one (1) year, such suspension to be immediately stayed in favor of probation subject to the Board's terms

and conditions; he was ordered to pay a civil penalty in the amount of $250; and a public reprimand was

placed on his permanent disciplinary record with the Board, because he has been convicted of a felony.

period of one (1) year, such suspension to be immediately stayed in favor of probation subject to the terms

reprimand was placed on his permanent disciplinary record with the Board, because he has been convicted

of one (1) year to practice as a barber in the Commonwealth of Pennsylvania, such suspension to be

immediately stayed in favor of probation subject to the Board's terms and conditions; and he was ordered

Erie County D Block Barber Specialist, license no. BO432823, of Erie, Erie County, was suspended for failing to pay

Philadelphia County J And D Barbershop, license no. BO433426, of Philadelphia, Philadelphia County, was suspended for a

period of one (1) year, such suspension to be immediately stayed in favor of probation subject to the terms

and conditions of the Board; they were ordered to pay a civil penalty in the amount of $1,000 and an

assessment fee of $46.93 towards costs of investigation; and a public reprimand was placed on their

permanent disciplinary record with the Board , because they owned or operated a barber shop without a

current and active license; and they operated an establishment without supervision of a designated barber

PA Board of Chiropractic Westmoreland County William Harry Roscoe, license no. DC006967L, of North Huntingdon, Westmoreland County, was

Lycoming County Heister H. Linn, Jr., D.D.S, license no. DS018760L, of Williamsport, Lycoming County, had his license

to practice as a dentist in the Commonwealth of Pennsylvania suspended for a period of no less than two

years, with said suspension immediately stayed in favor of probation, was levied a civil penalty $7,500,

and an assessment for the costs of investigation of $7,125.10 because he failed to conform to the accepted

practice as a medical physician and surgeon temporarily suspended, pending a hearing, on the grounds

Out of State Timothy Adam Drabick, PA license no. OH000275, of Burlington, New Jersey, was indefinitely

suspended for no less than 3 years, such suspension to be immediately stayed in favor of no less than 3

years of probation subject to the terms and conditions set forth by the Board, because he violated a lawful

order of the Board previously entered by the Board in a disciplinary proceeding, and cannot practice the

profession with reasonable skill and safety to patients by reason of illness, addiction to hallucinogenic,

Mario Angelo Criscito, PA license no. MD033201L, of Livingston, New Jersey, was actively suspended

for a minimum period of one year and until he applies for and receives approval from the Board for

reinstatement to active status, subject to the terms and conditions set forth by the Board, because he had a

license or other authorization to practice the profession suspended, revoked, refused or otherwise

disciplined by the proper licensing authority of another state, territory, possession or county, or a branch

William Louis Manion, PA license no. MD029222E, of Medford, New Jersey, was indefinitely and

actively suspended to practice as a medical physician and surgeon in the Commonwealth of Pennsylvania,

because he had a license or other authorization to practice the profession disciplined by the proper

Surender Gorukanti, PA license no. MD440125, of Brooklyn, New York, was suspended for two (2)

years, with such suspension to be immediately stayed in favor of probation subject to the Board's terms

and conditions; and he was ordered to pay a civil penalty in the amount of $5,000, because he pleaded

Navigation Commission for the Delaware River and its Navigable Tributaries No disciplinary actions taken.

Secretary of the Commonwealth – Notaries Public

Allegheny County Frank R. Jones, notary commission no. 1232698, of Kennedy Township, Allegheny County, was

suspended for six months, with such suspension immediately stayed in favor of no less than six months of

probation, ordered to take six hours of notary courses on notary practice and procedure, and ordered to

pay a civil penalty of $250.00, and costs of monitoring of $300, because he failed to be familiar with the

duties and responsibilities of a notary public by notarizing post-dated documents, failed to have a party to

the document date the form, and charging a fee in excess of the fees set by the Acting Secretary of the

Commonwealth, failed to be of good character, integrity, and ability by notarizing post-dated documents,

failing to have a party to the document date the form, and charging a fee in excess of the fees set by the

