PRESENTAZIONE

Dall’idea alla sperimentazione, dalla sperimentazione alla realizzazione, dalla rea-lizzazione all’applicazione clinica, dalla diagnostica alla terapia chirurgica, dall’i-niziale supporto tecnologico alle tecnologie avanzate; le esperienze, i risultati, leindicazioni, le controindicazioni, i costi ed i benefici: è questa la storia, lo stato at-tuale e le prospettive della Chirurgia Toracica Videoassistita, descritta da C. Lavi-ni, C. Ruggiero e U. Morandi in modo completo e aggiornato in questo testo mo-nografico ottimamente illustrato, che sarà di sicuro interesse non solo sul pianodella clinica ma anche per la sua valenza scientifica.

I Curatori, forti della lunga e approfondita esperienza clinica svolta presso lascuola di Chirurgia Toracica dell’Università di Modena, avvalendosi della loro at-titudine alla speculazione scientifica e a dimostrazione del loro spiccato senso cri-tico, oltre a contribuire personalmente all’opera, si avvalgono anche della collabo-razione di esperti italiani e stranieri che hanno maturato analoghe esperienze, ideeed opinioni nello specifico campo della videotoracoscopia chirurgica diagnosticae terapeutica.

Da Jacobaeus alla chirurgia robotica: è questo lo schema basilare dell’opera cheè stata concepita in più capitoli. Nella parte generale iniziale vengono esposte leapparecchiature, lo strumentario, i principi e le tecniche di base, l’anestesia, manon viene trascurata la descrizione delle strutture anatomiche intratoraciche amezzo dell’osservazione videotoracoscopica. Attuale e utile è il capitolo sugliaspetti medico-legali.

Nella parte speciale, un ampio spazio viene riservato alla più frequente patolo-gia pleuro-polmonare sia per quanto riguarda la diagnostica che per quanto con-cerne l’aspetto terapeutico, che richiede interventi chirurgici più o meno estesi. Ilvalore della metodica viene dimostrato sia agli effetti della diagnosi che della sta-diazione per neoplasie maligne del polmone e della pleura.

Le lesioni non neoplastiche come i versamenti pleurici benigni, l’empiema, l’en-fisema idiopatico e l’enfisema bolloso vengono trattate con eguale attenzione.

Nella parte dedicata al pneumotorace, per il cui trattamento viene rimarcatol’importante ruolo della chirurgia videotoracoscopica, viene sottolineata anchel’importanza dell’impiego complementare delle sostanze pleuroadesive la cui uti-lità è suffragata da interessanti contributi scientifici.

Un altro capitolo riguarda l’impiego della videotoracoscopia relativamente alladiagnosi, anche con fini stadiativi e al trattamento delle lesioni neoplastiche beni-gne e maligne del mediastino; particolare attenzione è riservata alle indicazioni ealle tecniche di accesso alla loggia timica.

La trattazione si arricchisce con la descrizione di tecniche che trovano un’indi-cazione meno frequente e nel trattamento di varie patologie: la simpaticectomia,la splancnicectomia, la vagotomia tronculare, la chirurgia del dotto toracico, le viedi accesso al pericardio, il trattamento di lesioni del diaframma e traumatiche deltorace e della colonna vertebrale. Il volume tratta anche le indicazioni e l’impiegodella metodica in chirurgia pediatrica, e si completa con la descrizione delle tecni-che di chirurgia robotica e con l’analisi dell’impiego attuale della chirurgia video-toracoscopica e delle prospettive in un prossimo futuro.

PREFAZIONE

Sono passati quasi cento anni da quando Hans Christian Jacobaeus nel 1913 effet-tuò il primo intervento di pleurolisi con un rudimentale toracoscopio.

Probabilmente lo stesso pneumologo svedese non ebbe mai la percezione com-pleta di avere aperto la strada ad una tecnica estremamente innovativa che avrebbeavuto negli anni successivi una importante valenza diagnostica e che soprattuttoavrebbe consentito la nascita della chirurgia toracica videotoracoscopica.

L’introduzione delle tecniche videoendoscopiche ha indubbiamente rivoluziona-to la Chirurgia Toracica in questi ultimi 15 anni ed ha reso più agevole, rapido, si-curo l’approccio a diverse patologie del torace; per alcune di queste l’accesso vi-deoassistito rappresenta ormai incontestabilmente il gold standard, per altre non c’èancora uniformità di consensi.

L’utilizzo della videocamera può essere effettuato tanto nell’ambito di un inter-vento totalmente videoendoscopico quanto in assistenza ad un approccio “open”;questo impiego “flessibile” della videocamera ha permesso l’introduzione di tecni-che diverse, alternative, ed altrettanto risolutive.

Il termine di Chirurgia Toracica Videoassistita comprende tanto le metodichecompletamente videoendoscopiche quanto quelle con accesso “open” videoassi-stito: il titolo dato al nostro lavoro vuole pertanto sottolineare l’intento di com-prendere tutto quanto in Chirurgia Toracica si avvalga del supporto video in sen-so lato.

Il volume si compone di 43 Capitoli che trattano le tecniche relative alla patolo-gia della parete toracica, della pleura, del polmone, del mediastino, dell’esofago, delpericardio, del diaframma; due capitoli sono dedicati alla Chirurgia pediatrica edalla Chirurgia vertebrale toracoscopiche.

Il corpus della monografia ci sembra completo ed esaustivo per quanto con-cerne le tematiche e gli argomenti trattati. Non mancano capitoli di grande inte-resse, tutti accompagnati da un’iconografia originale e ricca, quali la patologianeoplastica ed infettiva mediastinica, le resezioni polmonari maggiori, le re-sezioni esofagee. Un opportuno risalto viene dato anche all’esplorazione vi-deoassistita del mediastino (videomediastinoscopia cervicale-VAM e videome-diastinoscopia anteriore) e del pericardio (VPC), come pure alla videotoraco-scopia esplorativa (VTE) come indagine preliminare del tempo exeretico incorso di cancro polmonare. Vengono pure compresi nella trattazione temi digrande impatto e già proiettati nel futuro come la Chirurgia Assistita dal Com-puter (CAS), rappresentata dall’Augmented Reality, dalla Chirurgia Robotica,dalla Telechirurgia.

Hanno collaborato al volume Autori italiani, europei, americani, asiatici, austra-liani, tutti estremamente competenti nell’argomento specifico della loro trattazione;inoltre, l’opera rappresenta un’opportuna messa a punto delle tecniche videoassisti-te in Chirurgia Toracica se consideriamo che l’ultimo importante lavoro monogra-fico italiano scritto sull’argomento risale al 1999.

Per garantire un approccio multimediale integrato al volume abbiamo pensato dicorredare la maggior parte dei capitoli con filmati prodotti dai rispettivi Autori: neltesto il riferimento ad un video specifico viene contrassegnato da un’icona rappre-sentante due dischi sovrapposti.
