    The value of cerebrospinal fluid (CSF) lactate andlactate dehydrogenase (LD) activity in the rapiddiagnosis of meningitis was investigated in threegroups of patients - a 'no meningitis', an asepticmeningitis and a bacterial meningitis group. Thesensitivity achieved in the detection of bacterialmeningitis by CSF lactate values of 2,85 mmol/l(93,8%) and 3,9 mmol/l (89,6%) was greater than thatreached by conventional chemical investigationsusing a CSF protein value of 1 g/l (81,5%) or a CSFglucose value of 2,2 mmol/l (68,80/0) as the indicator.The sensitivity of an absolute CSF LD value of 40U/I (86,3%) in the detection of bacterial meningitiswas slightly lower than that of a CSF protein value of1 g/l (87%) and better than the sensitivity of either aCSF1serum LD ratio of. 0,1 : 1,0 (83,9%) or a CSFglucose level of 2,2 mmol/l (76,3%). As with con-ventional CSF chemistry, both investigations maygive normal values in the presence of bacterialmeningitis.

    The cerebrospinal fluid (CSF) lactate concentration has beenknown to be raised in bacterial meningitis for more than 50years. I,2 After a long period of neglect the last decade has seenconsiderable interest in CSF lactate as an aid in rapidlydifferentiating viral from bacterial meningitis. )-5 While someauthors doubt whether it has 'any significant role in themanagement of children with suspected meningitis',6 othershave claimed it to be 'on admission .... the most sensitive andthe most efficient test to distinguish bacterial meningitis fromaseptic meningitis'. 7

    Similarly, CSF lactate dehydrogenase (LD) activity is knownto be increased in cases of bacterial meningitis,S-lo and it hasbeen suggested that determination of CSF LD activity 'maybe superior to that of lactate, both in the diagnosis of bacterialmeningitis and in distinguishing it from aseptic meningitis'. 11

    Experience with these two investigations as aids in the rapiddiagnosis of meningitis is presented.

    Patients and methods

    CSF lactate and LD values were determined in three groups ofpatients, all of whom were children 13 years of age or youngerwith the exception of a small number of adults suffering fromtuberculous meningitis (TBM). All CSF was collected for normalclinical indications. This investigation was approved by the EthicalComminee of the Medical Faculty of the University of Stellen-bosch.

    Group 1 - 'no meningitis'. These children were investigatedfor possible meningitis, but subsequently shown to be free frommeningitis. CSF lactate levels were determined in 46 patients(median age 5,13 months) and CSF LD activity in 237 patients(median age 13,6 months). In 160 cases serum LD activity wasdetermined simultaneously with the CSF LD activity.

    Group 2 - aseptic meningitis. These patients did not receiveantibiotic therapy before or after investigation. Their CSF con-tained more than 20 x 106/1 white cells, no organisms could beseen on microscopy after Gram staining and no bacterial organismswere cultured from the CSF or blood. CSF lactate was determinedin 23 patients (median age 60,07 months) and CSF LD activity in73 patients (median age 55,4 months). Simultaneous serum LDactivity was determined in 30 of these laner cases.

    Group 3 - bacterial meningitis. The diagnosis of bacterialmeningitis was confirmed in these patients by culture of therelevant organism from the CSF. The CSF specimens evaluatedwere those drawn before starting therapy. CSF lactate was deter-mined in 48 cases (Neisseria meningicidis, 33 cases; Haemophilusinfluenzae, 4 cases; Screpcococcus pneumoniae, 4 cases; Escherichiacoli and Salmonella group B, 1 case each; and Mycobacceriumcuberculosis, 5 cases). The median age of these patients was 17,26months (age not available in 2 patients). Eleven of these patientswho presented with a CSF cell count of less than 250 x 106/1 havealready been reported on in more detail. I2 CSF LD activity wasdetermined in 95 cases (N. meningicidis, 55 cases; Screpl.pneumoniae, II cases; H. influenzae, 9 cases; E. coli, 4 cases; groupBB-haemolytic streptococcus, 3 cases; Proceus mirabilis andSalmonella group B, 1 case each; and Myco. cuberculosis, II cases).Three of the TBM patients were adults and the median age of theremaining paediatric patients was 14,20 months. In 31 cases theserum LD activity was determined simultaneously. Four of theTBM patients in whom CSF lactate was determined and 9 inwhom CSF LD activity was determined have been reported onpreviousIy. 13

    CSF was collected in tubes containing Long's solution l4 andlactate values were determined with an enzyme assay kit. CSF andserum LD activity were determined by an optimized standardmethod. CSF showing evidence of a traumatic tap and bloodshowing haemolysis were not analysed for LD activity.

    The CSF lactate values obtained in the three study groups areillustrated in Fig. 1. Values suggested in the literature abovewhich a diagnosis of bacterial meningitis should be consideredhave been indicated at 2,75 mmoVI and 3,85 mmoV1.4,5 The singlemarkedly elevated level seen in the 'no meningitis' group wasobtained in a 2-month-old infant who had experienced repeatedconvulsions. In Table I the sensitivity and specificity achieved by

    these values in detecting bacterial meningitis are compared withthose associated with the conventional criteria of a CSF proteinlevel of I g/I,15 and an absolute CSF glucose level of 2,2 mmol/l.'6

    In this series the sensitivity of CSF lactate in detecting bacterialmeningitis was bener than that of either CSF protein or CSFglucose, irrespective of which CSF lactate value was used. Inrespect of specificity the higher CSF lactate value of 3,9 mmol/Igave results only slightly bener than CSF protein or CSF glucose.

    CSFLDThe CSF LD values obtained in the three groups are illustrated

    in Fig. 2. A CSF LD activity of 40 U/I is indicated as the upperlimit of normal. 17 In Fig. 3 the CSF/serum LD ratio is indicatedfor those cases in which serum LD values were available. In TableII the sensitivity and specificity of an upper limit of normal forCSF LD activity of 40 V/I and a ratio of 0,1: 1,0 in the detectionof bacterial meningitis are compared with those of CSF proteinand glucose values obtained in these same patients.

    The CSF protein and the absolute CSF LD achieved a verysimilar sensitivity in the detection of bacterial meningitis - 87%and 86,3% respectively. The greatest specificity was found withCSF glucose (99%).

    In group I 48 children were aged less than 3 months. Themedian CSF LD value in this age group was 27,5 U/l comparedwith 10,0 U/I in the children older than 3 months (P = 0,0000).


    This experience wiJh measurements of CSF lactate confirmsits sensitivity in detecting bacterial meningitis. Whicheverlevel was selected as a decision point, its sensitivity was benerthan that of conventional CSF chemical tests.

    In contrast, CSF LD activity was neither more sensitive normore specific in detecting bacterial meningitis than theconventional CSF chemical criteria. Determination of theCSF/serum LD ratio, while improving the specificity of thetest, did not improve its sensitivity. The tendency for higherCSF LD values to be seen in the neonatal period has beennoted before and must be borne in mind when interpretingresults.

    It has been previously noted that the CSF LD response inTBM is subdued in comparison with that seen in purulent

    Recognition of these facts places the emphasis in doubtfulcases upon careful repeated clinical evaluation of the patient ,followed by repe
