Our environment has changed dramatically over the past few generations. Advances in medicine have provided new treatments. However, we live in an increasingly toxic world with potentially harmful substances that affect all of us, regardless of where we live. The interactions between these substances and human health are often poorly understood.

Low levels of exposure to substances can have negative biological effects over time and different tissues are more susceptible to harm than others. Obesity, ethnicity, socioeconomic factors, including levels of psychological stress and age, are factors that personalize and influence the impact of environmental toxicants.  

We are confronted on a daily basis with potentially toxic chemicals in the food we eat, the air we breathe, the liquids we drink, and the environment in which we live. Insecticides, pesticides, herbicides, some

industrial chemicals, and other substances remain in the environment—almost indefinitely in some cases—as persistent organic pollutants (POPs). Fragrances and disinfectants, for example, are absorbed through our skin; particulate matter from various sources including diesel exhaust and industrial emissions are absorbed more readily through our lungs. Both can cause chronic diseases.

Environmental toxicants contribute to the development of obesity, a worldwide concern. Toxicants accumulate in adipose tissue and interfere with some of its important signaling functions. This contributes to the development of metabolic syndrome, altered lipid metabolism and the risk of diabetes. The mitochondria are particularly sensitive to environmental toxicants and when impaired contribute to many different chronic diseases. There is evidence that the microbiome, which plays an important role in the biotransformation of toxicants, is impaired following exposure to certain environmental pollutants.

RECOGNIZING WHEN A PATIENT NEEDS DETOXIFICATION

HOW THE BODY NATURALLY DETOXIFIES

Scientific advancements in the technology of toxicity testing include the fields of toxicogenomics and exposome science. However, objective biomarkers that determine a person’s specific toxic load, as well as response to therapy, are still not well established. The use of measurement tools including genetic testing for single-nucleotide polymorphisms (SNPs) can be useful to determine whether a patient might benefit from detoxification and drainage. Detailed health histories (including potential exposure to environmental chemicals at home or work) as well as the recognition of common signs and symptoms of a higher toxic burden still remain useful as guides for diagnosis and ongoing clinical care.

Detoxification can be a therapeutic tool to prepare for other therapeutic interventions and potentially enhance their effectiveness. Some examples include:

1. Preparing for weight loss: supporting the organs of elimination as well as the body’s metabolic pathways can enhance the effectiveness of any weight loss program. In this case, it is important to consider that adipose tissue stores environmental toxicants (especially lipophilic ones), and therefore, the overweight patient might experience more severe initial reactions to detoxification. This type of patient needs to be treated more slowly and closely monitored.

2. Preparing for pregnancy: the presence of toxicants in pregnant women are known to influence fetal growth and development. Some toxicants may affect the quality of male sperm and impact outcomes of pregnancy. Fertility can be reduced in both women and men, and evidence suggests that a healthy pregnancy is more likely if both partners have a lower toxic burden.

3. Patients with many unexplained signs and symptoms: sometimes patients have a long list of unexplained signs and symptoms that make it difficult to determine the most appropriate treatment plan. A course of detoxification and drainage may help reveal the most helpful guiding symptoms for diagnosis and further treatment. (Note: it is important first to rule out any potentially serious disease (e.g., kidney disease, inflammatory bowel disease) before doing this.

4. Increasing overall wellness: the primary systems and organs of general drainage, detoxification, excretion and elimination are the lymphatic system, the liver, the digestive system and the kidneys. Additionally, a considerable amount of toxicant metabolites can be exhaled and/or removed via sweating through the skin. In general, the signs and symptoms of a higher burden of toxicity in patients that are otherwise healthy include: general feelings of malaise and feeling unwell, lethargy and low energy, unstable moods and irritability, skin rashes and/or acne and boils, bloating and gas, diarrhea and/or constipation, chronic low grade headache, lack of concentration and/or brain fog.

When this occurs, various changes can occur including:

1. Formation of advanced glycation end-products (AGEs), due to chronic exposure to excess sugar, can form cross-links that alter the structure of the ECM. AGEs may bind a specific receptor that triggers inflammation as well as interferes with different functions of the ECM, including its function as a molecular sieve. Moreover, an alteration in the types and sizes of ECM fragments can promote chronic inflammation and impair tissue repair.

2. Tissue acidosis (i.e. hyperacidity) affects the functioning of the hundreds of different enzymes involved in cellular metabolism as well as triggering inflammation and altering the microbiome.

3. Production of excess free radicals, which can damage cell membranes and the genetic material and interfere with cellular signaling.

