Low HIV-1 Resistance in Subjects Using Darunavir Once-daily Regimens Across Studies
E Lathouwers et al▪ Sponsored phase 2/3 studies investigating DRV 800mg QD-containing
ARV regimens with available genotypes/ phenotypes were included in this analysis
▪ Within the 7 studies, a total of 2,328 subjects wereenrolled; 1,686 subjects were treated with DRVQD–based regimens and, among these, 184 hadPDVF and 182 had post-baseline genotypesanalyzed (Table 5)
▪ Overall, 4/1,686 (0.2%) subjects developedtreatment-emergent (or had on-treatment [switchstudies]) primary PI and/or DRV RAMs
– Only 1 (<0.1%) subject lost phenotypicsusceptibility to DRV; this was possibly relatedto a prior VF with lopinavir + rtv (all of thedeveloping primary PI RAMs and most of thesecondary PI RAMs were also lopinavir RAMs)
▪ Among subjects treated with DRV monotherapy +rtv, only 1/264 (0.4%) had an on-treatmentDRV RAM; this RAM (L33F) was detected at Week12 (VL of 63 copies/mL) and the subject wasresuppressed at subsequent visits until Week 144
▪ Among all subjects who used an N(t)RTI backbone,10/1,103 (0.9%) developed treatment-emergent N(t)RTI RAMs to their backgroundregimen (including 8 subjects with the FTC RAMM184I/V)
– Nine of these subjects lost phenotypic susceptibility to≥1 N(t)RTI in their background regimen

Estudio abierto, aleatorizado, controlado, en 9 centros en Francia:
* CV VIH<50 c/mL durante >12 meses, no SIDA (excepto TB), nadir CD4+ >100/mm3, no mutaciones o fracas a un INSTI.

5 pacientes adicionales tuvieron fracaso virológico entre la semana 24 y 48 (5/70; 7%), con aparición de resistencias en dos de ellos. Siguiendo recomendaciones del DSMB, se suspendió el brazo de monoterapia y todos los pacientes fueron re-intensificados (8 pacientes no habían llegado a la semana 48). El FV se asoció a bajos CD4+ en la inclusión y presencia de “señal” de CV (detectable pero no cuantificable en la inclusión, 25% de casos).
