 10 Flemington Lane, Palm Coast, FL 32137 The Secretary shall keep minutes of all meetings of the Board and the general membership; ensure that a roster of Chorus members is maintained; notify Chorus members of the Annual Meeting, special meetings, and slates of nominees provided by the nominating committee; prepare and send out all correspondence for the Chorus.

Nancy retired to Palm Coast after a 34-year career in Nursing with her most recent 20 years as the Perioperative Nurse Manager at the James Cancer Hospital at Ohio State University Medical Center in Columbus, Ohio. RN, BSN, CNOR, RNFA, LCDR while serving in USN.
The Treasurer shall be accountable for the administration of all Chorus funds; maintain funds in a bank in Florida, maintain financial records of all receipts and expenditures, keep bank statements reconciled, and provide a financial report at Board meetings. Two (2) signatures, f rom the President, Treasurer, Assistant Treasxirer or Secretary, are required for all checks or other disbursement of Chorus funds over $500.00, however, the signers may not be two members from the same family. The Treasurer and Assistant Treasurer shall assist the President in preparing the annual budget, including individual concert budgets and provide financial records to an outside CPA for annual review.
Assistant Treasurer: Bob Cahill, 16 Whitcock Lane, Pakn Coast, FL 32164
Bob retired to Palm Coast after a long career in finance. Bob has a MS in Logistics, BS in Economics/Accounting. Served as a Senior Manager and Dkector level experience in large multi-national companies. Volunteer in Flagler County Small Business Incubator program.
The Assistant Treasurer shall assist the Treasurer; learn the duties and responsibilities of the Treasurer; and act for the Treasurer in the Treasurer's absence. Oversee and tabulate the monies raised during the concerts.

Choral Director (Ex Officio): Linda Hodges, 120 Frontera, St. Augustine, FL 32084
Linda received her degree in music education at Jacksonville University and has served as a choral director for over 25 years. Linda plays the clarinet and teaches voice and piano. She has served as the Director of Music at the Trinity Presbyterian Church since 2007.
The Choral Director shall have principal responsibility for all musical and technical aspects of the concert.
Marketing Committee Chair: Jane Gaulding, 10 Chetty Court, Pakn Coast, FL 32137
The Marketing Chakperson is in charge of supporting the marketing needs of the Chorus to the community at large. This is done through organization of advertisements regarding the Chorus and preparation of the concert programs and materials for the concert programs of the Chorus. This chak will also maintain and network with media and business supporters throughout the community for the needs and pubHc relations support of the Chorus.
Membership Committee Chair: Carol Reistetter, 23 Scarlet Oak Ckcle, Pakn Coast, FL 32137
The membership chairperson recruits new members for the chorus as well as makitakis a current and up-to-date dkectory of existing chorus members for the season. He/she collects dues, coordinates auditions and maintains attendance and organizational records for the chorus.
Concert Logistics Committee Chair: Gail Naworal, 23 Marshview Lane, Pakn Coast, FL 32137
The Concert Logistics chairperson is responsible for rehearsal and concert set-up details including risors, chairs, sound system and kghting needs. He/she is also responsible for procuring people to usher at concerts and serve as parking attendants. This person also coordinates with the local Sheriffs office for traffic direction, when necessary.
Fundraising Committee Chair: Lynda Guggenheim, 40 Riverdale Lane, Pakn Coast, FL 32164
The Fundraising Chairperson coordinates activities for the raising of money for the chorus program. This includes creating events to raise money from raffles, silent auctions and other types o f financial generating activities.
Finance Committee Chair: Robert F. Cahill, 16 WTiitcock Lane, Pakn Coast FL 32164
The Finance Committee Chak works closely with the Chorus Treasurer and the Board of Dkectors in supervising and maintaining the budget.

The Pianist received her music degree from Texas Christian University, FT Worth, Texas. After graduation, through a FuUbright Scholarship, she studied pipe organ in Copenhagen, Denmark. She has worked with both piano and organ for many years in addition to teaching both instruments.

It is with heavy sadness hut with aSundant joy we dedicate this concert in memory of our departed friend and Chorus Secretary CaroCyn TrisSie. Jfer efforts on hehaCf of the Community Chorus of<Pa[m Coast from the very Beginning untiC her untimeCy passing in
January 2016 are the reasons this chorus has seen and experienced much success over the
years. She covered aCC the detaiCs and made sure the needs of the Chorus were aCways fuCfiCCed. She was the bachfione and the quiet operative behind the scenes ma^ng
everything come together.

community spirit aruCfeCCowship much the way CaroCyn envisioned this effort at the start.