Secretary of the Commonwealth, and failed to maintain an accurate notary journal by failing to record in

his notary journal the date and time of the notarial act, the full name and address of each individual for

whom the notarial act is effect, a statement to the effect that the identity of the person is based on personal

knowledge, and a brief method of identification and any identification credential presented, including the

date of issuance and expiration of an identification credential if the identity of the individual is based on

satisfactory evidence. (05/31/18)

Bucks County Gregory V. Abalov, notary commission no. 1243283, of Holland, Bucks County, permanently and

voluntarily surrendered his notary commission, because he submitted notary public applications

containing material misstatements, including a fraudulent social security number on two (2) of his notary

public reappointment applications and checked “no” to the question of entering a guilty plea to a felony or

lesser included offenses on three (3) of his notary public applications, failed to be of good moral character

to be a notary public by submitting applications that contained material misstatements of fact, and failed

to be of good moral character by pleading guilty to a felony and lesser offenses incompatible with the

duties of a notary public during the five (5) year period prior to submitting his June 18, 2008 application

for appointment as a notary public. (06/01/18)

Arnold Ross Roher, notary commission no. 1139979, of Quakertown, Bucks County, agreed to the

suspension of his notary commission for three months, such suspension immediately stayed in favor of

probation, to successfully complete a six-hour notary practice and procedure course, and to pay a civil

penalty in the amount of $500 and probation assessment fees in the amount of $150 because he failed to

maintain an accurate chronological notary register by failing to record the character of the act for some

notarizations and failing to list each notarization separately by combining the notary fees of multiple

notarizations; failed to be familiar with the duties and responsibilities of a notary public by failing to

include the essential notarization language on the two documents and failing to maintain an accurate

chronological notary register; and failed to be of good character, integrity and ability, by failing to include

the essential notarization language on the two documents and failing to maintain an accurate

chronological notary register. (06/21/18)

Lancaster County Randy B. Tiedemann, notary commission no. 1166873, of Strasburg, Lancaster County, permanently and

voluntarily surrendered his notary commission because he entered upon the duties of the office of notary

public after he failed to take and subscribe the constitutional oath of office, obtain a surety bond, and

record the bond, commission, and oath of office in the office of the recorder of deeds for the county in

which Respondent maintained an office at reappointment; acted as a notary in transactions in which he

was a party directly or pecuniarily interested by notarizing vehicle titles of vehicles that his dealership

purchases and that his vehicle dealership sells through his salesperson; failed to maintain a notary register;

made a material misstatement or omission of fact on his applications in 2010 and 2014 by answering

“No” to the legal question; and failed to be familiar with the duties and responsibilities of a notary public

by acting as a notary public from 2010 to 2014 without completing the commissioning process, failing to

maintain a notary register, making a material misstatement or omission of fact on his applications in 2010

and 2014, and acting as a notary in transactions in which Respondent was a party directly or pecuniarily

PA Board of Nursing Allegheny County Samantha Nix, license no. PN289476, of Brackenridge, Allegheny County, was suspended for one year, a

minimum of three months to be actively served to run concurrent with a previous sanction, was required

to complete six hours of education in ethics and was assessed a civil penalty of $500, based on being

found guilty of a crime of moral turpitude and failing to report same to the Board. (12/15/17)

suspended for no less than 3 years, such suspension was immediately stayed in favor of no less than 3

years of probation, subject to the terms and conditions set forth by the Board, because she was addicted to

alcohol or hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination

or became mentally incompetent, and was convicted or plead guilty or entered a plea of nolo contendere

or was found guilty by a judge or jury of a felony or a crime of moral turpitude in the courts of this

Commonwealth, or received probation without verdict, disposition in lieu of trial or an Accelerated

Rehabilitative Disposition in the disposition of felony charges. (6/06/18)

Leslie Michelle Davis, license no. RN643012, of Pittsburgh, Allegheny County, voluntarily surrendered

her license, because she is unable to practice professional nursing with reasonable skill and safety to

patients by reason of mental or physical illness or condition or physiological or psychological dependence

upon alcohol, hallucinogenic, or narcotic drugs which tend to impair judgment or coordination. (6/06/18)

