ABSTRACTAt KenSci we have developed machine learning applications for better care

management and efficient operations across the care continuum. We transitioned

years of research at the University of Washington, done in partnership with

various health systems and Microsoft Research, into actionable cost saving

solutions to make healthcare more affordable and clinical outcomes more

actionable with insights from data. We are on a mission to enable widespread social change.

In this talk, we present a practical machine learning approach for chronic care

management and will discuss why it nicely augments care decisions. We will also

demonstrate a few Kensci solutions for cost and risk stratification.

BIOAnkur M. Teredesai, Ph.D., is the co-founder and Chief Technology Officer of KenSci.

He also holds a Professorship in Computer Science; Systems at the University of Washington.

His research spans data science with its applications for societal impact in healthcare.

Apart from his academic appointments at RIT and the University of Washington,

Teredesai has significant industry experience, having held various positions at

C-DAC Pune, Microsoft Research, IBM T.J. Watson Labs, and a variety of technology startups.

He has published over 75 papers on machine learning, has managed large teams of

data scientists and engineers, and has deployed data science solutions in healthcare.

His recent applied research contributions include cost and risk prediction for

readmission due to chronic conditions such as congestive heart failure.

Other applications of his work have enabled predicting lengths of stay and sepsis,

as well as predicting medication pathways to lower risks of mortality and rehospitalization.

He is the Executive Director of Center for Data Science, and serves as the Information Officer

for ACM SIGKDD (Special Interest Group in Knowledge Discovery and Data Mining),

the leading organization of industry and academic researchers in data science.

He is currently an associate editor for ACM SIGKDD Explorations and IEEE Transactions on

Big Data and serves on program committees of major international conferences

in machine learning and healthcare.

Dr. Josh Luke is a best-selling author, hospital CEO, healthcare futurist, thought-leader and international

motivational speaker. He is an experienced hospital CEO, health system Vice President and nursing home

administrator. He is regarded as a "futurist" on the Affordable Care Act and how it will shape the continuum of care

and has been described as an as an innovator, forward-thinking and a strategist on teaching ACO’s, BPCI’s, acute

hospitals and post-acute leaders how to position themselves for revenue growth in a post-ACA model.

He currently serves as Chief Strategy Officer for Nelson Hardiman Healthcare Law and its subsidiary consulting firm,

Compliagent. Having uniquely held executive positions in both acute and post-acute settings, Dr. Luke was selected

to author the book Readmission Prevention: Solutions Across the Provider Continuum published by the American

College of Healthcare Executives, the best-selling book from Health Administration Press in 2015.

In 2016, Xlibris publishing released Luke’s first book written for the mainstream public, Ex-Acute: A former hospital

CEO tells all on what’s wrong with American healthcare, What every American needs to know. In 2016 Luke is

conducting an international book promotion tour

In 2013, Luke was named Vice President of Post Acute Services for Torrance Memorial Health System. In that role he

designed a population management strategy (Total Wellness Torrance), working with the ACO, Bundled Payment &

IPA teams. TWT and its Post Acute Network received the 2013 Excellence in Programming award from CAHF. His

broad range of prior experience with some of the leading companies in healthcare, positioned him well to be an

expert on readmission prevention and care coordination. He served as the CEO of multiple acute hospitals in

California, as CEO of HealthSouth Las Vegas (acute rehab hospital), Administrator in the Skilled Nursing Division of

Kindred, and had direct oversight of home health and hospice services in his health system Vice president role.

Tracy Wilson is currently the Chief Executive Officer at Kindred Rehabilitation Hospital Northeast Houston. As a

Speech-Language Pathologist, she has 22 years of post-acute experience with the past 13 years in leadership roles.

Tracy began her career in Oklahoma as a therapist in SNF, IRF, HH and LTACH settings. In 2011, she moved to Houston,

TX with her husband and three children to pursue an area rehab operations position for the Kindred Houston LTACH and

inpatient rehab market. In 2014, Tracy joined the Kindred Clinical Performance team which worked to improve quality

metrics and adherence to regulatory compliance for acute hospital-based IRF’s across the nation. In September, 2016,

she became the CEO of a free-standing rehabilitation hospital near her home in Kingwood, TX. This experience has

allowed her to gain a better understanding of the local challenges and opportunities for post-acute services and their

vital role in preventing readmissions to the acute care hospital. Tracy has a B.S. of Communication disorders degree

and M.S. in Speech Language Pathology from University of Oklahoma. Tracy enjoys traveling, particularly to Disney

Syed Raza, MD

Vice President Medical Operations

Syed Raza received his BA in Biology degree at the University of Texas in Austin

and attended the Baylor College of Medicine for his internship and residency. He

started his internal medicine/hospitalist career at St. Luke’s Episcopal Hospital

in 1999 where he helped initiate that facility’s chest pain program and was the

number 1 admitter for chest pain, COPD, and heart failure patients by volume.

She is a registered nurse and a licensed attorney in the state of Texas and has served as an expert witness on issues of

nursing practice and nursing management. Ms. Roux has also served as adjunct professor at Texas Woman’s University

Houston for the MBA program teaching health care law and health care leadership classes. She has published several

articles related to legal issues affecting healthcare, and has served as the coordinating author for the legal checkpoints

column for Nursing Management magazine. She is a Certified Emergency Nurse by the Board of Certification for

Emergency Nursing, certified as a Nurse Executive, Advanced by the American Nurse Credentialing Center Commission

on Certification and a Fellow of the American College of Healthcare Executives. Ms. Roux is also a Pathway to

Excellence Reviewer for the American Nurse Credentialing Center.

In 2006, he started a private practice hospitalist group in the Woodlands, TX. He admitted 15 primary care

physicians and since then, the practice has expanded rapidly. They now admit for 90+ physicians and attend at 2

acute care hospital, 2 surgical hospitals, and several LTACS/rehab facilities. There are currently 24 physicians in

the two hospitalist groups. In 2013, he became medical director for St. Luke’s Hospital in the Woodlands. After

St. Luke’s became part of CHI, Syed was named VP of Medical Operations, a position he currently holds.

Sherri Welch currently serves as Chief Operating Officer at College Station Medical Center. She has 24 years of extensive

medical background including nursing positions in emergency and trauma medicine, intravenous therapy, home health

and worker’s compensation medical case management. She has also managed an urgent care center, and an occupational

medicine center. She joined The Med in 2001 as the Associate Director of Patient Care Services. She received her BSN

from Prairie View A&M College of Nursing and in August, 2006, became Chief Nursing Officer after graduating with her

MHA from the Texas A&M Health Science Center’s School of Public Health. Sherri became a Fellow in the American

College of Healthcare Executives in 2013.

She is a Community Advisory Board member of Blinn College School of Nursing and Texas A&M University Health Science

Center School of Nursing. She serves on the board of Texas A&M School of Public Health Alumni and Texas A&M School of

Public Health MHA Professional Advisory Committee. She was named 2017 Distinguished Alumni. She also serves on the

board Ronald McDonald House Charities Brazos County. Her volunteer activities include the Bryan/College Station

Chamber of Commerce, March of Dimes and the American College of Healthcare Executives. She is a member of the

Texas Organization of Nursing Executives, the American College of Healthcare Executives and Sigma Theta Tau.