4. Altered membrane potential changes (i.e. abnormal transfer of electrical charges across the cell membrane) can alter cellular pumps, enzymatic reactions, and generation of cellular energy.

In sum, persistent and/or substantial disturbances to the ECM negatively impact the functional units of organs. This can progress to the general signs and symptoms of chronic disease including muscle aches, fatigue, and exhaustion. This also may play a role in the development of pathologies associated with chronic disease including poor wound healing, diabetes, metabolic syndrome, obesity, asthma, hypertension, and cancer.

DETOXIFICATION HAPPENS ON A CELLULAR LEVEL: THE EXTRACELLULAR MATRIX

CELLULAR MATRIX DYSFUNCTION

Toxins and toxicants are usually either neutralized by the liver or excreted in the urine or feces. Lymphatic fluid transports these substances and their metabolites from the extracellular matrix (ECM) to the subclavian veins, arteries and finally to the liver for excretion and elimination by the gastrointestinal tract and urinary tract.

The ECM (also referred to as cellular microenvironment) was first described by Professor Alfred Pischinger in 1953 as the ground regulation system. He proposed that normal functioning of the various cells of the different bodily tissues and organs was dependent on support and signaling provided by the ECM surrounding them.

The ECM and the cell make up a functional unit, and therefore, it is impossible to achieve and maintain homeostasis without both parts of the functional unit being healthy. The ground regulation system exists throughout the body and provides another pathway for organ systems to communicate with each other as well as for environmental signals to influence gene expression and induce functional cellular changes.

Normal functions of the ECM are disrupted by environmental toxicants (including heavy metals, persistent organic pollutants, and other synthetic substances), persistent psychological stress, and dysbiosis.

Communication between cells and the ECM is dynamic on multiple levels, and was described as “Dynamic Reciprocity” by Dr. Mina J. Bissell in 1982; today, dynamic reciprocity is more often referred to as bi-directional communication.

The ECM is made up of the fluid, and many different types of fibers and other components arranged in mesh-like network outside of the cells. It is a major part of the cellular microenvironment which also includes input from nerve fibers, endocrine factors, and the microbiome.

Functions of the Extracellular Matrix

The ECM transmits communication between the cells and its surroundings. Substances and information are exchanged between somatic cells and nerves, blood or lymph, via signaling molecules that include ECM fragments, hormones, and neurotransmitters. The ECM helps regulate many different important physiological processes including inflammation, electrolyte balance, acid-base balance, cellular proliferation, differentiation and apoptosis, organogenesis and the microbiome. The ECM also acts as a selective molecular barrier, allowing only certain substances into cells, providing structural support, binding sites and acting as a reservoir for micronutrients.

Nearly 2000 liters of blood flow through the liver every day. Some of the liver’s most important tasks include producing and storing carbohydrates, synthesizing plasma proteins (e.g., albumin, fibrinogen, clotting factors) and hormones (e.g., insulin-like growth factor IGF-1, angiotensinogen, thrombopoietin), metabolizing fats (e.g., cholesterol, lipoproteins) and producing bile. It also helps regulate important minerals such as iron and copper and stores essential vitamins such as vitamin A, D, B12, and K.

The liver also is important for the immune system. It produces acute phase proteins (e.g., C-reactive protein), complement, and immunoglobulins. The liver plays a key role in the filtering, deactivation, breakdown, and excretion of endogenous and exogenous substances that could become toxic. This includes medications, environmental chemicals, and their byproducts, as well as substances normally found in our body but in specific forms or ratios (e.g., estradiol: estrone and its metabolites 2-OH estradiol and 4-OH-estradiol).

In an ideal situation, these substances are first activated and enzymatically modified and then conjugated and excreted via the kidneys or with bile in the intestines and the feces. If this does not occur properly, potentially harmful substances can accumulate and cause disease.

Hevert Detox Liver

Hevert Detox Liver is a homeopathic medicine containing Carduus marianus, Chelidonium majus, radix and Taraxacum officinale. This formula, derived from well-known medicinal plants, is designated to support the liver and gallbladder function. Thus Hevert Detox Liver relieves symptoms of indigestion, such as bloating, constipation or pain in the upper abdomen.

A deterioration in liver function affects the organs of digestion. The body reacts with a range of symptoms, such as abdominal pain, flatulence, abdominal distension, diarrhea or constipation. When one is feeling unwell, it can often be related to the function of the gastrointestinal tract and the excretory function of the hepatobiliary system. Gut-derived signaling molecules are produced by specific cells of the GIT (e.g., enterochromaffin cells and other enteroendocrine cells), by the gut microbiota, or by interactions between resident microbiota and host cells. There are so many different important signaling substances produced by the gut microbiota that they have recently been described as a neglected endocrine organ.