Beaver County Michael Joseph Basile, license no. RN567326, of Aliquippa, Beaver County, was ordered to voluntarily

surrender his license, because he is unable to practice professional nursing with reasonable skill and safety

Bedford County Jill Lynn Miller-Graham, PA license no. RN584719, last known of Fishertown, Bedford County, had her

practical nurse license revoked based on pleading guilty to crimes of moral turpitude. (01/31/18)

Berks County Jaclyn Proitte Ruyak, license no. PN262816, of Reading, Berks County, was indefinitely suspended for no

less than 90 days and then placed on probation through her criminal probation, based on her pleading

guilty to a misdemeanor. (03/22/17)

Alicia Lynn Schanely, license no. RN567928, of Oley, Berks County, was indefinitely suspended for 3

years, such suspension was immediately stayed in favor of 3 years of probation subject to the terms and

conditions set forth by the Board, because she was unable to practice nursing with reasonable skill and

safety to patients by reason of physical or mental illness or condition or physiological or psychological

dependence upon drugs or alcohol. (6/06/18)

Blair County Brent Tyler Jenkins, license no. RN590812, of Hollidaysburg, Blair County, will be granted reinstatement

then indefinitely suspended for no less than three years, such suspension to be immediately stayed in

favor of no less than three years of probation, said probation to be subject to the Board's terms and

conditions, because he is unable to practice professional nursing with reasonable skill and safety by

reason of mental or physical illness or condition or dependence upon alcohol, hallucinogenic or narcotic

Bucks County Christine D. Kline, license no. RN267503L, of Southampton, Bucks County, agreed to the voluntary

surrender of her license to practice as a registered nurse in the Commonwealth of Pennsylvania, because

she has violated a lawful disciplinary order of the Board in that she failed to reschedule and attend the

mental and physical examination due to serious illness. (06/06/18)

Carbon County Gregory J. Sabo, license no. RN574340, of Lansford, Carbon County, was indefinitely suspended for no

less than three years, such suspension to be immediately stayed in favor of no less than three years of

probation, said probation to be subject to the Board's terms and conditions, because he was unable to

practice professional nursing with reasonable skill and safety by reason of mental or physical illness or

condition or dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs which tend to

Chester County Tracyann McCarthy, license no. PN286898, of West Chester, Chester County, was indefinitely suspended

retroactive to July 17, 2017, and was assessed costs of investigation of $350 based on being addicted to

alcohol, hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination.

Clarion County Melissa M. Gumpher, license no. PN254096L, of Clarion, Clarion County, voluntarily surrendered her

license, because she was convicted or pleaded guilty or entered a plea of nolo contendere or was found

guilty by a judge or jury of a felony or a crime of moral turpitude in the courts of this Commonwealth,

failed to report the disposition of the criminal action to the PA Nurse Board, had her license suspended by

the proper licensing authority in the state of Florida, and failed to notify the Board of disciplinary action

in the nature of a final order taken against her within 90 days of receiving notice or on the biennial

Columbia County Elizabeth M. Kile, license no. PN087409L, of Bloomsburg, Columbia County, was assessed a $2000

civil penalty, $263.20 in costs of investigation fee and was reprimanded for practicing on a lapsed

Crawford County Amanda J. Rankin, license no. PN263368, of Meadville, Crawford County, was indefinitely suspended

until she complies with the order to undergo a mental and physical examination. (05/23/17)

Cumberland County Susan Orr Parker, license no. RN257595L, of Carlisle, Cumberland County, was indefinitely suspended

based upon being disciplined by another state and being unable to practice with reasonable skill and

safety to patients by reason of mental or physical illness. (09/25/17)

Brandi Jo Peffer, license no. PN275713, of Carlisle, Cumberland County, was indefinitely suspended

retroactive to January 25, 2016 for being addicted to alcohol or hallucinogenic, narcotic or other drugs

which tend to impair judgment or coordination. (11/02/17)

Jonathan Kwaku Setuagbe, license no. RN694604, of Camp Hill, Cumberland County, was ordered to pay

the costs of investigation of $1,048.08, attend and successfully complete at least 4 hours of remedial

education on the topics of Nursing Law and Ethics and Safe Medication Administration subject to the

terms and conditions of the Board and submit acceptable proof of successful completion of the courses to

the Board no later than August 31, 2018, and a public reprimand was placed on his permanent disciplinary

record with the Board, because he was guilty of unprofessional conduct by dispensing Zofran to a patient

without a prescriptive order, and misappropriated drugs from an employer or patient. (6/06/18)

