Chatfield Independent Senior Living – Director, Resident Services/Concierge Supervisor

Director, Resident Services – Provide support and resources for residents and their families in the areas of physical/emotional/spiritual welfare and cognitive health. Liaison for residents/discharge planners/nurses when residents are hospitalized or discharged to a rehabilitation facility in order to provide a safe discharge; promote Brookdale Health Services and services provided by Personalized Living (PL), Brookdale’s non-medical home care, staffed by CNAs who are Brookdale associates.

o Perform assessments to determine appropriateness of prospective residents for independent living

o Meet with prospective residents during visits as part of the marketing process

o Responsible for coordinating with all departments and facilitating bi-weekly Collaborative Care meetings; maintain online program containing data on each resident, identifying physical, emotional and cognitive changes

o Input information from Internal Communication Reports (ICRs) into BAIRS (Brookdale online reporting tool)

o Advocate for residents with questions/requests by following up with appropriate department or locating resources (within Brookdale and available public programs) to ensure needs are fulfilled. Partner with PL Manager to meet with residents and family members as necessary to modify current assistance in place, if necessary, or identify opportunities to provide assistance for new residents as indicated by their assessment.

o Organize a yearly remembrance service for residents and associates to fulfill spiritual needs, a flu clinic for physical needs and a charitable drive for school or other supplies to provide a sense of purpose. Various event organization is ongoing to meet these and other needs such as intellectual, emotional, and physical.

o Manager on Duty on weekdays, weekends or holidays as scheduled.

o Assist Executive Director with resident council meetings, family/associate issues, and special projects and assignments as requested.

o Assist other departments as needed.

Director, Concierge – Ensure that procedures and followed and staffing for the 24/7 operation are fulfilled to meet the needs of the community.

Coordinated care plans based on professionals’ recommendation and clients’ wishes balanced with realistic expectations. Quarterly follow-up ensured care plan was current and adjusted as necessary depending upon observation of client and communication with treating professionals and family feedback.

Interviewed, hired and trained support staff in all aspects of office procedure including Timematters ®, (proprietary database management system for the legal profession). Compiled procedures manual for training and business continuity.

Promoted to Supervisor within four months of employment; supervised four Regional Program Coordinators to ensure departmental goals for each region were accomplished.

Provided information, resources and advice to dementia caregivers and professionals through the Alzheimer’s Association Helpline, a 24/7, 365/day per year information and referral phone service.

Coordinated and administrated the CT Chapter MedicAlert+Safe Return® Program, a national identification program that provides support and assistance when an individual with dementia becomes lost locally or far from home.

Developed and presented primer on critical long-term care insurance issues at Vanderbilt University for university employees. Presentation reestablished working relationship between the university and MetLife agents, leading to resumption of sales.

Quoted in several nationwide publications; provided on-air radio interviews.