The Gut-Brain Axis

The gut and the brain from embryology share communication pathways via the nervous system (e.g., enteric nervous system and vagus nerve connection), the immune system (e.g.,different cytokines) and the circulatory system (e.g., endocrine factors). An abundance of research in neurology and gastroenterology indicates that hidden in the walls of the digestive system, is a “brain in your gut”. This information is “revolutionizing medicine’s understanding of the links between digestion, mood, health and even the way you think.  Scientists call this little brain the enteric nervous system (ENS). And it’s not so little. The ENS is two thin layers of more than 100 million nerve cells lining your gastrointestinal tract from esophagus to rectum,” which are more neurons than there are in the spinal cord.

The main role of the ENS is to integrate local and central signals mediating the function of the gastrointestinal tract. This includes controlling many digestive processes “from swallowing to the release of enzymes that breakdown food to the control of blood flow that helps with nutrient absorption to elimination.” It also helps mediate local immune processes by a process termed neurogenic inflammation.

More recent discoveries indicate that the ENS has effects beyond the GIT including blanketing “the body with signals regarding metabolic processes to the brain, liver, and likely skeletal muscle and fat”. Jay Pasricha, Director of the Johns Hopkins Center for Neurogastroenterology and a well-known researcher on the ENS, states that because of this widespread effect of the ENS, “not surprisingly, its influence extends to disease conditions such as insulin resistance and type II diabetes thus our focus on the pancreas”. Even further, increasing evidence suggests the “ENS might play a major role in the occurrence of anxiety and depression” and other common mood disorders, adding to the increasing body of evidence that gut-brain communication works both ways.

The Gut Microbiota

The gastrointestinal tract is home for numerous beneficial commensal bacteria which aid digestion, support the immune system and play a role in metabolizing drugs and other environmental toxicants into less harmful byproducts. Scientific evidence also indicates gut microbiota play critical roles in regulating the local and central functions of the gut-brain axis and therefore, the most up-to-date and accurate term for this axis is the Gut-Microbiota-Brain axis. • After processing by the liver, many metabolic wastes and potentially toxic byproducts of environmental chemicals are excreted into the bile, stored in the gallbladder, and then secreted into the small intestine. Some of these substances are recycled via the hepatic portal system (i.e. enterohepatic circulation), but many of them are excreted out of the body through the large intestine and the feces. Maintaining regular bowel movements is an important way to ensure that these potentially toxic substances are passed out of the body before they can cause harm.

• The intestinal barrier provides protection between the external environment and the ‘interior milieu’. If there is leakiness in the intestinal barrier (i.e. a leaky gut), more toxins can be released into the body as they move through the intestines. During detoxification in the liver, lipid-soluble substances are modified and activated and made more water-soluble so they can be excreted via the bile into the intestines or removed via the kidneys. Maintaining a slightly alkaline pH of the urine helps with the excretion of toxic metabolites, suggesting alkalinizing foods and therapies should be included as part of a detoxification program. Furthermore, renal excretion and the flushing out of water soluble wastes works best when the glomerular filtration rate is high enough. If the volume of urine is too low, the concentration of different mineral salts and uric acid could increase to the point where kidney stones can form, or uric acid crystals could lodge in the joints and cause gout.

Sufficient cardiac output is needed to maintain adequate perfusion through the kidneys and therefore, is necessary for proper renal excretion. It is also important to have a large enough volume of fluid plasma. This can be maintained by a fluid intake of at least 0.4 to 0.5 US gallons (1.5 to 2 liters) of pure water per day. Finally, the heart can function less efficiently as we age, reducing renal perfusion and glomerular filtration rates. This suggests cardiac function should be supported in older patients.

DOSAGE & ADMINISTRATION

For Adults and Children Over 12 Years

• Take 20 drops in water 3 times a day

Duration of Detoxification Regimen

A normal regimen with all the four Detox Kit bottles will last between 2 and 3 weeks (depending of the drop count accuracy). This is the minimal time frame for maintenance of health.

For patients that have never done this type of detoxification you might need to recommend a second Detox Kit, with one week break between the two, to assess results.

In healthy patients, one Detox Kit twice a year is enough.

In patients with functional disorders and other conditions that are not yet too advanced but are persistent, two kits, twice a year might be necessary.

In patients with severe and advanced disease states, Detox Kit is not recommended.