Delaware County Demeetra A. Jackson, license no. PN294278, of Brookhaven, Delaware County, had her license

indefinitely suspended for no less than three years with such suspension to be immediately stayed in favor

of no less than three years of probation, because she is addicted to alcohol or hallucinogenic or narcotic

drugs or other drugs which tend to impair judgment or coordination or has become mentally incompetent.

Loriann Kralle, license no. PN275816, of Secane, Delaware County, was indefinitely suspended for no

less than three (3) years, such suspension to be immediately stayed in favor of no less than three (3) years

of probation, unless that period of probation is extended or modified for cause by mutual agreement of the

Respondent and the Professional Health Monitoring Program (“PHMP”), Disciplinary Monitoring Unit

(“DMU”) case manager subject to the terms and conditions of the Board, because she is addicted to

alcohol or hallucinogenic or narcotic drugs or other drugs which tend to impair judgment or coordination

or has become mentally incompetent; and she violated a lawful order of the Board by failing to comply

with the terms of Final Adjudication Order issued by the Board on November 9, 2015. (6/06/18)

Erie County Amanda Manno, license no. PN292710, of Erie, Erie County, was indefinitely suspended, retroactive to

March 14, 2016, for being addicted to alcohol or hallucinogenic, narcotic or other drugs which tend to

impair judgment or coordination. (09/27/17)

Sarah Joyce Young, license no. PN270591, of Erie, Erie County, was indefinitely suspended for no less

than three years, such suspension to be immediately stayed in favor of no less than three years of

probation, because she is addicted to alcohol or is addicted to hallucinogenic or narcotic drugs or other

drugs which tend to impair judgment or coordination or has become mentally incompetent, and has

violated a lawful disciplinary order of the Board. (6/06/18)

Jefferson County Brittany Sarah Lucas, license no. PN295566, of Brookville, Jefferson County, voluntarily surrendered her

license, because she was addicted to alcohol or was addicted to hallucinogenic or narcotic drugs or other

drugs which tend to impair judgment or coordination, so long as such dependence shall continue, or had

become mentally incompetent. (6/06/18)

Lackawanna County Kenneth A. Burton, license no. PN260584L, of Scranton, Lackawanna County, voluntarily surrendered

his license, because he was unable to practice practical nursing with reasonable skill and safety by reason

of mental or physical illness or condition or dependence upon alcohol, hallucinogenic or narcotic drugs or

other drugs which tend to impair judgment or coordination. (6/06/18)

Lancaster County Shane Matthew Laughman, license no. PN281128, of Mount Joy, Lancaster County, was indefinitely

suspended retroactive to August 30, 2017, and was assessed $350 in costs of investigation, by reason of

his addiction to alcohol or hallucinogenic, narcotic or other drugs which tend to impair judgment or

Faith Engel Cranford, license no. RN240999L, of Lancaster, Lancaster County, was ordered to pay a civil

penalty of $2,400.00 and a public reprimand placed on her permanent disciplinary record with the Board,

because she held herself out as a registered nurse without at the time of so doing a valid, unexpired,

unrevoked, and unsuspended license. (6/06/18)

Hong Van Tran-Speros, license no. RN634684, of Lancaster, Lancaster County, was suspended for no

less than three (3) years, with such suspension stayed in favor of no less than three (3) years of probation,

so that he can participate in the disciplinary monitoring unit, because he is unable to practice professional

nursing with reasonable skill and safety by reason of mental or physical illness or condition or

dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs which tend to impair judgment

or coordination and violated a lawful order of the Board. (6/06/18)

Lebanon County Sarah J. Mason, license numbers RN532424 and PN259118L, of Cleona, Lebanon County, had her

licenses suspended for no less than three years retroactive to July 31, 2017, based on findings that she

violated the terms of a consent agreement entered into with the Commonwealth. (10/18/17)
